Book excerpt: While women have played critical roles in the military since the American Revolution, they were not offered veteran status until the last few decades. The concept of a female veteran is relatively new, and one that many female service members have not recognized themselves. The current study examined the factors impacting female service members' ability to construct a veteran identity following their separation from the military. The study also investigated whether the female service members' veteran identity influenced their preference and use of Veteran Health Administration (VA) hospitals and clinics, and how well they reintegrated back into civilian society after their service. This study used a mixed-method design that included a descriptive statistical survey and a correlational analysis of 128 women veteran participants. The study found a positive correlation between women's veteran identity and preference of VA health care services. However, the findings did not support the hypothesis that women's veteran identity is negatively correlated with reintegration. Further ancillary analyses were conducted in order to gain additional information on variables that may influence women's veteran identity, use of VA services, and reintegration.

This disparity continues in the mental health outpatient treatment options for women Veterans. Veteran Administration (VA) healthcare services may not be suited to their needs as women, and civilian services are not well suited to manage their needs as Veterans. Therefore, this dissertation study used constructivist Grounded Theory methods to explore the experiences of women Veterans when accessing mental health outpatient services, their decision-making process when make the choice to enter mental health service, and aspects of the experience that are important or meaningful to them. Twelve women Veterans revealed meaningful, personal stories on their experiences of trauma and their use of mental health outpatient services. While addressing each of these factors, what emerged from the data was a broader Grounded Theory Process model of how women Veterans process trauma, and the categories of Trauma, Transitions, Identity and Structure. Women Veterans who participated in the study used mental health outpatient services to assist in reestablishing identity after trauma and to propel forward in their lives. This research provides key insight into how women Veterans make healthcare related choices and process traumatic events, like military sexual trauma (MST). This has implications for research, practice, and policy to improve the provision of care for women Veterans.

Book excerpt: Women Veterans are among the fastest growing groups of new VA health care users of the VA healthcare system, and currently reflect approximately eight percent of all U.S. Veterans. With Operation Enduring Freedom and Iraqi Freedom (OEF/OIF), women comprise a larger percentage of the military (11.3 percent) than of prior military operations. As of fiscal year 2010, 51.3 percent of female OEF/OIF Veterans had enrolled in VA health care, in sharp contrast to women from previous eras (an estimated 11 percent). Of this group, 88 percent have used VA health care more than once. To better understand the needs of this rapidly growing group, women Veterans' health research has expanded as well. Two previous systematic reviews examined the literature on women Veterans' health and health care up to 2008. In this review, we specifically explored women Veterans' post-deployment health, with two main areas of concentration: 1) post-deployment effects on reproductive health for women Veterans, and 2) post-trauma sequelae among women Veterans from the OEF/OIF cohort. In response to a growing need to understand the effects of military service on health status, this report supplements our prior review by focusing directly on the reproductive and trauma effects on women in the military or Veterans who have been deployed. The goal is to broaden the knowledge of VA policy leaders and clinicians about post-deployment health issues for women. The Key Questions were: Key Question #1: What research has been published on the effects of deployment on postdeployment reproductive outcomes? We operationalized “reproductive effects” to encompass the following: fertility issues, birth defects, menstrual effects (e.g., change in cycles, loss of cycles), urinary tract infections, sexually transmitted infections, and reproductive cancers (e.g., cervical, ovarian, etc). Key Question #2: What research has been published on post-trauma sequelae in OEF/OIF women Veterans, including: mental health problems, suicide, cardiovascular disease, risky health behaviors (including: tobacco use, hazardous alcohol use, substance abuse, suicide, homicide, assaultive behavior, and eating disorders), and other post-trauma sequelae?

Concurrently, women are one of the fastest growing groups of new users in the Department of Veterans Affairs (VA) Healthcare System, with particularly high rates of utilization among veterans of Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF). Of the more than 100,000 OEF/OIF women veterans, over 44% have enrolled in the VA system for health care. Thus, women veterans represent an integral part of the veteran community. The body of literature dedicated to women veterans' health and health care issues has grown significantly since the publication of one previous systematic review focused on women veterans. To address the growing demand and potential needs of women veterans in the Department of Veterans Affairs (VA) Healthcare System, this project sought to assess the state of women veterans' health research and stratify the literature into domains relevant for VA research and policy. Women's military experiences and responses to their military experiences are often distinct from those of men, and these differences can affect both their health status and their health care needs as active duty personnel and as veterans. This, together with the rise in the number of women veterans in the VA system, calls for increased understanding of women veteran health issues and areas of potential knowledge deficit in order to guide VA care and VA research efforts. The body of research literature dedicated to women veterans and women's military health and health care issues has significantly grown and expanded in size and scope since the publication of the first systematic review of women veterans research. This project updated that review by examining the literature on women veterans' health and health care from 2004 to 2008.

During the same period, the number of women veterans treated on an outpatient basis rose by about 32 percent, or 119,300.
