Book excerpt: America's healthcare system in the twenty-first century faces a variety of pressures and challenges, not the least of which is that posed by the increasingly multicultural nature of American society itself. Large numbers among the Hmong, immigrants from the landlocked Asian nation of Laos, continue to prefer their own ancient medical traditions. That these Hmong Americans should continue to adhere to a tradition of folk medicine, rather than embrace the modern healthcare system of America, poses questions that must be answered. This book takes up the task of examining Hmong American concepts of health, illness and healing, and looks at the Hmong American experience with conventional medicine. In so doing, it identifies factors that either obstruct or enable healthcare delivery to the Hmong, specifically a target sample of Hmong Americans resident in Colorado. Drawing upon scientific methods of data collection, the research reveals attitudes currently held by a group of American citizens toward health and medicine which run the gamut from the very modern to those which have prevailed in the highlands of Southeast Asia for centuries.

Book excerpt: The purpose of this study was to explore the cultural knowledge and health perspectives of second-generation Hmong, who have an increased risk of overweight, obesity, and chronic illnesses associated with being overweight and obese. In examining health perspectives that guide their actual health behaviors, I sought to examine if and how the second-generation live and reconcile their two worlds socially and emotionally. A total of 17 interviews were conducted with 11 second-generation students from Southern California institutions, 7 first-generation parents (two sets of parents), and 1 aunt who is considered the 1.5 generation. First and second generations are active players in syncretizing opposing cultural elements with mundane concerns of day to day life. Second-generation participants gather what they know to be traditional and modern, but most of their belief systems are grounded through a Eurocentric stance in which Eastern is the alternative to what they consider normal. However, Hmong traditions and Eastern models of understanding health are conceivable because of their direct experiences and relationships with their family and friends who share similar backgrounds, which then allow them to accept or at least consider a multicultural approach to health. This process is constant for the second generation where each individual adopts some components of each approach, as to why many participants stated, "we do whatever works."

Rapid advancements in medical technologies, breakthroughs in understanding the genetic underpinnings of health and ill health, improvements in the effectiveness and variety of pharmaceuticals, and other developments in biomedical research have helped develop cures for many illnesses and improve the lives of those with chronic diseases. By itself, however, biomedical research cannot address the most significant challenges to improving public health. Approximately half of all causes of mortality in the United States are linked to social and behavioral factors such as smoking, diet, alcohol use, sedentary lifestyle, and accidents. Yet less than five percent of the money spent annually on U.S. health care is devoted to reducing the risks of these preventable conditions. Behavioral and social interventions offer great promise, but as yet their potential has been relatively poorly tapped. Promoting Health identifies those promising areas of social science and behavioral research that may address public health needs. It includes 12 papersâ€"commissioned from some of the nation's leading expertsâ€"that review these issues in detail, and serves to assess whether the knowledge base of social and behavioral interventions has been useful, or could be useful, in the development of broader public health interventions.

Despite the huge population of Hmong who have settled in California, Minnesota, Wisconsin and Michigan, they still experience challenges when utilizing the modern health care system and its services. This study explores Hmong Americans' perceptions toward both traditional and modern health care and their utilization of these services. Participants in this study were Hmong American adults who are former refugees and immigrants from Laos and Thailand. The findings of this research study reveal that there are indeed challenges and obstacles that have prevented Hmong Americans from utilizing modern health care, as well as its services, such as different philosophies toward health and illness, providers' lack of understanding about the Hmong's cultural beliefs and/or practices, and lack of sufficient English comprehension skills to communicate with health care providers.

The study used Nola Pender's Health Promotion Model as a theoretical framework. The Health Promotion Lifestyle Profile II, a 52-item questionnaire; the General Self-Efficacy Scale, a 10-item questionnaire; and demographic characteristics (age range, gender, race, marital status, work status, and education) were used in the study. A convenience sample of 106 adult church members participated in the study. Spearman's nonparametric correlations were used to calculate the relationship between self-efficacy, the Health Promotion Lifestyle Profile II and its six subscales (health responsibility, physical activity, nutrition, spiritual growth, interpersonal relations, and stress management). There was a strong correlation between self-efficacy and spiritual growth, interpersonal relations, stress management, and the total Health Promotion Lifestyle Profile II. There was no difference in gender in self-efficacy and the Health Promotion Lifestyle Profile II subscales (leaf 24).

Book excerpt: Guided by Pender and colleagues' (2006) revised health promotion model (HPM), this descriptive correlational study was designed to explore the relationships between personal factors (comprised of sociodemographic factors, degree of acculturation, and perceived stress), perceived health self-efficacy, perceived social support, and health promotion lifestyle behaviors (HPLBs) among a group of Arab American women (AAW) living in Southern California. A second purpose was to explore the psychometric properties of the translated version of the perceived health competence scale (PHCS). A convenience sample of 267 AAW were administered a paper copy of a self-reported survey. Four of the study's five standardized measures were available in Arabic and were tested in the Arab American populations; only the PHCS was translated to Arabic. The HPM guided the synthesis of relevant literature concerning AAW's health promotion behaviors; the model facilitated understanding of their health needs, risks, and challenges. The analysis revealed a bivariate association between the health-promotion lifestyle profile II (HPLP II) total score and the participants' age, years of residency in the United States, acculturation, perceived stress, self-efficacy, and social support. Multiple linear regression was used to examine the relationship between the study variables. Acculturation, perceived stress, perceived health self-efficacy, and perceived social support explained 46% of the variance in HPLBs scores. The Cronbach's alpha for the PHCS was .819 in both versions. Based on the participants' language preference; Arabic or English, two study groups were formed. The groups' responses on HPLP II and PHCS were compared. The Pearson product moment correlation coefficient indicated a lack of association between the participants' spoken language and their responses on both scales. Independent sample t-tests showed statistically significant differences in the physical activity, interpersonal relations, and spiritual-growth HPLP II subscales. No statistically significant differences were found on the PHCS groups' responses. The findings of this study can inform future intervention studies to address specific health promotion behaviors, such as nutrition, eating behaviors, stress management, and physical activity among AAW and women from other minority groups.
