Evidence-based practices for pressure ulcer prevention are numerous and easy to find, yet implementation by nursing staff is lagging. When patients develop pressure ulcers, the wound team may be consulted to advise on treatment options. In this study, the wound team implemented methods to prevent pressure ulcers. This research project was conducted in a 96-bed long-term care facility. The project focused on the effectiveness of a wound team in implementing evidence based practices in a long-term care facility and reducing the incidence of pressure ulcers in that facility. This program utilized a multidisciplinary approach to patient care that could easily be transferred to other long-term care facilities. Over a 3-month period the incidence of facility-acquired pressure ulcers was reduced by more than 25%.
