Patients experience anxiety related to the uncertainty of any surgical procedure. Increased levels of pain and anxiety can lead to negative effects both physiologically and psychologically. Narcotics and sedatives are most commonly administered to alleviate both of the conditions. When given in large amounts and coupled with anesthetic drugs, adverse effects such as over-sedation can occur. This is an adverse effect that can be problematic and costly for not only the patient, but the nurse and facility as well. Respiratory depression and over sedation go hand-in-hand. When over sedation occurs, a patients respiratory effort can be greatly impacted. Over-sedation and other adverse effects can prolong not only the recovery period, but ultimately delay patient discharge. By adding a non-pharmacologic intervention such as music therapy to the treatment plan, the nurse will be able to reduce pain and anxiety, thus reducing the amount of narcotics and sedatives required. This can be used as a nursing driven intervention to be implemented in the pre-operative phase and follow the patient through the post-anesthesia phase. With the appropriate implementation plan, stakeholder acceptance, and resources, music therapy can be a beneficial influence on physiological parameters and psychological state. Music therapy can provide relaxation, lessen muscle tension, and provide a healing influence for patients. Music therapy can have a great impact on not only patient satisfaction, but anesthesia outcomes as well. Literature has shown that music therapy used as a complementary therapy has been proven as an inexpensive way to reduce perioperative pain and anxiety.

Book excerpt: The use of music as an intervention in the perioperative setting may be a beneficial tool in decreasing anxiety and pain associated with the surgery. The patient facing a surgical procedure may have anxiety related to the surgery itself, the physical challenges post-surgery, financial concerns, and the prospect of postoperative pain. The implementation of music in the perioperative setting is a cost-effective, non-invasive, non-pharmacologic intervention without adverse effects. The current preoperative procedure does not include the provision of a music intervention. This proposal utilizes the use of portable music players and headphones for patients in the perioperative setting. The staff will receive instruction on the intervention, questions and concerns will be addressed prior to induction of the program. The music intervention will be employed preoperatively, can be used intra-operatively pending the type of surgical procedure, and post-operatively and its effectiveness will be measured by using the Visual Analog Scale (VAS). The use of music has been shown in previous research as beneficial in decreasing anxiety and pain, leading to lessened use of pharmacologic interventions. Both staff and patients will be queried regarding their opinions on the intervention and its benefits or limitations. The collected data will be analyzed and the appropriateness, cost-effectiveness, and impact on patient satisfaction will be determined by upper management, surgery co-management, and the anesthesiology group. Further dissemination of the evidence will be made to additional departments, appropriate organizations, and associations.

One of these therapies include the use of music, as literature has found that it reduces anxiety in patients undergoing surgical procedures that require sedative0́ ̄medications. In patients undergoing cataract surgery, the use of music during the procedure has been found to have a positive impact on patients, as their anxiety levels have been reduced. As a result, this quality improvement project focused on the reduction of anxiety with the use of music therapy, in patients undergoing cataract surgery in an outpatient surgical facility. Using the Hamilton Anxiety (Ham-A) scale, the anxiety level of0́ ̄104 patients was measured pre-and post-operatively. Overall, findings indicated that most of the Ham-A scores of the patients had decreased, and these patients also documented in the "Yes/No 1-Question Survey" that music therapy was therapeutic in reducing their anxiety0́ ̄from the pre-operative period to the time of discharge from the post-anesthesia care unit. However, 10 patients had no change in their anxiety level pre- and post-operatively.
