The pathogenesis of glaucomatous optic neuropathy remains poorly understood, and several pathogenic mechanisms are proposed to coexist. As the world population ages, OAG will become more prevalent and advances in the diagnosis and treatment of glaucomatous optic neuropathy are important to protect and improve the quality of life of our aging population. Treatment of OAG has been directed at lowering intraocular pressure (IOP) which is the only current therapeutic strategy available to patients with glaucoma. While a wide variety of studies have demonstrated that lowering IOP decreases the risk of glaucoma development and/or progression, many studies have also shown that some patients continue to lose vision despite significant lowering of IOP. There have been many attempts to elucidate the etiology for the deterioration in glaucomatous optic neuropathy despite low levels of IOP. Over the past several decades, deficits in the ocular circulation of patients with OAG have become well established and these may explain the continued progression of OAG patients despite lowered IOP. The purpose of the present publication is to provide an updated view of ocular blood flow and vascular dysregulation in OAG. The importance of the topic was demonstrated by the focus of the 2009 6th Consensus meeting of the World Glaucoma Association which focused entirely on blood flow deficits in patients with OAG. Although a great deal of knowledge on vascular risk factors in glaucoma has already been established, many questions remain. Do ocular blood flow deficits precede glaucoma progression? How does ocular perfusion pressure fit into the IOP and blood flow paradigm? What conclusions can be drawn from recent evidence showing the fluctuation of OAG risk factors including IOP, blood pressure and ocular perfusion pressure? We hope this update d current prospective will serve as a foundation for future investigations which will be designed to answer these and other important considerations in the management of glaucoma. The needs for an unobstructed light path from the cornea to the retina and a relatively fixed corneal curvature and distance between refractive structures pose significant challenges for the vasculature to provide nutrients and remove metabolic waste. To meet these needs, the ocular vascular beds are confined to the periphery of the posterior two thirds of the eye and a surrogate circulation provides a continuous flow of aqueous humor to nourish the avascular cornea, lens and vitreous compartment. The production of aqueous humor (and its ease of egress from the eye) also generates the intraocular pressure (IOP), which maintains the shape of the eye. However, the IOP also exerts a compressing force on the ocular blood vessels that is higher than elsewhere in the body. This is particularly true for the intraocular veins, which must have a pressure higher than IOP to remain patent, and so the IOP is the effective venous pressure for the intraocular vascular beds. Consequently, the ocular circulation operates at a lower perfusion pressure gradient than elsewhere in the body and is more at risk for ischemic damage when faced with low arterial pressure, particularly if IOP is elevated. This risk and the specialized tissues of the eye give rise to the fascinating physiology of the ocular circulations.

There has been only sparse attention to the diagnosis and treatment of childhood glaucoma. Both pediatric ophthalmologists and glaucoma specialists provide care for such children. In some instances, they manage these individuals alone and, in others, the management is shared. For this consensus, the participation of both groups was solicited. The global faculty, consisting of leading authorities on the clinical and scientific aspects of childhood glaucoma, met in Vancouver on July 16, 2013, just prior to the World Glaucoma Congress, to discuss the reports and refine the consensus statements. As with prior meetings, it was a daunting task to seek and obtain consensus on such a complicated and nuanced subject. It is unclear how each of us decides how we practice, and evidence to guide us often is sparse. It is remarkable how few high level studies have been conducted on the management of childhood glaucoma. Hence, this consensus, as with the others, is based not only on the published literature, but also on expert opinion. Although consensus does not replace and is not a surrogate for scientific investigation, it does provide considerable value, especially when the desired evidence is lacking. The goal of this consensus was to provide a foundation for diagnosing and treating childhood glaucoma and how it can be best done in clinical practice. Identification of those areas for which we have little evidence and, therefore, the need for additional research also was a high priority. We hope that this consensus report will serve as a benchmark of our understanding. However, this consensus report, as with each of the others, is intended to be just a beginning. As science expands, the emphasis is on data, interpretation, and dogma. We disagree; open minds open new approaches. Using methodologies that are primarily molecular and genetic, we seek to refine the causes of glaucoma as well as how it is best treated, especially incorporating thoughts and hypotheses about new methods of treatment. Glaucoma is a complex disease, and genetics proves that a variety of proteins are culpable at one level. At another level, however, there are likely final common pathways and numerous feedback loops which have defied explanations to date. The search for answers goes on in basic science researcher’s laboratories and clinical ophthalmologist’s offices and operating rooms. We are all well-served by understanding that glaucoma is a neurodegenerative disease. Current attempts to solve the disease have focused on two strategic arenas: the trabecular meshwork function and its impact on intraocular pressure as a major risk factor for the disease; and the optic nerve dysfunction leading to visual loss. Genetic mutations have yielded puzzling clues to the cause, but without resolution. For example, mutations in myocilin and optineurin genes are closely connected to the phenotype, but how do they cause the disease? In the next two years, priority areas of research are signaling pathway discoveries, biomarker panels, epigenetic factors, and continued genomic studies to yield answers to the common final pathways of the disease. The final pathways are complex and redundant, such that the overlap of bio-informatics will be challenging. Current promising leads suggest the innate immune system holds important clues to both trabecular meshwork and optic nerve pathophysiology. When the primary open-angle glaucoma disease pathways are unraveled, drug discoveries and new treatment modalities will be available for better regulation of intraocular pressure and neuroprotection for the optic nerve. This volume discusses the glaucoma pipeline from several perspectives as well as future candidate classes.
