This study compared individual psychotherapy patients with unmanaged insurance (n = 26), managed insurance without concurrent review (n = 24), and managed insurance with concurrent review (n = 210) on rates of improvement in psychological status and on initial patient ratings of the therapeutic bond. Investigators collected background information and Mental Health Indexes (MHI, Howard, Davidson, et al., 1988; Howard, Brill, et al., 1993) from all patients at the initiation of treatment. MHIs were subsequently administered to patients at regular intervals over the course of treatment to track improvement. Therapeutic Bond Scales (Saunders, Howard, & Orlinsky, 1989) were obtained from patients around session five. No group differences were found in rates of patient improvement. Patients with concurrent review rated their therapy relationships more highly than did others, but this finding is likely due to measurement bias. Despite the null findings of the current project, continued research on the clinical impact of managed care is necessary to inform mental health policy and practice.
