Some human populations escaped tropical infections by migrating into cold climates but then had to procure fuel, warm clothing, durable housing, and crops from a short growing season. Waterborne infections were averted by owning your own well or supporting a community reservoir. Everyone got vaccines in rich countries, while people in others got them later if at all. Antimicrobial agents seemed at first to be an exception. They did not need to be delivered through a cold chain and to everyone, as vaccines did. They had to be given only to infected patients and often then as relatively cheap injectables or pills off a shelf for only a few days to get astonishing cures. Antimicrobials not only were better than most other innovations but also reached more of the world’s people sooner. The problem appeared later. After each new antimicrobial became widely used, genes expressing resistance to it began to emerge and spread through bacterial populations. Patients infected with bacteria expressing such resistance genes then failed treatment and remained infected or died. Growing resistance to antimicrobial agents began to take away more and more of the cures that the agents had brought.

Available in PDF, EPUB and Kindle. Book excerpt: Improving health is one of the main goals of water and environmental sanitation (WES) interventions. Despite this, many aid and development workers may have only a limited knowledge of the infections they try to prevent. Although the relevant information does exist, it is often scattered in specialised literature and rarely finds its way into the field. This manual addresses this problem by presenting information on these infections in relation to the interventions that fieldworkers typically control û i.e: water supply, sanitation, drainage, solid waste management, and vector control. It has been produced primarily for non-medical aid and development workers, but anyone working in WES, or in the prevention of infections related to WES, will find this book useful.

Book excerpt: As the human population grows-tripling in the past century while, simultaneously, quadrupling its demand for water-Earth's finite freshwater supplies are increasingly strained, and also increasingly contaminated by domestic, agricultural, and industrial wastes. Today, approximately one-third of the world's population lives in areas with scarce water resources. Nearly one billion people currently lack access to an adequate water supply, and more than twice as many lack access to basic sanitation services. It is projected that by 2025 water scarcity will affect nearly two-thirds of all people on the planet. Recognizing that water availability, water quality, and sanitation are fundamental issues underlying infectious disease emergence and spread, the Institute of Medicine held a two-day public workshop, summarized in this volume. Through invited presentations and discussions, participants explored global and local connections between water, sanitation, and health; the spectrum of water-related disease transmission processes as they inform intervention design; lessons learned from water-related disease outbreaks; vulnerabilities in water and sanitation infrastructure in both industrialized and developing countries; and opportunities to improve water and sanitation infrastructure so as to reduce the risk of water-related infectious disease.

Book excerpt: The rise in the incidence of infections is caused by multi drugresistant (MDR) bacteria, it is essential to elucidate the basic mechanism ofantibiotic resistance to discover effective methods for diagnosis and treatmentof infections. The use of pathogen-specific probes offers a faster alternative forpathogen detection and could improve the diagnosis of infection. High resolutionmelting analysis techniques are useful for the detection of multi drugresistant pathogens. Rational Structural Based Drug Design is a common methodto identify a lead compound and take it forward for further developments. This book provides information about recent strategies involved in thediagnosis and treatment of infections caused by MDR bacteria. The volume coversthe use of molecular probes for the quantification of pathogenic bacteria, alongwith other techniques mentioned above. Chapters also cover the use of identificationof novel drug targets from the Lipid A biosynthesis and also from quorum sensingmediated biofilm formation in MDR bacteria. Chapters also cover herbal alternatives for the treatment of MDRbacteria like the use of Cassiaaungustifolia in treatment of various diseases. The reference is suitablefor biomedical students, cellular and molecular biologists.

Book excerpt: The National Strategy for Combating Antibiotic Resistant Bacteria, published in 2014, sets out a plan for government work to mitigate the emergence and spread of resistant bacteria. Direction on the implementation of this strategy is provided in five-year national action plans, the first covering 2015 to 2020, and the second covering 2020 to 2025. Combating Antimicrobial Resistance and Protecting the Miracle of Modern Medicine evaluates progress made against the national strategy. This report discusses ways to improve detection of resistant infections and estimate the risk to human health from environmental sources of resistance. In addition, the report considers the effect of agricultural practices on human and animal health and animal welfare and ways these practices could be improved, and advises on key drugs and diseases for which animal-specific test breakpoints are needed.

Book excerpt: Environmental surveillance (ES) for SARS-COV-2 has been used in many locations to augment clinical data and support public health decision making. ES provides additional evidence on virus in circulation at population level such as presence or absence, early warning of increasing or decreasing trends, and information on variants of concern or interest. This guidance updates interim guidance published in April 2022 and provides advice on: What situations ES has been shown to add value to public health decision making. What is needed to plan and coordinate an effective ES programme. How to carry out data collection, analysis, interpretation and communication of results. This guidance is targeted at public health officials who want to understand and integrate complementary ES into COVID-19 control strategies.

Book excerpt: WASH in health care facilities is essential for quality care – on this there is universal consensus. Many countries are taking action, but more collaborative, focused and expansive effort and investments are needed. This report focuses on global and national efforts and progress to improve water, sanitation, hygiene (WASH), cleaning and health care waste management in health care facilities. It builds upon the first update of progress published in 2020. In addition, in response to demands from countries and health actors on providing a consolidated package of information and interventions on climate-resilient and sustainable infrastructure services in health care facilities, data on electricity and examples of electrification are included. It includes a summary of country progress in implementing national actions (“practical steps”) articulated in the 2019 World Health Assembly Resolution on WASH in health care facilities, drawing on data from a “country tracker” and provides insights on successful approaches and challenges from country experiences in improving and sustaining WASH in health care facilities. The main audiences for the report are national health authorities; those engaged in WASH and infection prevention and control (IPC) in health care facilities, including ministries of health, water, infrastructure, energy and financing; global and national WASH and health partners, including donors and health funders; and actors working on climate-resilient and low-carbon health care facilities and health systems.

Book excerpt: In 2015 the World Health Assembly endorsed the Global action plan on antimicrobial resistance calling on countries to develop and implement national action plans on antimicrobial resistance (AMR). For most countries, the greatest challenge is not developing a national action plan; rather, it is the implementation of the plan based on evidence-based prioritization of activities, systematic monitoring of progress, and ensuring sustainability of efforts. The purpose of this publication is to to provide a practical, stepwise approach to the implementation of the national action plan on AMR within the human health sector; and to provide a process and collation of existing WHO tools to prioritize, cost, implement, monitor and evaluate national action plan activities. The target audience of the publication are national/subnational stakeholders working on AMR within the human health sector. This includes national health authorities, national multisectoral coordination groups, senior technical experts and policymakers involved in implementing AMR activities at all levels of the health system, and implementation partners to accelerate sustainable implementation and monitoring and evaluation of national action plans on AMR.
