Book excerpt: Feeding development begins during the embryologic and fetal periods with maturation of the head, face, and neck, the emergence of early oral motor reflexes, and continues well into early childhood. Children learn to eat in a predictable sequence. They transition from nutritional intake of a single consistency (liquid) to complex, multi-textured foods in just two short years. Children continue to refine their feeding skills through elementary age. Simultaneous to feeding development during childhood is speech and language development. One component of speech and language development is articulation. Articulation refers to the establishment of clear and distinct sounds in speech. Speech sounds are developing from birth and should be fully developed by age eight all the while, gaining clarity with repetition and feedback. Articulation and feeding both require intact orofacial structure and adequate oral motor function. Although the relationship between these two developmental processes is implied because of their shared developmental periods and anatomical structures, it has not been fully explored in the literature. This study investigated the relationship between feeding and articulation in children with known articulation deficits. A total of ten participants were recruited from the University of Alabama Speech and Hearing Center (UA SHC), but only three were included after exclusion. Participants demonstrated overlapping sound errors that corresponded to overlapping feeding skill error. For clinical purposes, the implication of an articulation and/or feeding screener would be beneficial in the evaluation process of either disorder to aid in the success of the child's therapy.

Book excerpt: There appear to be few data regarding the development of nonlinguistic oral-motor skills in children with normal articulation. Such data are necessary for a better understanding of the age at which children with disordered articulation should acquire certain nonlinguistic movements. The goal of the present study, therefore, is to provide preliminary developmental norms for assessing the structure and function, using nonlinguistic tasks, of a child's developing oral-motor system. This was accomplished by investigating the following research questions: Is there a significant difference in individual oral-motor skills as age increases? Is there a significant difference between ages in overall oral-motor performance? Is there a significant interaction between age and lip, mandible, and tongue development? Thirty-six children between the ages of 4 years and 5 years, six months, from Boise, Idaho, were included in this study. All subjects had normal hearing and articulation and had not received any special services. All subjects completed the oral-motor assessment protocol to test the functioning of the lips, tongue, and jaw. The subjects' responses were scored on a scale of 0 to 7. The results of the study indicated that a signficant interaction of age and structure reflected development of oral-motor skills as age increases for the tongue and mandible but not for the lip. Post hoc comparisons indicated that the labial movements appear to be the most developed, the mandibular movements appear to be the next to develop, and the lingual movements appear to be the slowest to develop. The results of this study are the first stop in a large undertaking to generate developmental norms for non-linguistic speech movements. Additional validity and reliability studies need to be conducted on the protocol designed for this study to document its efficacy. Further studies on a larger, more diverse sample of participants need to be conducted. These studies should focus on the potential difference in maturation of nonlinguistic speech movements between males and females, as well as the implications of these findings.

The Supplemental Security Income (SSI) program for children provides financial assistance to children from low-income, resource-limited families who are determined to have conditions that meet the disability standard required under law. Between 2000 and 2010, there was an unprecedented rise in the number of applications and the number of children found to meet the disability criteria. The factors that contribute to these changes are a primary focus of this report. Speech and Language Disorders in Children provides an overview of the current status of the diagnosis and treatment of speech and language disorders and levels of impairment in the U.S. population under age 18. This study identifies past and current trends in the prevalence and persistence of speech disorders and language disorders for the general U.S. population under age 18 and compares those trends to trends in the SSI childhood disability population.
