Book excerpt: The sibling relationship provides a unique context for the development of interpersonal skills and lays the groundwork for extensive social situations with other children. Siblings of children with autism spectrum disorders (ASDs) experience multiple stressors and increased caregiver demands that children with neurotypical siblings do not face. While some siblings do not experience significant adjustment problems, it appears that other children may have greater vulnerability to the development of significant adjustment problems and internalizing disorders. Very few studies have investigated the effect of a support group for children who have a sibling with ASD. The present study evaluated the effects of Project SibSTAR (Straight Talk about Autism Realities), an eight week support group that incorporated didactics, discussion, and role plays while using rigorous research design and psychometrically-sound measures. Twenty siblings of children with ASD were randomized to either the active or delayed intervention group. While the groups received the same intervention, the time-lagged design allowed for the assessment of a multiple baseline design across both groups and skills. In addition, participants reported on measures of psychological and social functioning at baseline, post group, and 6 week follow-up assessments. During these assessment sessions, data were also collected on play-based interactions within the sibling dyad. Outcomes were assessed across a variety of domains, including intervention acceptability, concepts taught during group, play-based behavioral principles, and self-report of internalizing symptoms, self-competence, and perceived social support. Results indicated that participants increased their knowledge of autism, coping and problem solving skills, and their knowledge of behavioral skills. Further, trend level decreases in internalizing symptoms were found from baseline to post group assessment, and significant differences were found on measures of perceived social support and social self-competence. While participants indicated high satisfaction with the intervention package, no significant differences were noted on play-based interactions with their sibling. The implications and future directions of this line of research are discussed.

Studies have suggested that the quantity and quality of interaction between the pair is significantly poorer relative to sibling dyads affected by other developmental disabilities. However, research has shown that siblings can act as effective interventionists for their sibling with an Autism Spectrum Disorder. The present study assessed the efficacy of a home-based treatment program to teach siblings to use three sets of behavioral skills while playing with their brother or sister with autism. In a multiple baseline design across skills, three sibling dyads were trained to a) elicit play and play related speech, b) to deliver reinforcement and c) to prompt the child with autism following an incorrect or non-response. Siblings were also given a target word for each session which they attempted to teach. As evidenced in the completer dyad, siblings successfully acquired these behavioral skills, they maintained over time, and generalized to untrained contexts. Siblings with autism showed increases in responding to and initiating play-based interactions, and one of the children spontaneously verbalized target words. Siblings found the treatment to be acceptable, and parents indicated satisfaction with the procedures. These findings support the hypothesis that siblings can utilize behavioral skills to act as effective interventionists in a play setting with their brother or sister with autism.

A specific deficit in joint attention has been identified in this population; it appears to be universal and pivotal to the development of more complex social skills and language. Behavioral interventions targeting joint attention are evidenced to be effective in teaching these skills to young children with autism, but these treatments have traditionally been implemented by adults. In the present study three typically developing children were trained to implement a joint attention intervention to their siblings with autism. Gains in responding to joint attention were observed for all three targets; gains in initiations were observed in two targets. These differential results provide information about the merits of conceptualizing joint attention as a set of specific skills rather than an individual construct. Siblings found the treatment to be acceptable, and parent ratings indicated high satisfaction with the procedures. The implications of these findings for treatments targeting joint attention and for siblings as interventionists are discussed.

Book excerpt: Because children with Autism Spectrum Disorders (ASD) have difficulty with social interactions, they may not regularly engage in play activities with their siblings. Using an alternating treatment design, this study examined the efficacy of video-modeling (VM) and video self-modeling (VSM) to improve social initiations and duration of play between three children diagnosed with ASD, ages 6-7 years old, and their siblings. Dependent variables were (a) latency to initiate interaction; and (b) duration of time engaged together in play. In-home sessions of video instruction taught children to approach their sibling and say, “Lets play.” Preference assessments identified toys used during intervention. Generalization exposed participants to novel toys without viewing a video model. Baseline results indicated two children did not initiate play at baseline nor did they engage in play with their sibling. One child initiated play in three baseline sessions but did not remain engaged throughout the entire session. After exposure to the intervention, all children more quickly initiated social play across VM, VSM, and Generalization. Upon acquiring this pivotal social skill, children diagnosed with ASD maintained social engagement with siblings across a variety of play activities. This research demonstrated that VM and VSM were equally effective in training the target skills. Keywords: autism, video modeling, play engagement, sibling.
