L'évolution historique de la profession infirmière en psychiatrie montre une grande instabilité de son statut et de son identité sociale. Nous proposons en premier lieu un rappel des concepts théoriques et méthodologiques des différentes disciplines étudiant la souffrance au travail suivi d'une revue de la littérature internationale sur les principales études menées auprès des infirmiers en psychiatrie. Nous présentons ensuite les résultats de notre étude, basée sur l'analyse qualitative d'entretiens semi-structurés menés auprès de dix infirmiers et infirmières travaillant en psychiatrie. Les principaux points que nous développons dans l'analyse des résultats sont : les effets subjectifs de la confrontation à la maladie mentale ; la violence ; la chronicité de la maladie ; les rapports dans le collectif soignant et la disparition de la formation et du diplôme spécialisé

Au-delà de la souffrance individuelle, la souffrance institutionnelle est également à prendre en compte. Ainsi, la santé au travail s'articule autour de conditions de travail confortables couplées à la possibilité d'épanouissement au travail. Les stratégies d'adaptation à la souffrance au travail peuvent viser à diminuer le problème, diminuer l'impact du ressenti négatif sur le soignant. Ces stratégies s'appuient sur le soutien social. Nous proposons dans ce travail des pistes de solutions pour la santé psychique des soignants en psychiatrie, tant sur le plan individuel que sur le plan collectif. Au CHU de Nantes, plusieurs projets sont en train de voir le jour.

La psychiatrie est considérée comme un lieu à haute prévalence de violence (Shield et Wilkins, 2009). Les patients souffrant de maladies mentales sont les plus à même de devenir violents (Pich et al., 2010). Ces phénomènes peuvent avoir de sérieuses conséquences sur les soignants (Roldan et al., 2013). La violence génèrerait un coût économique et humain considérable (Gadon, 2006). Objectif : Identifier les facteurs de risque et les moyens de prévenir la violence verbale et physique de patients hospitalisés en urgences psychiatriques envers le personnel soignant. Méthode : Critères d'inclusion : patients adultes (18 à 65 ans) admis en urgences psychiatriques. Type de résultats : facteurs de risque et outils/interventions. Deux bases de données ont été consultées : CINHAL et PsycINFO. Dix articles ont été inclus et analysés à l'aide de la Grille de Fortin. Les données relevées ont été classées en facteurs de risque non-modifiables, modifiables et outils. Résultats : Des facteurs de risque de violence comme le genre, l'âge, le diagnostic, le nombre d'hospitalisations, l'unité, la relation patient-soignant, les conflits ou la réactivité au stress ont été identifiés. Des interventions comme la prévision du risque, la formation du personnel, la médication involontaire, l'isolement, la contention ou la massothérapie ont cherché à réduire ces facteurs de risque. Discussion : Les résultats ont été éclairés par la théorie des relations interpersonnelles de Peplau. Rompre la relation patient-soignant pourrait engendrer de la violence. Pour préserver cette relation, des moyens existent.
