There has been a dramatic global increase in the rates of women living with HIV/AIDS. Among young women, especially in developing countries, infection rates are rapidly increasing. Many of these women are also mothers with young infants. When a woman is labeled as having HIV, she is treated with suspicion and her morality is being questioned. Previous research has suggested that women living with HIV/AIDS can be affected by delay in diagnosis, inferior access to health care services, internalized stigma and a poor utilization of health services. This makes it extremely difficult for women to take care of their own health needs. Women are also reluctant to disclose their HIV-positive status as they fear this may result in physical feelings of shame, social ostracism, violence, or expulsion from home. Women living with HIV/AIDS who are also mothers carry a particularly heavy burden of being HIV-infected. This unique book attempts to put together results from empirical research and focuses on issues relevant to women, motherhood and living with HIV/AIDS which have occurred to individual women in different parts of the globe. What Adia Benton chronicles in this ethnographically rich and often moving book is how one war-ravaged nation reoriented itself as a country suffering from HIV at the expense of other, more pressing health concerns. During her fieldwork in the capital, Freetown, a city of one million people, at least thirty NGOs administered internationally funded programs that included HIV/AIDS prevention and care. Unfortunately, the complex system currently in place for financing and delivering publicly financed HIV care undermines the significant advances that have been made in the development of new technologies to treat it. Many HIV patients experience delays in access to other services that would support adhering to treatment. As a result, each year opportunities are missed that could reduce the mortality, morbidity, and disability suffered by individuals with HIV infections. Public Financing and Delivery of HIV/AIDS Care examines the current standard of care for HIV patients and assesses the extent the system currently used for financing and delivering care allows individuals with HIV to actually receive it. The book recommends an expanded federal program for the treatment of individuals with HIV, administered at the state level. This program would provide timely access and consistent benefits with a strong focus on comprehensive and continuous care and access to antiretroviral therapy. It could help improve the quality of life of HIV/AIDS patients, as well as reduce the number of deaths among those infected.

This new book focuses on HIV and aging in the context of ageism with regard to prevention, treatment guidelines, funding, and the engagement of communities and health and social service organizations. The lack of perceived HIV risk in late adulthood among older people themselves, as well on the part of providers and society in general, has led to a lack of investment in education, testing, and programmatic responses. Ageism perpetuates the invisibility of older adults and, in turn, renders current medical and social service systems unprepared to respond to patients’ needs. While ageism may lead to some advantages – discounts for services, for example – it is the negative aspects that must be addressed when determining the appropriate community-level response to the epidemic.
