Book excerpt: Abstract: Objectives This multicenter randomized controlled clinical trial was conducted to investigate whether the loading protocol of single dental implants placed in the midline of edentulous mandibles will influence the implant survival or prosthetic maintenance. Materials and Methods In total, 158 patients were randomly assigned either to the immediate loading group (n = 81) or to the delayed loading group (n = 77). All implants were loaded with an overdenture retained by a ball attachment. Results After 5 years, 102 patients attended the follow-up investigation. Immediately loaded single implants in the midline of the edentulous mandible revealed a statistically significant lower survival rate than implants loaded conventionally over an observation period of 5 years. In the immediate loading group, 9 implants failed within the first three months of implant loading. No further implant loss was recorded for this group. Two implants failed in the delayed loading group, whereas one implant had to be removed during second-stage surgery and the second five years after implant loading. Non-inferiority of the survival rate of the midline implant of the immediate loading group, compared with the delayed loading group, could not be shown (p = .79, CI immediate loading: 74.9%; 100.0%, CI delayed loading: 73.0%; 100.0%). The observed difference in implant survival between the two treatment groups over time was statistically significant. Conclusions The results of the present study indicate that immediate loading of a single mandibular implant in the edentulous mandible should be considered only in exceptional cases

A single mandibular implant in the edentulous mandible improves patientu2019s satisfaction and oral health related quality of life significantly. It was the aim of this multicenter randomized controlled clinical trial to show that immediate loading of a single dental implant in the edentulous mandible will result in comparable clinical outcomes as using the standard protocol of closed healing and delayed loading. Here, the patientsu2019 chewing efficacy after 24 months of observation was analyzed.Methods: One hundred and sixty-three edentulous patients received one implant in the edentulous mandible and 158 patients were randomly assigned either to the immediate loading group (n=81, group A) or the delayed loading group (n=77, group B). All patients chewed an artificial standardized test food with 40 chewing cycles at baseline, 4 weeks after implant placement (only group B), and 1, 4 and 24 months after implant loading. After chewing, the test food particles were centrally sieved using a sieve apparatus with 5 sieves of different sizes. The percentage share of each of the 5 sieve capacities was calculated resulting in 100% for the whole test food of each chewing sample.The statistical analysis was performed using the Wilcoxon signed rank and rank sum tests as well as the Friedman-test. Additionally, a linear interpolation was used to identify a theoretical sieve, through which 50 % of the chewed test food would pass.Results: The chewing efficacy increased over time in both groups. After 24 months of implant loading, a significant increase regarding chewing efficacy was observed for both groups compared to the baseline data (pu22640.05). Between the two groups, chewing efficacy did not differ significantly at any point in time (p>0.05). Conclusion: A single midline implant in the edentulous mandible increases chewing efficacy significantly, independently from the loading protocol. This study was financially supported by the Deutsche Forschungsgemeinschaft (DFG, German research foundation, KE 477/8-1).

The purpose of the study was to compare the clinical outcomes and survival rates of immediately-loaded short implants and conventional length dental implants (Pw Plus dental implant system, Thailand) in replacing mandibular molar teeth.Materials and methodsThe clinical study was approved by the Human Experimentation Committee. Fifty implants in 46 patients were included in the study. Twenty five short dental implants (6 mm.) and 25 conventional dental implants (10 mm.) were randomly placed as single tooth replacements in the edentulous molar areas of the mandibles. Provisional, CAD-CAM, ceramic block (Shofu HC) crowns were cemented to the abutments of the implants, and immediately loaded. The clinical parameters, such as insertion torque (IT), resonance frequency analysis (RFA), marginal bone level changes, and complications, were recorded and statistically analysed.ResultsTwo short implants and two conventional implants were excluded from the study due to insufficient insertion torque (less than 35 Ncm). Two short implants disintegrated, and one conventional implant failed. The accumulate survival rate of short implants was 91.30%, compared to 95.65% for conventional implants. However, there was no significant difference between the two implant types (P (Fisheru2019s exact) = 1.00). Only minor complications were found (three provisional crown fractures with short implants and two with conventional implants).The differences in IT among the two implant types were not significant (P =.264). There was no significant difference in ISQ value for short or conventional implants between baseline (short: 73.86 u00b1 2.38 mm, conventional: 75.05 u00b1 3.26mm, P=.088) and four months after loading (short: 72.37 u00b1 1.35 mm, conventional: 72.89 u00b1 1.87 mm, P=.165). The mean change in marginal bone level four months post-loading was 0.24 u00b1 0.29 mm for short implants and 0.26 u00b1 0.29 mm for conventional implants ; there was no statistical difference between the two implant types.ConclusionsWithin the limitations of this study, the findings seem to confirm that the immediate loading of short implants is a viable option, comparable to conventional length implants in terms of implant survival, marginal bone loss and ISQ value.

Short implants are most often placed in compromised bone situations. The presentstudy compares test and control implants under the same conditions.Aim/Hypothesis:The aim of this multicentre randomized controlled study was to compare the clinical performance of implants with a length of 6 mmwith an 11mm control group, when using early loading in the (pre)molar region in both maxilla and mandible. This paper will presentthe five-year data of the study.Materials and Methods:Six study sites were involved in this international randomized controlled multi-center study. To be included, patients needed to be:between the ages of 20 to 70 years at enrolment, a history of edentulism in the study area for at least 4 months, adjacent teeth tohave natural roots, presence of an opposing dentition and sufficient bone volume to accommodate at least an 11 mm implant.Ninety-five patients with two or three teeth missing in the premolar and molar regions were included. Each patient was randomlyallocated to treatment groups to receive implants with a length of either 6 mm or 11 mm (OsseoSpeedTM 4.0 S), Astra Tech ImplantSystem, Dentsply Sirona Implants. Implants were placed using a one-stage surgical procedure, with loading after 6-7 weeks andrestored with a screw-retained fixed prosthesis. Clinical and radiographic examinations were performed pre-operatively, post-surgery,loading, 6 and 12 months after restoration placement, and annually thereafter.Results:In total 209 implants were inserted (63% mandible, 37% maxilla). From loading to the 5-year follow-up, a mean marginal bone levelchange of 0.02 u00b10.52 mm (bone gain) in the 6 mm group and -0.18 u00b11.05 mm (bone loss) in the 11 mm group was found (P-value:0.923; Mann-Whitney U test). Presence of bleeding on probing was detected around 44% of the 6 mm implants and 33% of the 11mm implants. Probing pocket depths were generally shallow, with a mean of 2.57 u00b10.75 mm in the 6 mm group and 2.91 u00b11.53 mm inthe 11 mm group at time of the 5-year follow-up. Two 6 mm implants failed to integrate (early loss) and one was lost before the 2-yearevaluation. In the 11mm group 1 implant was lost 4 weeks after loading due to bone loss. Besides the loss of these four implants, nocomplications were recorded in the two treatment groups during the five years in use. This give a cumulative survival rate of 93.6% forthe subjects with 6 mm implants and 97.8% for the control group, five years post loading.Conclusions and Clinical Implications:The final data from this randomized multi-center study with a 5-year follow-up indicates that treatment with the Astra Tech ImplantsSystem short implant (6 mm) is as reliable as treatment with implants of standard length (11 mm). This provides a good treatmentoption in situations with limited bone height in the premolar and molar regions.

Although nano-surface modifications increase osseointegration, initial implant stability at time of placement is an important clinical factor in implant survival and is considered a prerequisite for early or immediate loading protocols.Aim:This RCT evaluates the 3 years outcome of modifications of the OsseoSpeed implant (Dentsply Sirona Implants, Mu00f6lndal, Sweden) in terms of survival, bone level and peri-implant health. Alterations in the OsseoSpeed EV consisted of changes in dimensions of body shape, drilling procedures, drill design and abutment connection compared to the original OsseoSpeed TX.Material and methods:120 partially edentulous patients, enrolled in five clinics, were randomized to test (EV) and control (TX). 59 subjects received 79 EV and 61 subjects received 87 TX implants respectively. Cemented permanent restorations were placed on average 9 weeks later on titanium stock abutments. Peri-implant bone level changes, plaque, probing pocket depths and bleeding indices were assessed at restoration placement (baseline), and thereafter at 6, 12 and 36 months.Statistical comparison between EV and TX were done with Mann Whitney U-test with p0.05 as significance level.Results:104 subjects (50 EV, 54 TX) were assessed (66 and 75 implants respectively) after 3 years. Three patients discontinued due to failure, 7dropped out and 6 were not available. The groups were comparable (p0.05) regarding age, gender, race, smoking habits, tooth loss reason, periodontal history, implant location, length and diameter. Mean implant insertion torque was 30u00b113 Ncm and 22u00b19 Ncm for EV and TX, indicating improved stability for the modified protocol. Five of 79 EV implants were lost (93.6%, 4 prior and 1 post-loading). One of 87 TX implants failed prior to loading (98.9%). This difference was not statistically significant. Regarding crestal bone change, a mean gain was observed from baseline to 3 years of 0.29mm (SD 0.72) and 0.21mm (SD 0.48) for EV and TX respectively. Probing pocket depth at 3 years was 2.72mm (SD 0.73) vs. 2.82mm (SD 0.73). Plaque was present on 8.3% and 9.0% of surfaces, and bleeding on probing remained stable with a mean of 17.4% and 18.0%. None of the parameters were statistically significantly different between groups (p>0.05).Conclusion:Peri-implant health did not differ between OsseoSpeed EV and TX implants. A gain in crestal bone levels was observed in both groups but EV implants indicated an improved primary stability.Although not possible to determine, it cannot be excluded that in three of the four early EV failures, a deviation in the cortical bone preparation may have contributed to the outcome.Experiences from this study led to modifications of the suggested drilling protocol including widening of the spongious bone preparation to facilitate the implant placement within recommended torque value.

Book excerpt: Aim: To assess the performance of microtextured dental implants with or without a machined collar at one year after placement. Material and Methods: Fifty-eight patients received dental implant therapy to restore missing teeth in one area of the maxilla or mandible at the UAB Department of Graduate Periodontology. Residents in the program placed microtextured implants with a microtextured collar with microgrooves (test group) and microtextured implants with a machined collar (control group). Sites were randomized to the test group or control group. Study visit protocol was followed and implants were restored by off-site dentists. Sixty-nine (69) implants were assessed at one year for a final study visit. The primary outcome was to determine the difference between crestal bone levels in the test versus the control group implants as measured by differences in radiographic crestal bone heights at baseline and one year. Data collected consisted of periapical radiographs and clinical photographs that were taken at specific time points throughout the study period. Statistical analyses using individual t-tests and ANOVA were performed to analyze the data. Results: Statistical analysis determined that a significant difference in marginal bone loss was present between the roughened collar (0.65±0.43) and machined collar (1.20±0.62) MTX implants at one year (p
