Book excerpt: Les Accidents Vasculaires Cérébraux Ischémiques (AI) restent un problème de santé publique, avec une incidence forte dans la population en âge de travailler. Environ un tiers des AI du sujet jeune restent de cause indéterminée, rendant la prévention secondaire inadaptée. La consommation de toxiques pourrait être la cause de ces AI. L'objectif de notre étude épidémiologique à inclusion prospective sur 2 ans et analyse rétrospective chez les AI de moins de 55 ans était de déterminer la prévalence de la consommation de toxiques et les caractéristiques démographiques et cliniques des patients consommateurs. Nous avons montré une prévalence de 13,8% de la consommation de toxiques chez les AI de moins de 55 ans, avec un lien inverse avec l'âge, statistiquement plus élevée chez les moins de 45 ans. Le premier toxique retrouvé est le cannabis (11,9%), le deuxième la cocaïne (5,9% des 18-35 ans), semblant sur-représentés par rapport à la population générale. La moitié des patients testés positifs aux opiacés prenait des traitements opiacés connus, mais aucun patient ne consommait d'amphétamine. L'interrogatoire sur la prise de toxiques manquait le plus souvent, en lien avec l'âge. Les consommateurs de toxiques étaient plus jeunes, tous tabagiques, sans différence pour les facteurs de risque habituels, le type ou le territoire des AI, le délai au dépistage urinaire ou le taux de thrombolyse. Il semble important de réaliser systématiquement un interrogatoire sur la prise de toxiques chez les AI jusqu'à 55 ans, d'améliorer les délais de dépistage urinaire et de réserver ces dépistages aux AI de moins de 45 ans, sans recherche d'opiacés.

Book excerpt: Objectifs : caractériser cette population sur le plan médical et social, déterminer les facteurs prédictifs de décès, le pronostic fonctionnel et les lieux de vie à court terme et à 6 mois. Méthodes : étude prospective sur 67 patients hospitalisés au centre hospitalier de Valenciennes. Résultats : L’âge moyen était de 85.1 ans avec une nette prépondérance féminine. Le facteur de risque le plus souvent retrouvé était l’HTA (75 %). Plus les patients étaient âgés, plus l’état clinique initial était grave et les complications fréquentes. Les patients les plus âgés bénéficiaient moins des examens complémentaires et des progrès thérapeutiques, n’avaient pas accès à la rééducation. L’étude retrouvait 16 % de décès à la phase aiguë et 33 % de décès à 6 mois. Les facteurs de risque significatifs de décès à la phase aiguë étaient la gravité clinique initiale (NIH>10) et l’existence de complications. A 6 mois, l’âge supérieur à 90 ans devenait facteur de risque de décès. 45 % des patients vivant initialement au domicile y vivaient toujours 6 mois après l’AVC. Concernant la dépendance fonctionnelle à 6 mois, le seul facteur de risque significatif était l’existence d’une démence préalable. Conclusion : Le pronostic vital et fonctionnel de ces patients n’était pas aussi défavorable que ce qui est communément admis. L’âge ne doit pas un critère limitant la prise en charge. Des outils d’aide à la décision pourraient guider cette prise en charge.

This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.
