Book excerpt: L'installation en médecine générale libérale est un pas important dans la vie de tout médecin généraliste, que beaucoup de jeunes médecins ont du mal à franchir depuis quelques années. Bien que la pratique de cabinet libéral soit le quotidien des médecins généralistes remplaçants, ces derniers ne font l'objet que de peu d'études sur la vision de leur installation. Une étude qualitative a donc été réalisée, sur la base d'entretiens semi-dirigés, ciblant des médecins remplaçants de la région centre. Ils ont choisi la médecine générale libérale pendant leur spécialisation. Tout d'abord attirés par le contact humain, si particulier à cette pratique, ils sont aussi animés par une volonté d'organiser leur travail en toute indépendance professionnelle. Malgré une satisfaction de leurs confrères actuellement installés, la gestion d'entreprise, les charges financières et administratives impressionnent. De plus, les conjoints se trouvent pour la majorité dans une situation professionnelle indéfinie (parfois précaire, compte tenu du marché du travail) alors que l'installation est souvent perçue comme définitive ou presque. Les remplaçants appréhendent l'installation, non pas par peur d'une évolution somme toute « naturelle » de leur métier, mais par peur d'une perte de qualité de vie, d'une privation de leur liberté, ne considérant plus la médecine générale comme le sacerdoce qu'elle pouvait être. Ils redoutent aussi la mise en place du Tiers Payant Généralisé, et l'alourdissement des charges du cabinet, ce qui peut constituer des freins. Le statut des remplaçants étant parfois stigmatisé par les patients et le suivi de ces derniers souvent difficile, la quasi-totalité, des remplaçants interrogés, souhaite tout de même s'installer. Plusieurs leviers, tels que le travail en cabinet de groupe allégeant les charges administratives, la télémédecine permettant la pratique dans les grandes villes ou bien encore la revalorisation financière de la consultation pourraient inciter plus de remplaçants à vouloir s'installer pour que les patients bénéficient d'une meilleure offre de soins.

Book excerpt: Introduction : Le nombre de médecins généralistes diminue et ce, probablement jusqu'en 2025, alors que nous notons une augmentation du nombre de remplaçants. Malgré les mesures incitatives actuelles, nous manquons de plus en plus de médecins généralistes installés et particulièrement dans notre région. Cette recherche a pour objectif d'obtenir les raisons de la non installation des médecins remplaçants. Méthode : Il s'agit d'une étude qualitative réalisée par des entretiens semi-dirigés individuels auprès de médecins généralistes remplaçants thésés champardennais. Le recrutement a été effectué jusqu'à saturation des données. L'analyse des données a été faite par regroupement thématique selon le principe de théorisation ancrée. Résultats : Seize médecins généralistes remplaçants ont été interrogés. Quatorze freins directs à l'installation ont été cités. Parmi ceux-ci, les plus souvent rapportés étaient la charge administrative, la charge de travail, le salaire par rapport au nombre d'heures de travail, la gestion du cabinet, le coût financier d'un cabinet et s'engager sur le long terme. Les participants ont également évoqué l'attrait du statut de remplaçant, qui donne des raisons supplémentaires au retard voire à la non installation des remplaçants : liberté de travail, revenu intéressant pour une charge de travail allégée, temps libre important, souplesse professionnelle pour s'adapter au travail du conjoint, mobilité, disponibilité pour une grossesse. Conclusion : Les raisons de la non installation sont donc multiple. D'un côté, nous avons des arguments considérés comme des inconvénients propres à l'installation et d'un autre côté, nous avons des arguments affirmant le confort du statut de remplaçant.
