Book excerpt: Nous avons réalisé une étude rétrospective sur 5 ans concernant 290 adolescents hospitalisés pour tentative de suicide (TS) dans le service de pédiatrie du Centre Hospitalier Général de Chambéry. Ce travail avait pour but de faire le point d'une part sur la prise en charge des adolescents suicidants à l'hôpital général et d'autre part sur la place actuelle des services de pédiatrie générale par rapport à l'évolution des pathologies de l'adolescent. Notre population, à prépondérance féminine avait une moyenne d'âge de 15 ans et 7 mois. Les TS, dont 90 % étaient des intoxications médicamenteuses volontaires, étaient survenues fréquemment dans le contexte d'un environnement familial défavorable, de difficultés scolaires, et de perturbations psychologiques. 31 %étaient des récidivistes. Surconsommation de soins, décès d'un des deux parents, psychopathologie chez un parent, antécédent personnel de TS ou antécédent familial de TS ou suicide étaient des facteurs de risque majeur de TS. L'hospitalisation frôlait les 100 % avec une durée moyenne de séjour de 5 jours. La prise en charge était multidisciplinaire avec une équipe formée à l'approche des adolescents La proximité des différents intervenants permettait une continuité des soins de l'entrée à la sortie du patient, avec un suivi initial réalisé principalement dans le service. Le suivi à plus long terme était principalement proposé au Centre de Santé pour Adolescents, situé au sein du Centre Hospitalier. Nous avons retrouvé un faible taux de récidive (10 %) sur 5 ans des adolescents hospitalisés. Il faut offrir aux adolescents suicidants une hospitalisation de courte durée dans des services appropriés et accessibles avec du personnel motivé et formé. Les services de pédiatrie générale nous paraissent un lieu d'accueil intéressant pour ces adolescents. Une orientation vers un service spécialisé sera réalisé en cas de troubles psychiatriques graves mais ces cas s'avèrent peu nombreux.

Ce travail a pour objectif de décrire le profil des adolescents accueillis en hôpital de jour psychiatrique, leur inscription dans les soins et leur devenir médico-social à moyen terme, à partir d'une cohorte de 100 patients. Cette étude repose sur un travail rétrospectif, basé sur une relecture des dossiers, et catamnestique, à partir d'une enquête téléphonique basée sur un questionnaire semi-structuré. Les données rétrospectives font ressortir l'hétérogénéité de la population accueillie, tant au niveau de la symptomatologie présentée et des diagnostics posés, qu'au niveau de la trajectoire médicale et sociale antérieure. La prise en charge est marquée par une difficulté d'inscription aux soins pour 53% de la population. Le taux de perdus de vue s'élève à 11%. Les données catamnestiques dessinent 3 profils évolutifs d'un point de vue clinique: 29% évoluent défavorablement vers l'instabilité avec ou sans soins, dont 3 décédés ; 34% évoluent vers une stabilité qui reste tributaire des soins ; 26% évoluent favorablement vers une stabilité sans soin. L'analyse statistique des données à partir du critère de l'inscription aux soins fait apparaitre deux populations distinctes, avec deux comportements différents face aux soins. Les troubles de l'adolescence qui échappent aux prises en charges institutionnelles proposées constituent un groupe à risque. Leur prise en charge et leur orientation impliquent une vigilance importante, avec une pratique de réseau et une vision à long terme des difficultés.

Nous discuterons l'intérêt de l'hospitalisation en milieu adulte, souvent préconisée par défaut en l'absence de disponibilité de structures spécifiques en urgence. La mise en place du projet de soins individualisé du jeune, en concertation avec son milieu (familial ou éducatif) apparaît alors essentielle afin de maintenir une continuité de la prise en charge, favoriser la rémission et l'insertion sociale et scolaire ou professionnelle.

Beaucoup d'études n'adoptent pas une approche globale des processus d'adolescence, mais davantage une approche centrée sur une modalité comportementale donnée. Par ailleurs, les changements culturels, la multiplication des écrans, et l'avènement des réseaux sociaux nécessitent de se réinterroger régulièrement sur l'état de santé mentale des adolescents. L'objectif de ce travail est de caractériser en terme clinique une population d'adolescents pris en charge par le dispositif soins adolescents de l'hôpital Valvert sur une période donnée. Méthode : une étude épidémiologique transversale monocentrique dont l'objectif principal est descriptif, regroupant des informations concernant l'ensemble des nouveaux patients pris en charge par le service entre le premier janvier deux mille dix-neuf et le trente et un décembre deux mille dix-neuf. Nous nous sommes secondairement intéressés aux adolescents pris en charge par les services de la protection de l'enfance. Des données anonymisées sur les éléments cliniques, ainsi que démographique et sociaux ont été recueillies dans les dossiers médicaux des patients. Résultats : au total 138 patients ont été inclus dans l'étude. Le diagnostic le plus représenté est le trouble de la personnalité border line avec une prévalence de 13, 93%. Vient ensuite le trouble anxiété généralisée de l'enfance avec une prévalence de 13, 11%. La proportion de patient souffrant de schizophrénie est fréquente avec une prévalence de 8,20%. L'état de stress post traumatique représente 6, 56% des diagnostics retrouvés au sein de cette population. Les troubles du comportement alimentaire englobant anorexie mentale et boulimie sont moins fréquent (4, 10%)Le trouble du déficit de l'attention avec hyperactivité est moins représenté avec une prévalence de 3,28 %. Les addictions concernent 19,40% des adolescents inclus. Concernant les tentatives de suicide, 28, 7 % ont fait une tentative de suicide au moment de la prise en charge en 2019. Il y a 45, 11% d'absentéisme scolaire.52 adolescents soit 39, 39% des adolescents ont fait l'objet d'un signalement. Le nombre d'hospitalisation en pédiatrie s'élève à 129 sur 138 adolescents soit 93% des adolescents. Seuls 12,69% sont hospitalisés en pédopsychiatrie. Les prévalences retrouvées sont plus importantes qu'en population générale d'un facteur allant de 3 à 17 selon les pathologies. Conclusion : outre son intérêt épidémiologique, cette étude souligne les particularités de cette population en termes de santé mentale, à un moment donné de sa prise en charge. Une étude similaire sur l'année 2020 serait intéressante en terme de comparaison, afin d'étudier l'évolution de l'état de santé mentale d'une population d'adolescent en ayant pris en compte le contexte épidémique (COVID9).

Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
