Book excerpt: Background: Type 2 diabetes (T2D) is the leading cause of death in the U.S., and it incurs tremendous health expenditures associated with various complications due to poor diabetes control. Medication adherence, which is correlated with patients' health literacy, should be consistently practiced in order to achieve optimal diabetes control. However, no literature has systematically explored the multiple psychosocial and communication barriers and facilitators at the individual level that mediate the linkage between health literacy and medication adherence. A deeper understanding of the specific communication and psychosocial factors related to medication-taking behaviors across different levels of health literacy among people with T2D will guide the development of effective interventions and customized strategies to enhance medication adherence. Objectives: (1) To examine whether the barriers and facilitators associated with medication adherence differ among people with T2D across different levels of health literacy. (2) To explore patients' perceptions of the barriers and facilitators of medication adherence across different levels of health literacy. (3) To understand how the qualitative interview data complement the quantitative survey results about the barriers and facilitators of medication adherence. Methods: This mixed-methods study used an explanatory sequential design, including a quantitative study with a survey questionnaire followed by a qualitative study with semi-structured interviews. The Health Literacy Pathway Model was used to identify the psychosocial and communication factors that may influence medication adherence. The target population was patients who were 20 years of age or above, were diagnosed with T2D, understood English, and were prescribed at least one oral diabetes medicine daily. Analysis of covariance and direct content analysis were used respectively in quantitative and qualitative analysis. A joint display was used to present the integrated findings from quantitative and qualitative data. Results: In the quantitative phase, 205 participants provided complete information in the survey questionnaire. In the qualitative phase, 23 participants completed semi-structured interviews. Confirmed by quantitative and qualitative data, holding stronger self-efficacy, having fewer concern beliefs about medication, and possessing fewer perceived barriers to medication-taking are necessary for better medication adherence. Linking medication-taking to daily routine and focusing on the internal locus of control is an imperative approach to foster self-efficacy of medication use. Addressing patients' life experience and clarifying medication misinformation may help participants cope with their concerns with medications. Solving the high cost of medications and therapy-related problems could lessen participants' perception of barriers to medication-taking. Conclusion: To optimally support patients' diabetes care, practitioners should address concern beliefs among non-adherent patients with low health literacy, as well as emphasize self-efficacy and perceived barriers to medication adherence among all non-adherent patients with T2D.

The study shows that, according to patients, providers can both direct and facilitate T2D health literacy development in a number of ways including explaining T2D and how to manage it, considering the context of those explanations, tailoring T2D-related messages to individual patients, providing additional T2D educational resources, and following up with patients. This study extends current literature on patient-provider communication and health literacy by using patients' perspectives to identify specific strategies that healthcare providers can employ in their practices.

The health literacy level of an individual has been shown to have a direct impact on a person's overall health. Social support and corresponding communication have also been shown as vital. This study used a mixed-methods survey to examine how diabetes health literacy levels impact the communication of support to a person with diabetes. The hypothesis was supported, showing a relationship between interaction with a person with diabetes and an increased diabetes health literacy level. This study is the first to examine how support is communicated to a person with diabetes while also examining the role of health literacy. Recommendations for future research based on the findings are offered in an effort to examine the importance of types of support and levels of diabetes health literacy.

Despite the severity of this health condition, medication non-adherence among T2D patients has remained high, hindering them from achieving blood sugar control and desired health outcomes. Aim: To implement text messaging as an Evidence-based Intervention (EBI) to improve medication adherence and blood sugar control among T2D patients. Methods: Fifty T2D patients aged 18 years and above who had been identified as non-compliant, were engaged in the project for a period of 10 weeks. Participants' demographic information was collected using demographic survey forms. The initial step of the project involved the PM performing pretests for blood glucose. Later, blood glucose tests were performed at week 6 and week 10. Throughout the project, the participants were reminded through text messaging to take their medication, eat balanced meals, exercise, check their blood sugars, ask them how they are feeling, check their feet, remind them about clinical checks, ask them if they are feeling jittery and record blood sugar in the logs. Changes in blood sugar levels at weeks 6 and 10 were analyzed by PM. Findings: Thirty-four percent and 60% of the participants recorded improved blood sugar levels at weeks 6 and 10 respectively indicating that text message intervention at the end of the project was effective. The intervention exceeded the 50% benchmark at week 10. Implications: Text message intervention should be implemented as an evidence-based intervention to improve medication adherence among T2D patients. Nonetheless, there is a need to integrate the intervention with other interventions for more improvements in medication adherence. One in every three Americans with Type 2 diabetes has limited or low health literacy skills. Limited or low health literacy skills can impact a patient’s overall health outcome; therefore, a critical element for disease management and patient adherence is health literacy. The purpose of this three-month scholarly project was to determine if the utilization of an educational intervention specific to one’s level of health literacy, combined with a short message service reinforcement tool, could improve glycemic control and self-efficacy in Type 2 diabetes mellitus patients with limited and low HL. A sample of four participants was recruited for this scholarly project. Participants completed pre and post self-efficacy and health literacy questionnaires. During the three-month intervention phase, participants interacted with the project leader utilizing the text messaging reinforcement tool. After the three-month intervention phase, participants had an overall 9% increase in self-efficacy confidence, a 42% overall increase in health literacy knowledge, and an overall HbA1c mean value reduction of 0.5%. This combination therapy intervention is easy and convenient for health care providers and Type 2 diabetes mellitus patients with limited or low health literacy. This intervention is an efficient and effective method to improve patients self-efficacy and diabetes knowledge; furthermore, this method improves patient self-management skills, glycemic control, and patient longevity.

Despite the provision of comprehensive diabetes management programmes, patients are often not able to achieve the desired outcomes. The aims of the study were to investigate factors affecting self-care behaviours of patients with Type 2 DM among Hong Kong Chinese people and to explore barriers to self-care and strategies to overcome barriers from patients' and nurses' perspectives. The conceptual framework was developed from the Health Belief Model and the Theory of Planned Behaviour. A sequential explanatory mixed methods design was adopted. Structured questionnaires were used to investigate key variables for predicting self-care for 183 patients with Type 2 DM. Subsequently, 22 patients and 17 nurses were selected for focus group interviews. The quantitative findings showed that both attitudes and modifying factors (knowledge, demographics, socio-economic factors, insulin therapy) had a significant effect on self- care behaviours. They showed that older patients, those of lower socio-economic status, being treated with insulin, having better knowledge about DM, and with positive attitudes reported better adherence to self-care. The qualitative findings indicated that the patients and nurses perceived barriers to self-care were consistent including physical, psychological, socio-economic, environmental and cultural factors. The focus group interviews revealed that nurses understood most of the barriers to self- care perceived by their patients. The Diabetes Self-care Model was developed in the study to inform and guide healthcare professionals when designing diabetes management programmes by addressing key variables affecting self-care behaviours and patient outcomes. The unique position of nurses and their understanding of patients' needs are essential for improving patient outcomes. Therefore, the roles of nurses should be reinforced and further developed to address the needs of the patients through improving the effectiveness of diabetes management programmes and patient empowerment.

It is associated with microvascular and macrovascular complications which can lead to potential threats such as to amputations and even death. The irony of the disease is that these complications are preventable with appropriate treatment and self-management. The Emergency Medicine Department (ED) at University of Southwestern Medical center conducted this study to assess health literacy Parkland Memorial Hospital patients with T2D. The objective for the research study was to assess the for association of health literacy with management of T2D. Methods: This was a prospective study with collection of personal health information (PHI) and 30 day-follow up for ED recidivism. Eligibility was assessed by pre-screening via EPIC (Electronic Medical Record System for Parkland). The tool for measuring health literacy was the Short Assessment of Health Literacy (SAHL) and data was collected. Results: The total number of subjects enrolled was 23 with ages 18 or above male and females both with Spanish or English speaking only with T2D. Mean age of the subjects was ~50 years with standard deviation of 10 years, males were over half than women. About 74% were white hispanic males. According to the data collected, 30% of the patients demonstrated inadequate health literacy based on SAHL score survey. Since the study could not reach adequate power due to low enrollment, no significant associations could be made from this small sample size. Total number of subjects required to have adequate power was 400. Conclusions: Due to low enrollment period at this time the recommendation would be to continue collecting data to have a larger sample size to afford the observation of statistically relevant associations. If any, statistically significant associations are found, then future studies will focus on improving diabetes outcomes through the development of educational tools at the individual patient’s appropriate literacy level. There are many reasons to improve diabetes care and explore all possible factors that contribute to poor outcomes. Millions of people are living with uncontrolled diabetes and the burden is not only on the patient but also on the community as a whole. Quality care should aim for improved benchmarks for patients with diabetes and their knowledge about the disease, such as 1) obtaining HbA1c levels below 8%, 2) blood pressure in the normal range, 3) having regular foot exams to keep a check on any developing signs of pressure sores and 4) most importantly having dilated eye exam on a regular basis.

Book excerpt: Introduction Medication adherence remains a problem among Type-2 diabetes (T2D) patients despite availability of effective treatments. Three analyses of extant data sets were conducted to examine barriers to using medication as prescribed as an alternate method to assess medication adherence: 1) development and psychometric evaluation of the Murage-Marrero-Monahan-Medication barriers (4M) scale to assess patients' perceived barriers; 2) patient demographic factors associated with barriers to using medication as prescribed, and 3) the association between patients' perceived barriers to medication use and cardiovascular disease (CVD) risk factor control. Methods Twelve focus groups and a cross-sectional study of 362 T2D patients contributed to develop and evaluate psychometric properties of the 4M scale. A cross-sectional survey of 964 T2D patients was used for the other two studies. Analysis of covariance identified demographic factors associated with reported barriers. Multivariable logistic regression was used to identify barriers associated with CVD risk factors (glucose, blood pressure and lipids) categorized as either poor or good control. Results Exploratory factor analysis with Varimax rotation resulted in a 19-item 4M scale with acceptable psychometric properties. As a five-domain (or single-domain) structure, coefficient alpha ranged from 0.70 to 0.83 (0.92). Both structures demonstrated discriminant validity and known-group validity. Age was inversely associated with all identified barriers while income was inversely associated with poor communication with providers and side effects. A unit increase in the overall barrier mean score on the 4M scale was associated with 92% increase in the odds of having poor control of two or more CVD risk factors compared to good control of all three risk factors (adjusted OR=1.92, 95% CI: 1.16 - 3.17; p
