Revascularization procedures such as coronary artery and peripheral bypass grafts, as well as access surgery represent a 2$ billion market yearly for the US alone. Despite intense research over many decades, no clinically suitable, shelf-ready, synthetic, vascular, small-caliber graft exists. There is therefore still a quest for such a clinical vascular prosthesis for surgical revascularization procedures and access surgery. Many approaches have been tried and are currently under investigation with promising results. These range from acellular and cell-based, stable or bio-degradable, synthetic scaffolds to biological or decellularized grafts, not forgetting self-assembly technologies for in vitro or in vivo VTE. All these approaches can be further enhanced by functionalization, e.g. with growth factors and drug elution. This updatable book aims to cover all the relevant aspects of Vascular Tissue Engineering (VTE) and novel alternatives to develop vascular grafts for clinical applications. The chapters in this book cover different aspects of manufacturing scaffolds with various polymers, mechanical characteristics, degradation rates, decellularization techniques, cell sheet assembly, 3-D printing and autologous mandril-based VTE. All the necessary in vitro tests such as biocompatibility and thrombogenicity are reviewed. Pre-clinical assessment of in vivo experimental models include patency, compliance, intimal hyperplasia, inflammatory reaction, cellular ingrowth and remodeling. Finally, early clinical trials will be periodically updated regarding results, regulatory aspects and post-marketing quality assessment. Furthermore, the reader should get an insight into various approaches, technologies and methods to better understand the complexity of blood surface and cell interactions in VTE. Translational research has yielded early human applications clearly showing the enormous need of research in the field to provide better solutions for our patients and this continuously updated book will hopefully become a reference in the field for life sciences.

Despite rapid advances in all imaging modalities. there has been no serious challenge to the role of bone scanning in the evaluation of the skeleton. The main reason for this is the exquisite sensitivity of the bone scan for lesion detection. combined with clear visualisation of the whole skeleton. In recent years several new diphosphonate agents have become available with claims for superior imaging of the skeleton. Essentially. they all have higher affinity for bone. thus allowing the normal skeleton to be visualised all the more clearly. However. as will be dis cussed. this may occur at some cost to the principal role of bone scanning. lesion detection. The major strength of nuclear medicine is its ability to provide functional and physiological information. With bone scanning this leads to high sensitivity for focal disease if there has been any disturbance of skeletal metabolism. However. in many other clinical situations. and particularly in metabolic bone disease. more generalised alteration in skeletal turnover may occur. and quantitation of diphosphonate uptake by the skeleton can provide valuable clinical information.

Neurosurgery, ophthalmology and otorhinolaryngology have already integrated the binocular mono scope as an indispensable tool into their armamentarium. General surgery is also making increasing use of it-as is the case with transplantations of autogenous free intestinal grafts. A great "victory march of microsurgery", however, appears to be taking place in plastic and reconstructive surgery. Reimplantation of arms, hands, and especially digits are hardly thinkable today without the possibility of high magnification. New microsurgical anastomosing techniques (interfascicular neurorrhaphy) have also improved the func tional results after nerve injuries. Time-consuming reconstructive proce dures, such as the transfer of a distant flap with its physical and psycho logical burden, are being increasingly replaced by free flaps transplanted by microvascular suture techniques-the most popular free flap being the groin flap in which the superficial circumflex iliac artery and vein is reanastomosed to the recipient vessels. Nevertheless, the writer was sceptical when Dr. TSCHOPP set about with his ambitious plan to transplant a free autogenous rib graft in combination with its adjacent intercostal musculature. The evidence of the results obtained has given the lie to my scepticism. Dr. TSCHOPP has not only succeeded in securing the complete arterial and venous supply to the musculoskeletal graft at the recipient site but has also managed to preserve the functioning entity of nerve and muscle.
