Book excerpt: Cross-cultural studies of the original MMPI have indicated that Hispanics may obtain elevated profiles in comparison to Whites, especially when clinical samples are compared. With the MMPI-2, there persists a controversy over Hispanic-White profile differences and the degree to which Hispanic acculturation influences MMFI-2 scores. This study compared White- and Mexican-Americans from outpatient psychiatric, college, and community samples on the MMPI-2. Mexican-American subjects were also administered the Acculturation Rating Scale for Mexican-Americans (ARSMA). Psychiatric patients (diagnosed with schizophrenia or schizoaffective disorder) also completed the Brief Symptom Inventory (BSI). A significant relationship between Mexican-Americans' ARSMA scores and MMPI-2 scale scores was found only in the psychiatric sample, with less acculturated patients tending to score higher on the MMPI-2 No overall ethnic differences were observed in the psychiatric patients' profiles. However, statistically significant and clinically meaningful (e.g., greater than 5 T-score points) ethnic differences were found, with White patients (White men in particular) scoring higher on the Psychasthenia, Schizophrenia, and Social Introversion Scales. Comparison of the combined college and community samples found both ethnic groups scored well within normal limits and indicated statistically significant overall profile differences between Mexican-Americans and Whites. Univariate analyses of the MMPI-2 Validity and Clinical scales revealed that the apparent profile differences were attributable to the tendency of Mexican-American women to score significantly higher on the Lie scale than other groups. No other clinically meaningful ethnic differences were observed. In summary, MMPI-2 comparisons of Mexican-Americans and Whites from college and community samples of similar ages and educational backgrounds indicated small mean differences which were isolated on specific scales, and were not always clinically meaningful. Although Mexican-American acculturation was found to be a significant moderator of Mexican-American patients' MMP-2 performance, White patients scored higher on the MMPI-2. However, considering the small number of patients included in this study and the inclusion of only schizophrenic spectrum patients, this finding is considered tentative and not generalizable to other diagnostic groups.

Book excerpt: ABSTRACT: This study evaluated ethnic differences on the Minnesota Multiphasic Personality Inventory-2 (MMPI-2) in 126 Caucasian and 106 Native Hawaiian and Other Pacific Islander (NHOPI) mental health center outpatients. MMPI-2 Restructured Clinical (RC) scale scores were first evaluated to determine the extent or influence that the demographic variables of race, gender, age, and education have on identified group MMPI-2 RC scale differences. When assessing for racial group mean differences, five scales studied were identified to be of significance. When assessing for age mean differences, two scales were observed to have been of significance, although there was no significant interaction between race and age for either of these scales. No differences in gender were observed and there were no gender by race interactions. The findings suggest that gender and age had less effect on how a participant would respond to MMPI-2 questions than race. Because preliminary analyses identified a racial group difference by education, this variable was controlled for through a MANCOVA. With the exception of RC scale RC8, racial group differences were not confounded by education. These results were generally consistent with the observed racial group differences on five of the MMPI-2 RC scales and the available extratest criterion data. To assess for test bias, a step-down hierarchical multiple regression procedure was used in which relevant clinical criteria were predicted from the RC scales with race as a potential moderator. Of the 19 scale criterion comparisons, nine demonstrated statistically significant intercept bias of which five achieved a small effect size. Two scales from the regression analyses revealed statistically significant slope bias, both of which demonstrated a small effect size. Although several of the MMPI-2 RC scales suggested some overprediction and underprediction of psychopathology in Native Hawaiian and Other Pacific Islanders, it was determined that racial group membership was not a substantive moderator between MMPI-2 scales scores and clinically relevant phenomena because the effect size of race on prediction was uniformly small. It was recommended that the MMPI-2 RC scales not be interpreted differently for Caucasian and Native Hawaiian and Other Pacific Islanders, as the magnitude of the predictor error did not warrant separate interpretive guidelines.

As such, a pertinent question is whether the MMPI-2 is an appropriate tool for assessing personality for this population. Nichols (2011) stated that the practicing clinicians who participated in the study failed to use research on cultural influences that may elevate the clinical and validity scale on the MMPI-2 with a specific ethnic population. In addition, Butcher and Williams (1999) stated that the MMPI-2 did not include an adequate normative representation of Native Americans. Furthermore, Butcher and Williams (1999) also stated that most studies about the MMPI-2 and Native Americans have shown numerous methodological flaws. Despite these flaws, proponents of the MMPI-2 continued to maintain that the use of the instrument is culturally valid, and it continues to be the most used personality assessment instrument with Native Americans (Butcher, 2009; Butcher & Williams, 1999). The focus of this qualitative study was to provide a detailed view of six clinicians' experiences in administering the MMPI-2 with Native Americans and their use of cultural information to interpret the profile. Using thematic analysis, the results yielded seven themes that were categorized in three distinct domains: A) trauma informs the responses to MMPI-2, B) awareness of clients' cultural belief system by the clinicians, and C) clinicians' awareness of their own cultural competence and sensitivity.
