Public and even physician interest in the subject remained minimal until the early forties, and even then there were not more than a dozen "fulltime" investigators worldwide. The first organization devoted to communicating regularly the results of hy pertension research was created in Cleveland in 1945; it became the Council for High Blood Pressure Research of the American Heart Association in 1949. The early history of hypertension is not a story of progressive achievement. Richard Bright is given great credit, followed by Mahomed, Allbutt, Riva-Rocci, Bergman and Janeway, but it was not until Volhard, Fahr and Allen that some semblance of order emerged. They were followed by a younger, much more vigorous group of investigators who in fact initiated modern research in hypertension. Two more generations have fol lowed. In the course of some fifty years, we have seen emerge a magnificent body of evidence that has prescribed good treatment and contributed a considerable understanding of the many complex mechanisms involved in the hypertensions. Treatment is now actively pro moted by public health agencies, including the World Health Organization. This is an other case of treatment preceding full understanding of the nature of the disease.

It aims to select all the more relevant data to guide the attitudes to prevent, diagnose, and treat hypertension. Hypertension accounts for more than 50% of deaths from stroke and ischemic heart disease worldwide. New blood pressure (BP) diagnostic thresholds for hypertension were released, which were set at 130/80 mmHg. As a consequence, millions of individuals in the world will be diagnosed as hypertensive, recognizing that they are at greater risk of presenting a CV event. Prevention and control of high BP will become the main focus for reducing the burden of CV disease, requiring a changing of cultural beliefs in some way similar to what happened in the last century with smoking. Strategies for prevention of the rising of BP with age, and the BP reduction in individuals already with high levels, are more complex than those related to smoking control. These strategies involve solid evidence to be implemented in populations. The extensive scientific literature dealing with hypertension and BP regulation is among the top dedicated to a single disease. The chapters and contents follow the clinical reasoning pathways. The characterization of the risks of high blood pressure is presented in the first chapter, discussing the evidence that led to changes in diagnostic thresholds and to the recommendations for maintaining BP within these limits in populations. Reasons for BP rising with age will follow, identifying the causes that must be fought to preventing the incidence of hypertension. Diagnosis of hypertension deserves a special chapter. The final chapter presents the fundamentals to select drug and non-drug therapies indicated in the prevention and controlling of high blood pressure.

Second, it suffers as a target organ of long-standing hypertension, and third, it experiences the effects of antihypertensive therapy. Perhaps most contested at the present time is the involvement of the kidney in the patho genesis of essential hypertension. More than a century ago, William Osler put forward three basic hypotheses about the "genuine contracted kidney. "l 1. "The hypertrophy can be regarded as an effect to overcome a sort of stop-cock action of the vessels when under the influence of an irritating ingredient in the blood greatly contracted and increased the peripheral resistance. " Clearly this hypothesis of an "irritating ingredient" is perhaps the most convincing nowadays, and numerous attempts have been made to identify a specific vasoconstrictive agent in the blood in essential hypertension. 2. "The obliteration of a large number of capillary territories in the kidney materially raised the arterial pressure. An additional factor of dimin ished excretion of water also heightened the pressure within the blood vessel. " Today we know that fluid volume overload in the presence of reduced renal mass seems to be the most likely mechanism accoun ting for renal parenchymal hypertension and, as shown by Guyton's group, for certain forms of experimental hypertension. 3.
