Hilfen denen vermitteln und zukommen lassen, die auf derartige Leistungen ambulant (im eigenen Haushalt) oder in Institutionen und Organisationen angewiesen sind.“ Ich habe dieses Zitat von Lowy vor der eigentlichen Einleitung gewählt, weil ich finde, dass dies ein guter Einstieg in mein Thema Case Management ist. Meine Entscheidung fiel auf dieses Thema, weil ich mich für diese Form der Therapie schon länger interessiert habe. Allerdings wollte ich meine Hausarbeit zuerst über die Soziale Einzelhilfe schreiben und bin bei Recherchen zu diesem Thema auf die weiterentwickelte Form gekommen, nämlich auf das Case Management. Somit entschied ich mich um, da es mein Interesse weckte. Ich wollte erfahren, wie es entstanden ist und wie die Vorgehensweise ist. In meiner Hausarbeit möchte ich mich genau damit beschäftigen. Zuerst werde ich auf die Geschichte eingehen und anschließend werde ich näher erläutern wie die genauen Abläufe sind. Wie das Zitat von Lowy schon verdeutlicht ist das Case Management in vielen verschiedenen Handlungsfeldern zu finden. Eingesetzt wird es beispielsweise in der Altenhilfe, Suchtkrankenhilfe, Obdachlosenhilfe, Krankenhaussozialarbeit und in der Rehabilitation behinderter Menschen. Interessant ist, dass sich das Case Management den ständig wechselnden Erfordernissen und den Veränderungen anpassen kann. Der Grundsatz des Case Management bleibt, egal wo es speziell eingesetzt wird, immer der gleiche. Es verhält sich neutral zu seinen Anwendungen. Alles wird individuell angepasst, was in Deutschland zu den unterschiedlichen Bezeichnungen führt. In der Sozialarbeit heißt es „Unterstützungsmanagement“, im Gesundheitswesen „Fallmanagement“.

Zum Schluss der Arbeit wird anhand eines Fallbeispiels eine schwierige Erstgesprächssituation zwischen einer Klientin und Sozial Arbeiterin dargestellt. Einzelfallhilfe ist neben sozialer Gruppenarbeit und Gemeinwesenarbeit eine der drei grundlegenden sozialpädagogischen Interventionsformen zur Lösung psychischer, materieller, gesundheitlicher oder sozialer Probleme.
