Book excerpt: These recommendations include approaches for differentiated service delivery (DSD) of antiretroviral therapy (ART) that are less intensive and provide person-centred services to improve both quality and retention in care. As a result many countries have introduced or are scaling up DSD ART models. Currently, countries and partners are seeking to strengthen monitoring of differentiated ART service delivery to address implementation gaps and crucially improve patient outcomes.

Book excerpt: An estimated forty million people carry the human immunodeficiency virus (HIV), and five million more become newly infected annually. In recent years, many HIV-infected patients in wealthy nations have enjoyed significantly longer, good-quality lives as a result of antiretroviral therapy (ART). However, most infected individuals live in the poorest regions of the world, where ART is virtually nonexistent. The consequent death toll in these regionsâ€"especially sub-Saharan Africaâ€"is begetting economic and social collapse. To inform the multiple efforts underway to deploy antiretroviral drugs in resource-poor settings, the Institute of Medicine committee was asked to conduct an independent review and assessment of rapid scale-up ART programs. It was also asked to identify the components of effective implementation programs. At the heart of the committee's report lie five imperatives: Immediately introduce and scale up ART programs in resource-poor settings. Devise strategies to ensure high levels of patient adherence to complicated treatment regimens. Rapidly address human-resource shortages to avoid the failure of program implementation. Continuously monitor and evaluate the programs to form the most effective guidelines and treatment regimens for each population. Prepare to sustain ART for decades.

Book excerpt: Infectious diseases are the leading cause of death globally, particularly among children and young adults. The spread of new pathogens and the threat of antimicrobial resistance pose particular challenges in combating these diseases. Major Infectious Diseases identifies feasible, cost-effective packages of interventions and strategies across delivery platforms to prevent and treat HIV/AIDS, other sexually transmitted infections, tuberculosis, malaria, adult febrile illness, viral hepatitis, and neglected tropical diseases. The volume emphasizes the need to effectively address emerging antimicrobial resistance, strengthen health systems, and increase access to care. The attainable goals are to reduce incidence, develop innovative approaches, and optimize existing tools in resource-constrained settings.

Book excerpt: "The World Health Organization (WHO), with support from the Pangaea Global AIDS Foundation, and funding from the Bill & Melinda Gates Foundation, convened a meeting of experts in HIV diagnostics and HIV-related tests, to identify short-term optimization priorities for WHO and its partners in the scale-up of HIV serological diagnosis, HIV treatment initiation and treatment monitoring. For this meeting, "short term" was defined as a 1-3 year timeframe, and the overall goal of the meeting was to identify the obstacles, challenges and opportunities to improve delivery of currently available laboratory technologies for HIV diagnosis, treatment initiation and treatment monitoring in resource-limited settings. In particular, two specific objectives were set: 1. To define the principles and priorities for optimizing HIV diagnosis, treatment initiation & monitoring, utilizing currently available laboratory technologies. 2. To identify and propose strategies to overcome obstacles to the timely implementation of these priorities and principles, including (but not limited to) quality assurance, regulatory approval, cost, available international and national funding, lack of sufficient normative guidance, product manufacturing scale up, required healthcare capacity and accessibility. The recommendations of this meeting are intended to inform the development of WHO's normative guidance to countries on how to make best use of existing diagnostic and testing technologies and approaches, as well as to provide some initial guidance to assay developers, policy makers, procurers and users on future strategies."--Page 3.

Book excerpt: Recency assays use one or more biomarkers to identify whether HIV infection in a person is recent (usually within a year or less) or longstanding. Recency assays have been used to estimate incidence in representative cross-sectional surveys and in epidemiological studies to better understand the patterns and distributions of new and longstanding HIV infections. This technical guidance outlines best practices regarding the appropriate use of HIV recency assays for surveillance purposes and updates 2011 technical guidance from the World Health Organization (WHO) and the Joint United Nations Programme on HIV/AIDS (UNAIDS) on the use of HIV recency assays.

This book was released on 2020-06-18 with total page 56 pages. Available in PDF, EPUB and Kindle. Book excerpt: Histoplasmosis is a disease caused by the fungus Histoplasma capsulatum. This disease is highly endemic in some regions of North America, Central America, and South America and is also reported in certain countries of Asia and Africa. It often affects people with impaired immunity, including people living with HIV, among whom the most frequent clinical presentation is disseminated histoplasmosis. The symptoms of disseminated histoplasmosis are non-specific and may be indistinguishable from those of other infectious diseases, especially disseminated tuberculosis (TB), thus complicating diagnosis and treatment. Histoplasmosis is one of the most frequent opportunistic infections caused by fungal pathogens among people living with HIV in the Americas and may be responsible for 5-15% of AIDS-related deaths every year in this Region. These guidelines aim to provide recommendations for the diagnosis, treatment, and management of disseminated histoplasmosis in persons living with HIV. Although the burden of disease is concentrated in the Americas, the recommendations presented within these guidelines are applicable globally. These guidelines were produced in accordance with the World Health Organization (WHO) handbook for guideline development. The Guideline Development Group elaborated the final recommendations based on a systematic review of scientific literature and critical evaluation of the evidence available using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) approach. These guidelines are intended for health-care providers, HIV program managers, policy-makers, national treatment advisory boards, researchers, and other professionals involved in caring for people who either have or may be at risk of developing disseminated histoplasmosis.

Book excerpt: Introduction: Previous operational research studies have demonstrated the feasibility of large-scale public sector ART programs in resource-limited settings. However, organizational and structural determinants of quality of care have not been studied. Methods: Using data on 13 urban HIV treatment facilities in Zambia, we assess the impact of structural determinants on health workers' adherence to national guidelines for conducting laboratory tests such as CD4, hemoglobin and liver function and WHO staging during initial and follow-up visits as part of Zambian HIV care and treatment program. Results: CD4 tests were more routinely ordered during initial history and physical (IHP) than follow-up (FUP) visits (85.5% vs. 93.0%; p
