Book excerpt: In this part of the report, a first-principles based model is developed to predict the overpressure and temperature effects of a perforating orbital debris particle impact within a pressurized habitable module. While the effects of a perforating debris particles on crew and equipment can be severe, only a limited number of empirical studies focusing on space vehicles have been performed to date. Traditionally, crew loss or incapacitation due to a perforating impact has primarily been of interest to military organizations and as such have focused on military vehicles and systems. The module wall considered in this study is initially assumed to be a standard Whippletype dual-wall system in which the outer wall protects the module and its inhabitants by disrupting impacting particles. The model is developed in a way such that it sequentially characterizes the phenomena comprising the impact event, including the initial impact, the creation and motion of a debris cloud within the dual-wall system, the impact of the debris cloud on the inner wall, the creation and motion of the debris cloud that enters the module interior, and the effects of the debris cloud within the module on module pressure and temperature levels. This is accomplished through the application of elementary shock physics and thermodynamic theory. Their reviews have been timely and their contributions significant. The International Space Station (ISS) Safety Task Force was established to review a broad range of Station vulnerabilities and consequences. The National Aeronautics and Space Administration (NASA) Authorization Act of 2005 (Public Law 109-155) required the establishment of an independent task force to discover and assess any vulnerabilities of the ISS that could lead to its destruction, compromise the health of its crew, or necessitate its premature abandonment. The legislation further identified particular positions within and external to NASA that would serve as mandatory task force members. Having received this legislative direction, the NASA administrator charted the ISS Independent Safety Task Force (IISTF) as an advisory body on February 28, 2006. The IISTF's approach to the assigned tasks was two dimensional. First, the Task Force identified vulnerabilities (hazards) that could cause ISS destruction, compromise crew health, or necessitate the premature abandonment of the ISS. The Task Force reviewed the controls against these vulnerabilities, which included design requirements, safety controls, and procedural/operational controls. Second, the Task Force reviewed the ISS Program's crosscutting management functions consisting of plans, procedures, governing processes, and management processes that should provide advanced indications and warnings that will avoid events that might lead to destruction of the ISS, loss of the crew, or abandonment of the Station as well as avoid crew health problems. The identified vulnerabilities and IISTF scope are aligned into the following categories in accordance with the charter: Loss of crew member and/or loss of Station, Premature abandonment of the Station, Crew health. The ISS Program provided presentations on each of these vulnerabilities as well as the crosscutting management functions. The Program also responded to a number of actions from the Task Force members. The ISS Program's crosscutting management functions are described in Section 3. Treatment of threats to and vulnerabilities of the ISS is described in Section 4. Observations and recommendations are in Section 5, and the Task Force's conclusions and a compilation of its recommendations are listed in Section 6. To address the various aspects of its assigned tasks, the IISTF reviewed a large volume of technical material and information provided by the ISS Program.

The accident investigation that followed determined that a large piece of insulating foam from Columbia's external tank (ET) had come off during ascent and struck the leading edge of the left wing, causing critical damage. NASA responded to the CAIB findings and recommendations with the Space Shuttle Return to Flight Implementation Plan.1 Significant enhancements were made to NASA's organizational structure, technical rigor, and understanding of the flight environment. The ET was redesigned to reduce foam shedding and eliminate critical debris. In 2005, NASA succeeded in returning the space shuttle to flight. In 2010, the space shuttle will complete its mission of assembling the International Space Station and will be retired to make way for the next generation of human space flight vehicles: the Constellation Program. The Space Shuttle Program recognized the importance of capturing the lessons learned from the loss of Columbia and her crew to benefit future human exploration, particularly future vehicle design. The program commissioned the Spacecraft Crew Survival Integrated Investigation Team (SCSIIT). The SCSIIT was asked to perform a comprehensive analysis of the accident, focusing on factors and events affecting crew survival, and to develop recommendations for improving crew survival for all future human space flight vehicles. To do this, the SCSIIT investigated all elements of crew survival, including the design features, equipment, training, and procedures intended to protect the crew. Whereas the probability of debris impact and spacecraft penetration may currently be calculated, another parameter of great interest to safety engineers is the probability that debris penetration will cause actual spacecraft or crew loss. Quantifying the likelihood of crew loss following a penetration allows spacecraft designers to identify those design features and crew operational protocols that offer the highest improvement in crew safety for available resources. Within this study, a manned spacecraft crew survivability (MSCSurv) computer model is developed that quantifies the conditional probability of losing one or more crew members, P(sub loss/pen), following the remote likelihood of an orbital debris penetration into an eight module space station. Contributions to P(sub loss/pen) are quantified from three significant penetration-induced hazards: pressure wall rupture (explosive decompression), fragment-induced injury, and 'slow' depressurization. Sensitivity analyses are performed using alternate assumptions for hazard-generating functions, crew vulnerability thresholds, and selected spacecraft design and crew operations parameters. These results are then used to recommend modifications to the spacecraft design and expected crew operations that quantitatively increase crew safety from orbital debris impacts. Williamsen, J. E.
