Book excerpt: Helieobaeter pylori has recently been recognized as a new genus according to specific taxonomic criteria; the "popular" name Campylobaeter pylori has been corrected by scientific progress. Following the discovery of the spiral microorgan ism in gastric mucosa by Marshall and Warren in 1982, it took only a few years for H. pylori to become established as a factor in the pathogenesis of gastritis and peptic ulcer disease. Interest in different aspects of H. pylori has grown continuously and has attracted scientists from various medical and biological disciplines such as gastroenterology, microbiology, pathology, immunology, and pharmacology. Indeed H. pylori provides an excellent model for interdisciplinary interaction and cooperation. To promote this concept of interdisciplinary research and exchange of knowledge, a European Campylobacter (Helicobacter) Pylori Study Group was founded in 1987 in Copenhagen. The second meeting of this expanding group was held from October 12-14,1989 in Ulm, FRG. The fact that more than 500 participants attended the conference and that 187 original contributions from all five continents were presented clearly confirmed that H. pylori has "scientifically infected" the whole world. Our understanding of the microbiological and pathogenetic aspects of H. pylori is continuously being challenged as new results follow swifthy from different research areas. This book includes an update and progress report on the various aspects of H. pylori presented and discussed in special workshops held during the meeting in Ulm.

Book excerpt: The discovery of proton pump inhibitors (PPIs) and their development over the years has dramatically changed the management of acid-related diseases. Today, the therapeutic domain of PPIs ranges from relief of symptoms to cure of mucosal lesions in the upper gastrointestinal tract. PPIs are among the most widely sold drugs in the world and are now even available as over-the-counter medication. This publication presents the experience of the last 25 years during which PPIs have become of enormous value in gastroenterology. The authors provide an update on a variety of subjects, starting with an introduction to the discovery and development of PPIs. This is followed by chapters on pharmacokinetics, pharmacodynamics and pharmacogenetics, gastroesophageal reflux disease, gastroprotection, Helicobacter pylori eradication treatment, peptic ulcer disease, functional dyspepsia, acid suppression in exocrine pancreatic insufficiency, and gastrointestinal and systemic side effects. Readers who are interested in a current overview of PPIs and their various applications will find this book of great value.

Available in PDF, EPUB and Kindle. Book excerpt: Gastric cancer has been one of the great malignant scourges affecting man kind for as long as medical records have been kept. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.
