(Contains some copyrighted material) Chemical, biological, and mid-spectrum agents are often referred to as weapons of mass destruction, or WMDs, and the official military definition of WMD includes these three kinds of agents, Chemical agents, biological agents, toxins, and point sources of radiation may cause mass casualties while leaving structures intact; a better term for these kinds of weapons is mass-casualty weapons, or MCWs. Unconventional weapons is a term used to refer to chemical agents, biological agents, toxins, nuclear and thermonuclear bombs, radiological dispersal devices (or RDDs, also called “dirty bombs”), and point sources of radiation used as weapons. . The list of chemical warfare agents officially designated as such by the US military includes those chemicals that are intended to cause death or serious injury and also those intended to cause incapacitation, that is, temporary inability to perform one’s military duties. The former are called toxic agents and include (1) lung-damaging agents (also called pulmonary or choking agents); (2) “blood” agents (specifically, cyanide compounds); (3) vesicants (blistering agents); and (4) nerve agents. Those designed to produce only temporary incapacitation are referred to as incapacitating agents. This handbook will address each of these groupings of “official” chemical warfare agents as well as riot-control agents, which are technically not chemical warfare agents according to the US military definition, but are widely used in law enforcement for mass incapacitation. Chemical agents may have chemical names as well as common names. Chemical agents developed for military use may also have a NATO code. The NATO code is a one- to three-letter designation assigned after World War II to provide standard recognizable shorthand identification. For example, the chemical compound O-isopropyl methylphosphonofluoridate has the common name sarin and the NATO code GB. Militarization of nuclear atomic forces, chemicals and biological agents has considerably enlarged the arena of warfare, but of possibly even greater concern is the threat of such agents being deployed by terrorists. This book was originally published in French in 2004: subsequent events, such as the London bombings in July 2005, have only reinforced the importance of all doctors and emergency personnel understanding the various agents that could be used and having the knowledge to deal with victims of an attack or even an industrial accident. The book has therefore been translated into English to make it available to a wider audience. The book was coordinated by Chantal Bismuth, Professor of Medicine who has acted as an advisor for the Minister of Health in France and is an international consultant in toxicology. Her co-editor, Patrick Barriot, is an anaesthetist with operational experience in the Paris Fire Brigade and the 11th division of Paratroops who is now responsible for the department of ‘Biological risks from new technologies’. The authors are representative of the doctors who would have to deal with the human casualties of warfare or a terrorist attack. They review all weapons of mass destruction, both chemical and biological, including the use of bacteria, anthrax and viruses such as variola and influenza. In each case, they describe the pathogenic agent, the human consequences, organizational aspects of care for the victims and best practice for treatment. As one author reports, “The infections caused by potential biological warfare agents are seldom taught in the course of medical studies and the majority of physicians never encounter these types of pathology in their daily professional practice. Since its eradication, people are not trained to recognize smallpox or to make the differential diagnosis between anthrax and bronchitis.” Other chapters cover the effects of nuclear weapons and radiation on humans as well as the features of Gulf War syndrome. An important chapter deals with the organization of medical responses to chemical or biological attack: “Planning, equipping, and training responder services are the best responses to the dispersion of chemical and biological agents.” The book addresses all those involved in the security of the civilian population, the organization of rescue services and the treatment of victims.

In 1995, the Aum Shinrikyo cult released sarin onto the Tokyo subway system, killing 12 and injuring hundreds. In 2001, anthrax spores were sent through the U.S. Postal Service killing five postal employees and infecting 22 others. The increasing complexity and scale of these incidents suggest the possibility of a large scale attack with a Weapon of Mass Destruction (WMD) causing thousands of casualties on US soil. Since these attacks are difficult to prevent and may happen anywhere and at any time, mass decontamination is one of the key elements to managing the consequences of such an event, saving lives, and limiting the number of injuries. These Guidelines are an update of the United States Army Soldier and Biological Chemical Command (SBCCOM) - now the United States Army Edgewood Chemical Biological Center (ECBC) - January 2000 Guidelines for Mass Casualty Decontamination During a Terrorist Chemical Agent Incident and January 2002 Guidelines for Cold Weather Mass Decontamination During a Terrorist Chemical Agent Incident. These Guidelines supersede these original 2000 and 2002 Guidelines and are expanded to include all chemical, biological and radiological hazards. These Guidelines represent the latest evolution in our approach to mass casualty decontamination. These Guidelines were developed for first responders to provide information and suggested procedures for mass casualty decontamination following a hazardous material (HAZMAT)/WMD attack. There is no perfect solution to mass casualty decontamination and no single process or method can account for all variables (e.g., hazard, time, number of victims, environmental conditions, resources). These updated Guidelines are intended to identify a simple, consistent mass decontamination process that could be applied with reasonable effectiveness to any HAZMAT/WMD incident. In other words, use the fastest approach that will cause the least harm and do the most good for the majority of the people. These Guidelines are based on exposure to all hazards and focus on civilian mass casualty decontamination. These Guidelines primarily focus on chemical, biological, and radiological (CBR) agents, but also include Toxic Industrial Chemicals (TICs), Toxic Industrial Materials (TIMs), and toxins (collectively referred to in this document as HAZMAT/WMD). Guidelines do not cover each type of threat individually, however, the basic principles outlined are applicable to all HAZMAT/WMD situations. This document addresses decontamination of an overwhelming number of victims resulting from a HAZMAT/WMD incident in a population center. Mass casualty decontamination requires a slightly different approach than the individual technical and equipment decontamination applied during typical HAZMAT incidents. These Guidelines are designed for use during the first minutes of a mass casualty HAZMAT/WMD incident to reduce contamination and minimize casualties.
