Available in PDF, EPUB and Kindle. Book excerpt: The co-operative spirit of citizens in twentieth-century Saskatchewan nurtured innovation in health care and health policy. 36 Steps on the Road to Medicare showcases the decisions that led to the province's medicare system - the forerunner of Canadian health care. Stuart Houston and Merle Massie document the range of Saskatchewan leadership on Canadian, North American, and world stages: municipal doctors and municipal hospitals, the first Red Cross Outpost Hospital in the British Empire, tuberculosis diagnosis and treatment, a successful pilot comprehensive regional health care plan, government-sponsored cancer clinics, innovative LSD and patient-oriented treatment for psychoses, the first full-time cancer physicist in Canada, and the world's first concerted clinical use of the betatron and Cobalt-60 in cancer treatment. They show how North America's first social-democratic government, the Co-operative Commonwealth Federation - elected in 1944 and led by the incomparable Tommy Douglas - created the blueprint for comprehensive health care and how sequential steps on the road to medicare were implemented quickly and within budget. When federal support for national hospitalization became available, Saskatchewan could afford to initiate medicare in 1962. Other Canadian provinces soon followed Saskatchewan's lead. Updated to engage with current debates, 36 Steps on the Road to Medicare navigates the history of medicare and demonstrates the spirit of innovation that Canada will need to save it.

Available in PDF, EPUB and Kindle. Book excerpt: The Saskatchewan Mental Hospital at Weyburn has played a significant role in the history of psychiatric services, mental health research, and providing care in the community. Its history provides a window to the changing nature of mental health services over the 20th century. Built in 1921, Saskatchewan Mental Hospital was considered the last asylum in North America and the largest facility of its kind in the British Commonwealth. A decade later the Canadian Committee for Mental Hygiene cited it as one of the worst facilities in the country, largely due to extreme overcrowding. In the 1950s the Saskatchewan Mental Hospital again attracted international attention for engaging in controversial therapeutic interventions, including treatments using LSD. In the 1960s, sweeping healthcare reforms took hold in the province and mental health institutions underwent dramatic changes as they began transferring patients into communities. As the patient and staff population shrunk, the once palatial building fell into disrepair, the asylum’s expansive farmland went out of cultivation, and mental health services folded into a complicated web of social and correctional services. Erika Dyck’s Managing Madness examines an institution that housed people we struggle to understand, help, or even try to change.

Book excerpt: When the CBC organized a national contest to identify the greatest Canadian of all time, few were surprised when the father of Medicare, Tommy Douglas, won by a large margin: Medicare is central to Canadian identity. Yet focusing on Douglas and his fight for social justice obscures other important aspects of the construction of Canada's national health insurance - especially its longstanding dependence on immigrant doctors. Foreign Practices reconsiders the early history of Medicare through the stories of foreign-trained doctors who entered the country in the three decades after the Second World War. By making strategic use of oral history, analyzing contemporary medical debates, and reconstructing doctors' life histories, Sasha Mullally and David Wright demonstrate that foreign doctors arrived by the hundreds at a pivotal moment for health care services. Just as Medicare was launched, Canada began to prioritize "highly skilled manpower" when admitting newcomers, a novel policy that drew thousands of professionals from around the world. Doctors from India and Iran, Haiti and Hong Kong, and Romania and the Republic of South Africa would fundamentally transform the medical landscape of the country. Charting the fascinating history of physician immigration to Canada, and the ethical debates it provoked, Foreign Practices places the Canadian experience within a wider context of global migration after the Second World War.

Book excerpt: Surveying the field of political history in Canada, one might assume that the politics of the nation have been shaped solely by the Liberal and Conservative parties. Relatively little attention has been paid to the contributions of the CCF and NDP in Canadian politics. This collection remedies this imbalance with a critical examination of the place of social democracy in Canadian history and politics. Bringing together the work of politicians, think tank members, party activists, union members, scholars, students, and social movement actors in important discussions about social democracy delving into an array of topics including municipal, provincial, and national issues, labour relations, feminism, contemporary social movements, war and society, security issues, and the media, Party of Conscience reminds Canadians of the important contributions the CCF and NDP have made to a progressive, compassionate idea of Canada.

Book excerpt: Burdened with perennially rising costs and responsible for providing health insurance to more than one sixth of all Americans, Medicare in its original form is fiscally and demographically unsustainable. In light of dramatic reforms under the Affordable Care Act (ACA), this book provides a comprehensive overview of the current state of Medicare. Eleanor D. Kinney explains how the ACA addresses systemic problems of cost and volume inflation, quality assurance, and fraud. Recognizing the potential for more radical change in the future, Kinney also explores the potential of Medicare to become a single-payer system. Comparisons are made with national health systems in Canada and the United Kingdom, from which the United States can draw valuable lessons. An approachable yet comprehensive account of Medicare and the ACA, this book will be invaluable for health care professionals and informed citizens.

Available in PDF, EPUB and Kindle. Book excerpt: Separate Beds is the shocking story of Canada's system of segregated health care. Operated by the same bureaucracy that was expanding health care opportunities for most Canadians, the "Indian Hospitals" were underfunded, understaffed, overcrowded, and rife with coercion and medical experimentation. Established to keep the Aboriginal tuberculosis population isolated, they became a means of ensuring that other Canadians need not share access to modern hospitals with Aboriginal patients. Tracing the history of the system from its fragmentary origins to its gradual collapse, Maureen K. Lux describes the arbitrary and contradictory policies that governed the "Indian Hospitals," the experiences of patients and staff, and the vital grassroots activism that pressed the federal government to acknowledge its treaty obligations. A disturbing look at the dark side of the liberal welfare state, Separate Beds reveals a history of racism and negligence in health care for Canada's First Nations that should never be forgotten.
