Book excerpt: During the past decade, plague infections have persisted with undiminished importance in foci of the Americas, Africa, and Asia, while infections caused by the other yersiniae were recognized only during this decade as important agents of diarrheal and appendicitislike outbreaks in Europe, North America, and Japan. The dramatic rise of plague was abetted by the military conflict in Vietnam in the latter 1960s and persisted into the 1970s. During these years more Vietnamese people probably died of plague than American ser vicemen died of combat injuries. In the United States during these same years, the numbers of human cases of plague increased severalfold owing to well entrenched endemic foci in the sylvatic rodent species of the southwestern states., In the latter 1960s, microbiologists had changed the name of the plague bacillus from Pasteurella pestis to Yersinia pestis. The other two pathogenic species, Yersinia enterocolitica and Yersinia pseudotuberculosis, were known causes of mesenteric lymphadenitis and were believed to be rare. However, there originated a strong interest in these diseases in the European countries of Sweden, Finland, Belgium, and France. Yersiniae were frequently dis covered in persons with diarrhea and acute abdominal pain simulating ap pendicitis. This discovery sparked a worldwide surge of scientific interest in the genus Yersinia. Previous writings about the yersiniae have rarely considered plague and nonplague yersiniae together. Diseases caused by these bacteria, although their epidemiological patterns and clinical pictures are very different, have some striking similarities.

Book excerpt: This report presents evidence that when avirulent strains of P. pestis that produce the capsular antigen Fraction I are grown under conditions that induce a high degree of phagocytosis resistance in typical virulent strains, they also acquire the same degree of resistance. Virulent strains that do not produce detectable amounts of this antigen acquire a much more limited degree of resistance to phagocytosis; in fact, they are not at all resistant to ingestion by guinea pig macrophages. It is suggested that the major factors determing the virulence of P. pestis are not associated with resistance to phagocytosis, but rather, with the intracellular survival and multiplication of the bacilli following phagocytosis.

As the efficacy of new vaccines cannot be tested in humans, it is essential to develop in vitro surrogate assays that are valid predictors of immunity. The F1 capsule antigen stimulates a protective immune response to most strains of Y. pestis. However, strains of Y. pestis that are F1( - ) but still virulent have been isolated, and an in vitro assay, the results which can predict protection against both F1(+) and F1( - ) strains, is needed. The virulence antigen (V) is an essential virulence factor of Y. pestis and stimulates protective antibodies. We investigated potential correlates of plague immunity that are based on anti-V Ab-mediated neutralization of Yersinia-induced macrophage cytotoxicity. The neutralizing activity of sera from mice vaccinated with an F1-V fusion candidate vaccine was determined. The decrease in the level of the apoptosis-specific enzyme caspase-3 significantly predicted survival in one- and two-dose vaccination experiments. Sera from F1-V vaccinated nonhuman primates were evaluated with MPhis assays based on caspase-3 and on other markers manifested at the different stages in cell death. Using murine- and human-derived MPhis in microscopic and fluorescence activated cell sorting-based live/dead staining assays of terminal necrosis, we demonstrated a strong association between in vitro neutralization of macrophage cytotoxicity induced by sera-treated Yersinia and in vivo protection against lethal infection. These results provide a strong base for the development of reliable in vitro correlate bioassays which are predictive of protective immunity to plague.
