Non-urgent visits to the ED result in financial and health related problems for those who utilize the ED in this manner, as well as managed care organizations. There are many influences included in patients' decisions to utilize the ED for non-urgent reasons. The purpose of this research was three fold. The first purpose was to identify reasons for utilizing a southwest Florida Emergency Department rather than other healthcare services for non-urgent care. The second purpose was to identify common factors among patients seeking non-urgent care in the emergency department in a southwest Florida rural emergency department. The third purpose was to determine if patients who visit a southwest Florida Emergency Department for non-urgent reasons receive less preventative care than national standards.

Previous studies note that annual ED visits have increased over time, while reimbursement for emergency care by insurers has steadily decreased. These challenges are magnified in rural areas, which typically have fewer health care resources, including medical staff, facilities, adequate financing, and modern technologies. More information is needed on the patient and hospital attributes of rural ED visits to further improve the emergency and overall care of rural patients. This Statistical Brief presents data on the use of EDs in rural areas from the Healthcare Cost and Utilization Project (HCUP) Nationwide Emergency Department Sample (NEDS). The information focuses on patient and hospital characteristics of rural and non-rural emergency department visits in the United States in 2008. Variations in the most frequently listed conditions for adults and children are also discussed. All differences between estimates noted in the text are statistically significant at the 0.05 level or better.

Book excerpt: The use of the emergency department (ED) by non-emergent patients is a costly expense within the healthcare industry. A potential cause of this issue may be related to the availability of primary health care providers (PHCPs) to the patient. A review of literature of this topic produced a minimal return for ED utilization and it’s relation to PHCP availability. The Andersen Behavioral Model was used as the framework for this study which investigated the question of; what relationship does primary health care provider (PHCP) availability have on the utilization of EDs by non-emergent patients, ESI triage level 4 and 5, between the ages of 18 and 49? A total of 101 surveys were collected over a 9 day period. This data was then analyzed to determine if there was a correlation between PHCP availability and non-emergent patient visits to an emergency department (ED). It was determined that there was a mild positive correlation between the presence and availability of primary health care providers and utilization of the emergency department by non-emergent patients. Petitioning for full practice authority for nurse practitioners in Pennsylvania may help to reduce this lack of availability of PHCPs.

EDs in urban areas are overcrowded and contend with large amounts of patients with non-urgent complaints. To identify reasons for seeking care, a convenience sample of 100 non-urgent patients (ESI triage acuity 4 and 5) presenting to one Midwestern, urban ED was used. This quantitative, descriptive study utilized a 14 item self-administered questionnaire to collect demographic data and to ask a series of closed and open-ended questions about ED use. Specifically, the questions inquired about a patient's usual sources of care, decision-making, and perceived urgency of illness. The Andersen framework of health services utilization was employed to analyze patients' predisposing factors, enabling/impeding factors, and needs for care. Understanding reasons for non-urgent ED use is important to future hospital planning for optimal patient outcomes and effective resource utilization. The results of this study showed patient's seeking care in the ED for non-urgent complaints did so because of a perceived sense of urgency about their condition, a desire for the best care, and the convenience/accessibility of the facility. While a large portion of the sample did not have a primary care provider (PCP), insurance/PCP status were not found to be a significant factor.

Book excerpt: The object of this study was to determine the percentage of patients seen in the WBAMCER for conditions triaged as nonurgent during a seven-month period from June to December in 1998. Additionally, the study, utilizing pre-recorded data, desired to determine if 12 independent variables were associated with nonurgent utilization of the ER. The registered nurses on duty triaged a total of 2,173 patients that met inclusion criteria. A large number (n=1260, 58% of sample) were triage to level 4 (nonurgent). Patients triaged to level 3 (treat within 24 hours) accounted for 37.8% (n=821). The remaining level two (urgent) and level one (emergent) accounted for nearly equal percentages of the population at 2.3% (n=49) and 2.0% (n=43) respectively. The typical profile of ER utilization by frequency showed the greatest usage by female active duty dependents between the ages of 18-35 who are CHAMPUS eligible and enrolled in TRICARE Prime. This profile also depicts the typical user as someone who presents with a sprain or strain on a weekday during the hours of 0700-1500. This profile is further typified by a person or sponsor between the rank of E5 and E9 with no third party insurance, lived in zip code 79924 (6 miles from WBAMC) and released without limitation after being treated in the ER. This study strengthens and quantifies the long-standing belief among practitioners and administrators regarding the prevalence of nonurgent use in the ER at WBAMC. These nonurgent patients may be candidates for referral to a more appropriate level of care, typically primary care, without a significant risk for deterioration of their condition. Identifying and quantifying this population is the first step in shaping important organizational initiative aimed at self-care education, utilization of telephone advice and triage, and other out patient medical management programs.

The problem of ED overcrowding is truly a multifaceted one, even definition of "overcrowding" has multiple interpretation depending on the authors. Whatever the correct definition the issue at hand is that truly emergent patients lives are at risk, if no intervention is made within one hour of onset of symptoms, those patients will die. Appropriate triage, patient education and redirecting non-urgent complaint to different areas of Emergency Department staffed with Nurse Practitioners or Family Medicine Physicians may be an answer to long term ED overcrowding. Having a fast track area in or next to Emergency Department may be financially sound as well. Unnecessary medical testing and occupancy of ED beds by non-emergent patients increase healthcare spending (Adesara, Spencer and Bost, 2011). By reorganizing patient flow after triage and directing level 4 and 5 patients to separate area of the ED would decrease main ED wait times, allow ED nurses and physicians to concentrate their efforts on patients who would have adverse outcome if their respective health issue is not addressed during first "golden" hour. It will also allow for appropriate patient education by case managers and social workers providing health teaching and follow up referrals resulting in continuity of care for those non03́emergent patients using ED for primary care concerns.
