Les patients douloureux chroniques semblent insatisfaits. La réponse du médecin pourrait être différente de leurs besoins. Nous avons voulu connaître le vécu des patients douloureux chroniques et leurs attentes vis-à-vis de leur médecin généraliste. Matériel et Méthode : Étude qualitative d'entretiens compréhensifs, semi-dirigés, jusqu'à saturation des données, par double analyse de contenu. Échantillon raisonné diversifié. Résultats : Quatorze entretiens ont été menés en région parisienne auprès de patients douloureux chroniques, depuis 5 mois à 60 ans, âge moyen de 53 ans, diversité d'âge et de milieu socioprofessionnel. Les personnes vivaient leur douleur dans un sentiment d’isolement et de stigmatisation. Elles craignaient une étiologie psychiatrique attribuée à leur pathologie. Elles attendaient des médecins de la coordination avec les autres intervenants, l'écoute, une prise en considération de leur douleur et souhaitaient avoir un rôle actif dans leur prise en charge. Conclusion : Le vécu de la douleur chronique génère des besoins d'écoute, de prise en considération et d'avoir un rôle actif dans la prise en charge. Le médecin traitant était identifié dans un rôle de coordination. Les capacités compréhensives et communicationnelles des praticiens pourraient être améliorées. Le modèle biopsychosocial, une approche phénoménologique ou analytique des personnes douloureuses chroniques pourraient améliorer la prise en charge.

Objectif : l'objectif principal de cette étude est d'analyser le vécu, les attentes et les représentations de patients douloureux chroniques à l'égard des professionnels de santé dont le médecin généraliste et les structures spécialisées dans le traitement de la douleur chronique, afin de recueillir les besoins et les difficultés rencontrés pour ainsi proposer des pistes d'amélioration. Méthode : Etude qualitative phénoménologique par entretien individuels semi-dirigés auprès de treize de patients adultes douloureux chroniques depuis au moins un an, habitant en Ille-et-Vilaine ou en Côte d'Or. L'analyse des données a été codée manuellement selon un modèle inductif et phénoménologique. Résultats : Les principales attentes des patients douloureux chroniques vis-à-vis des médecin, professionnels de santé et des structures dédiées sont : être soulagé, nouer une relation de confiance, être écouté et aussi compris, soutenu, informé, respecté et bien orienté. Être acteur des soins avec une bonne visibilité sur le parcours de soins dans une démarche de partenariat fait également partie des attentes des patients. Quant aux difficultés vécues, les plus fréquemment énoncées sont : le manque de reconnaissance des douleurs, d'écoute, de considération en tant que personne, de temps alloué et de respect de leurs décisions. Aussi, le discours des soignants est parfois qualifié d'anxiogène et culpabilisant. Enfin, l'information concernant les traitements de la douleur et l'accessibilité aux structures spécialisées peuvent également être améliorées. Discussion : Comme pour d'autres pathologies chroniques, le parcours des patients est long et complexe. Le médecin généraliste a un rôle essentiel, il doit pouvoir s'appuyer sur les médecins spécialistes et si nécessaire sur les structures spécialisées. Les patients doivent faire face à de multiples difficultés, ils comptent sur le médecin et les soignants pour les écouter, les soutenir et les soulager. Pour améliorer leur parcours de soins, les patients suggèrent de sensibiliser les médecins à travers leur formation universitaire sur le traitement de la douleur chronique et la relation médecin-patient. Le développement de la visibilité et de l'accessibilité aux structures doit être encouragés.

Les données ont été recueillies par des entretiens individuels semi dirigés jusqu'à saturation des données. L'analyse a été thématique, en double codage. Les témoignages d'un échantillon raisonné et diversifié de 18 patients ont été enregistrés. Le ressenti de la douleur et du handicap associé affectait la vie familiale, professionnelle et relationnelle. Une adaptation de la vie quotidienne était nécessaire. Les patients se sentaient isolés tant au niveau relationnel qu'administratif. Ils attendaient un savoir-faire technique mais aussi relationnel et de coordination de la part de leur médecin traitant. La prise en charge incluait fréquemment un médecin spécialiste et un kinésithérapeute mais aussi diverses médecines parallèles. Les patients cherchaient à se renseigner sur leur pathologie et quand le diagnostic était posé, il a permis une meilleure acceptation de la douleur. Les traitements bien que parfois craints ou peu efficaces étaient jugés nécessaires. Pour les patients, les éléments essentiels d'une bonne prise en charge étaient un examen clinique attentif, une écoute empathique, une information adaptée, un rôle central du médecin traitant et une considération de l'état psychologique, de l'environnement familial et professionnel.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
