Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Drawing on interviews with one hundred men and women who have lost jobs across Pennsylvania, Sarah Damaske examines the ways unemployment shapes families, finances, health, and the job hunt. Damaske demonstrates that commonly held views of unemployment are either incomplete or just plain wrong. Shaped by a person’s gender and class, unemployment generates new inequalities that cast uncertainties on the search for work and on life chances beyond the world of work, threatening opportunity in America. Following in depth the lives of four individuals over the course of their unemployment experiences, Damaske offers insights into how the unemployed perceive their relationship to work. She reveals the high levels of blame that women who have lost jobs place on themselves, leading them to put their families’ needs above their own, sacrifice their health, and take on more tasks inside the home. This “guilt gap” illustrates how unemployment all too often exacerbates existing differences between men and women. Class privilege, too, gives some an advantage, while leaving others at the mercy of an underfunded unemployment system. Middle-class men are generally able to create the time and space to search for good work, but many others are bogged down by the challenges of poverty-level unemployment benefits and family pressures and fall further behind. Book excerpt: This monograph began as a study of the consequences of labor force effects, in cluding unemployment, for the distribution of earnings. I began by developing a model of job search. But following my previous work on the distribution of earnings, the search theory took a different form from the standard literature. Workers and firms were engaged in mutual search which effectively assigned workers to jobs. A number of open questions immediately became apparent, including the relation bet ween unemployment and inequality, the nature and costs of unemployment, and the role of choice. These quickly provided sufficient material for the monograph. I began work on the project in 1980 at Miami University of Ohio. I wish to thank my chairman there, William McKinstry, for the support I received during my last year there. My colleagues Donald Cymrot and James Moser provided some early com ments on the project and I am indebted to Joseph Simpson for extensive computer assistance.

The authors document that the disproportionate increase in the unemployment inflow rate for men caused this discrepancy. Men are more concentrated in the sectors that were hit hardest by the recession. Men have flown into unemployment from non-participation contributing to men¿s higher unemployment rate. When a resulting reduced–form relationship is estimated, its explanatory power is surprisingly high: on average, it explains about three quarters of the variation in inequality measures for the OECD countries, and Granger Causality tests confirm the model’s predictions.

There is wide agreement across the political spectrum that high inequality is contributing to undesirable circumstances such as stagnant household income, rising poverty rates, and increased borrowing and debt, though there is much less agreement on remedies. Inequality in America provides a snapshot of the issues posed by the growing concentrations of income, focusing on the United States but drawing on international comparisons to help set the context. The authors examine the economic, technological, and political drivers of inequality and identify worrying trends associated with its rise. They demonstrate how specific factors have exacerbated income inequality, including technological change, international trade, changes in labor market participation, and the increasing role of the financial sector. Their clear and concise exposition makes the issues surrounding income distribution accessible to a wider public. As they write in the conclusion: "We have argued that tackling the worst effects of inequality and re-establishing a measure of equal opportunity requires increased investment in crucial public goods: first, education; second, a more progressive and simplified tax system; and third, increased international cooperation to avoid a race to the bottom. Education, tax, and other such policies are pursued by other highperforming advanced countries and can be shaped for the United States in a way that is fully consistent with an efficient and competitive American economy."
