Evidence-based practices for pressure ulcer prevention are numerous and easy to find, yet implementation by nursing staff is lagging. When patients develop pressure ulcers, the wound team may be consulted to advise on treatment options. In this study, the wound team implemented methods to prevent pressure ulcers. This research project was conducted in a 96-bed long-term care facility. The project focused on the effectiveness of a wound team in implementing evidence based practices in a long-term care facility and reducing the incidence of pressure ulcers in that facility. This program utilized a multidisciplinary approach to patient care that could easily be transferred to other long-term care facilities. Over a 3-month period the incidence of facility-acquired pressure ulcers was reduced by more than 25%.

As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

Data were collected from multiple review studies and results from these studies indicated that the implementation of multidisciplinary program as explained above will reduce pressure ulcers. In addition to the multidisciplinary approach, the enforcement of Braden scale which is widely in use in the United States of America, and other assessments tools such as Waterlow Score, and Norton Scale will be strictly enforced at time of admission into the unit. The expected outcome of this program, therefore, will reduce pressure ulcers in Geriatric community, restore quality of life for patients through on-going awareness, staff education, and positive feedback from patients' satisfaction survey at time of discharges (Schoelles and Sullivan, 2013).

A multidisciplinary approach is used to plan, disseminate and evaluate the proposal through meaningful contributions made by stakeholders.

Abstract: Background Pressure ulcer prevention has been an important and a common health care problem in every health care setting. Pressure ulcers are the clinical situation that can be avoided. To prevent pressure ulcer, the main strategy is to reduce either the pressure contact time, or magnitude of pressure, between a patient and his or her support surface. Pressure-relieving support surfaces are shown to be effective in preventing the development of pressure ulcer with strong evidence support. In Hong Kong, the low compliance of the ward staff to apply pressure-relieving support surfaces for at risk patients is compared with other countries. Therefore, a feasible evidence-based guideline of using pressure-relieving support surfaces to prevent pressure ulcers stands out from the multi-interventions pressure ulcer prevention guideline to maximize its effectiveness in this clinical issue. Purpose Reviewing the needs in modifying the current clinical practice and guidelines in local setting, the aim of the dissertation is to develop evidence-based guidelines for prevention of pressure ulcers using pressure‐relieving support surface for bedbound elderly. The findings are used to develop recommendations for nursing guideline to prevent pressure ulcers through the utilization of pressure‐relieving support surface in Hong Kong public hospital setting. Methods Four electronic databases published from 1937 to 2012 were searched. All the studies extracted were randomised controlled trials while focusing on using pressure-relieving support surfaces to prevent pressure ulcers of adult patients. The quality of each study was assessed by a methodology checklist, the Scottish Intercollegiate Guideline Network in order to justify the level of evidence of the studies' internal validity. Conclusion From the evidence, the clearest conclusion that can be drawn is that a range of pressure‐relieving mattresses and overlays in the prevention of pressure ulcers can outperform the standard hospital mattress. It can be applied to any health care settings. The reviewed studies have shown that pressure‐relieving mattresses are effective on at risk patients, especially they are in advanced age with low mobility. Therefore, this evidence-based guidelines will serve in the medical rehabilitation wards to reduce the number of elderly from developing pressure ulcer after using the pressure‐relieving supporting surfaces. The direct care costs associated with pressure ulcers in LTCF exceed $3.5 billion, while costs related to legal liability are even more substantial. Litigations in LTCF are likely to favor LTCF residents in up to 87% of cases while prevention of pressure ulcers offers high-quality, cost-effective outcomes. A systematic review of healthcare evidence reveals that while the development of pressure ulcers can have a devastating impact of LTCF populations, simple EBP steps such as using supporting surfaces, repositioning the patient, optimizing nutritional status, and moisturizing sacral skin can have a positive impact on reducing physical and emotional pain and associated healthcare costs. Purpose and Scope: The purpose of this Evidence-Based Practice (EBP) project was to describe the level of healthcare knowledge regarding pressue ulcer prevention (PUP) before and after participation in a PUP teaching intervention for LTCF staff. Using descriptive-interventional methodology, the study variable of nursing knowledge regarding pressure ulcer prevention was measured before and after participation in the PUP teaching intervention by the Pressure Ulcer Knowledge Tool. The instrumentation included a 47-item questionnaire with 3 subscales assessing knowledge regarding ulcers, wounds, and prevention/risk. Goal and Objectives: The goal of the project was to successfully implement a PUP teaching intervention to LTCF staff. The objectives were: (a) to describe aggregate demographic characteristics of the LTCF participants, (b) to describe PUP pre-intervention level of knowledge, (c) to describe PUP post-intervention level of knowledge, and (d) to describe PUP level of knowledge before and after LTCF staff participation in a PUP teaching intervention. Plan and Evaluation: Phase One: Conduct a literary needs assessment using a published evidence-based critique template. Phase Two: Design a PUP teaching intervention using the EBP approach. Phase Three: Implement a PUP teaching intervention for selected LTCF staff. Phase Four: Evaluate the level of PUP knowledge before and after LTCF staff participation in the PUP teaching intervention. Ouotcomes and Results: Pre-intervention and post-intervention meaures revealed a statistically significant increase in PUP knowledge from an aggregate percentage performance of .81 at pre-test to an aggregate percentage performance of .98 at post-test. Moreover, scaled reliability estimates based on internal consistency for the full instrument achieved significance with regard to the tool's performance in this interventional design. Although further inquiry is warranted, the findings suggest that LTCF staff can increase PUP knowledge with focused educational intervention regarding the concept, and that PUP education in LTCF may serve as a practice change outcome worthy of further consideration.

Never less, non-adherence to prescribed guidelines are often reported, and data obtained from National Nursing-Sensitive Indicators shows that at-risk patients continue to develop hospital acquired pressure ulcers (HAPU) at an unacceptable rate. The Aims of this Capstone project are to initiate a pressure ulcer education and prevention program in a medium-sized, community health care organization, and to change the attitudes and knowledge base of nurses by providing nursing staff with evidence-based knowledge, education, and skills regarding pressure ulcer development and prevention. This project consists of educating nurses on pressure ulcer pathophysiology and the importance of using early evidence-based pressure ulcer prevention education and techniques. Implementation of this project requires approval from key stakeholders. Applying Kurt Lewin's theory of change first requires the recognition that change needs to occur (Cresia and Friburg, 2011). Implementation consists of scheduled staff in-services, monthly meetings, and use of multi-technique pressure ulcer prevention products, risk-assessment, and screening forms. Data collection of pressure ulcer prevalence, for three months prior to implementation and for 3 months post-implementation, will be collected to evaluate the effectiveness of the program. Evaluation of the program will also consist of an evaluation of nursing staff's pre and post-implementation pressure ulcer prevention knowledge and skill. Dissemination of evidence to key stakeholders, health care organizations and to the greater nursing community will occur with the aim to increase knowledge of evidence-based pressure ulcer prevention techniques that will contribute to improved patient outcomes. This project will demonstrate how pressure ulcer prevalence rates, in the hospital-setting can be decreased through the consistent and knowledgeable use of evidence-based pressure ulcer prevention guidelines. PICOT Question: For the at-risk adult hospitalized patient (P), will the implementation of early, evidence-based pressure ulcer prevention interventions by nurses, who are educated in skills and knowledge regarding pressure ulcer development and prevention (I), as compared to those not educated, nor using early evidence-based pressure ulcer prevention interventions (C), decrease the probability of hospital acquired pressure ulcer development (O), in a three-month period (T).

Available in PDF, EPUB and Kindle. Book excerpt: Pressure ulcers (PU) are defined as injury to the skin and underlying tissue due to prolonged compression (Baumgarten and others, 2012). Nurses are responsible for completing the necessary tasks to prevent PU (Adejumo and Ingwa, 2010), such as early intervention, re-positioning, and risk assessment (Elliot, 2010). The PU Prevention Program is based on recent evidence-based research on how to change practice to reduce the chance of PU development. This program is proposed to change the practice of nurses, nursing aids and physical therapists in order to promote patient care. Since PU are debilitating to patients and can reduce their quality of life (Gorecki and others, 2009), PU development is important to the nursing field. This program will begin with education (Appendix A) to all staff involved before promoting practice changes. Education will include the use of an educational handout, a video, and 0−́hands on0+́ education. Practice changes will include frequent patient position changes, keeping skin clean and dry, and the use of skin care products such as barrier creams.To evaluate the progress of the program, an evaluation will be conducted. This will include a comparison of pre-program PU occcurance to post-program PU occurrence and a survey (Appendix B) on the opinion of the staff of the new program. After the evaluation is complete, the data collected will be presented to key stakeholders of local facilities and nursing communities for dissemination. From there, the other facilities can adopt this plan and change their current practice to fit the PU Prevention Program. Pressure Ulcers (PU) are considered a major problem in health care and can create further complications for the patient (Moore, Cowman, and Conroy, 2011). Though many make attempts to prevent PUs, they are still found in abundance, about 10% of patients in the hospital (Gorecki and others, 2009). This essay will further discuss PUs, their impact, and possible solutions to avoid this problem.
