La maggior parte dei testi che riguardano la ventilazione meccanica (ancorché scritti e pubblicati da autorevoli professionisti del settore), dedicano uno spazio insufficiente a trattare in modo esaustivo le problematiche relative al buon funzionamento di un ventilatore meccanico. Essi attribuiscono maggior enfasi: alle diverse modalità di utilizzo dei ventilatori meccanici nei vari stadi della malattia; agli effetti fisiologici della ventilazione meccanica; allo svezzamento dal ventilatore; alle vie aeree artificiali e ai loro vari componenti. Nella maggior parte dei testi in oggetto, vengono utilizzare parole e frasi ingannevoli, con significati poco chiari e fuorvianti. Questo testo si prefigge di presentare i concetti di base della ventilazione meccanica, partendo da un approccio alla conoscenza del ventilatore meccanico che permetta di individuare le reali potenzialità e i limiti e di tradurle in opportunità di cura e di guarigione per il paziente. E d'uopo notare che soltanto la perfetta conoscenza di un qualsiasi tipo di strumentazione potrà dare l'esatta percezione delle sue peculiarità e del suo corretto utilizzo. Il testo è un avvincente percorso attraverso i ventilatori meccanici che si snoda da una base teorica alla pratica applicata. Questo libro è rivolto idealmente ad un'ampia fascia di lettori, che va dai "veterani" della ventilazione meccanica, a chi da poco si approccia a questo mondo a dir poco affascinante. Per questa presupposta eterogeneità di fruitore si è ritenuto di dettagliare episodicamente alcuni concetti, fondamentali per taluni, ritenuti sicuramente superflui per gli altri. L'autore essendo supportato da una solida base di studi ingegneristici, per deformazione professionale ha ritenuto di ben evidenziare i concetti fisici ed ingegneristici che sottointendono i temi trattati, per meglio chiarificarne le dinamiche e le problematiche ad essi relativi.

After an initial chapter on the history of thoracoscopy, the next three chapters of the book are devoted to the general technique of performing thoracoscopy, a topic on which the team of Dr. Boutin and his colleagues write with ample experience. In these chapters they provide detailed de scriptions of how to perform the procedure and discuss the pros and the cons of the different approaches and the different equipment available. The following chapter discusses the complications of the procedure. The next chapter discusses the role of thoracoscopy in diagnosing pleu ral effusions. My own opinion here is in general that, as long as well trained personnel are available, thoracoscopy is preferable to open pleural biopsy, because it is associated with less morbidity and mortality and can be done if necessary under local anesthesia. The following chapter is directed at using to thoracoscopy to facilitate pleurodesis for chronic pleural effusions. With Boutin's method, talc pou drage is performed during thoracoscopy, and certainly, if a thoracoscopy is used to establish the diagnosis of a malignant pleural effusion, it is appro priate to attempt to induce a pleurodesis with talc at the time of the diag nostic procedure.

The plausibility of these criteria is undermined as soon as one confronts the question of the level of treatment that ought to be provided to human bodies that have permanently lost consciousness but whose brain stems are still functioning.
