Book excerpt: While the process of obtaining an Autism Spectrum Disorder (ASD) diagnosis for a child can involve several challenges for parents (Keenan, Dillenburger, Doherty, Byrne, & Gallagher, 2010), this process may be even more complex for immigrant parents (Begeer, El Bouk, Boussaid, Terwogt, & Koot, 2009; Jegatheesan, Fowler, & Miller, 2010). This qualitative study utilized an eco-cultural framework (Super & Harkness, 1986) to explore the experiences of eight immigrant families during their process of obtaining a diagnosis and services for their children with ASD. Each family participated in two in-depth, semi-structured interviews (45-90 minutes) with many open-ended questions to permit parents to recount their story of obtaining the ASD diagnosis and services for their children. Using open and axial coding, the data were analyzed inductively and deductively to reveal various themes related to the diagnosis, treatment services, relationships with professionals, and future expectations for their children. Given that Montreal is such a highly multicultural metropolis (Statistics Canada, 2007), it is important that professionals recognize how parental beliefs about the ASD diagnosis could shape their treatment decisions and adherence (Dale, Jahoda, & Knott, 2006). Data from this qualitative study support the importance of adopting a culturally responsive practice when working with families by considering families’ perceptions and the core meaning of their experiences (Harry, 2002). That is, a culturally responsive practice helps to ensure that parents are given a voice, permits professionals to understand how parent perceptions may be formed, and allows for professionals to revisit any relevant problems that may interfere with parents’ negotiation of services.

Some of these interventions are evidence-based; others are not. However, previous research has also found that parents select interventions for their children with ASD on the basis of several factors other than the evidence base, including their emotional representations of ASD and their perceptions of the timeline, consequences, and causes of ASD. Other research has found that people's cultural worldviews influence their decision-making on a variety of social and political issues. The current study examined the relationships between these factors and parents' perceptions of the efficacy of evidence-based and non-evidence-based interventions. Contrary to previous research, for parents of children with ASD, perceptions of intervention efficacy were not affected by their emotional representations of ASD or their perceptions of the timeline or consequences of ASD. There was a relationship between parents' perceptions of the causes of ASD and their perceptions of the efficacy of some interventions. Parents' cultural worldview was not related to their perceptions of the efficacy of evidence-based interventions, but was related to their perceptions of non-evidence-based interventions. Further research is needed to determine other factors that influence how parents perceive ASD interventions.

Book excerpt: Prevalence rates for autism spectrum disorder (ASD) suggest rates continue to increase, with recent data indicating 1 in 68 children diagnosed with the disorder. Interventions may prove beneficial for both the core symptoms of the disorder and related deficits, although data indicate that effects are stronger with early intervention (e.g., prior to four years old). Unfortunately, many children are undiagnosed until they are school age. Additionally, a number of findings indicate that in comparison to Caucasian children, African American children receive an ASD diagnosis later and less often. The current study examined the role of race/ethnicity on 119 mothers' knowledge about autism spectrum disorder, interpretation of symptomology consistent with ASD (e.g., labeling the child, level of concern, helpful services), attitudes towards mental health services, and trust in providers. Participants were exposed to a vignette of a child displaying typical developmental behavior, mild ASD symptomology, or moderate-to-severe ASD symptomology. Participants indicated their perception of the child's behaviors either as indicative of normal development, a medical disorder, a developmental disorder, an intellectual disorder, or an emotional disorder. Participants then completed measures of ASD knowledge, help seeking attitudes, and provider stigma. In comparison to Caucasians, African Americans displayed less ASD knowledge, were less able to identify a child with ASD symptoms as having the disorder, and held more biases toward medical clinicians. Inconsistent with previous studies, racial/ethnic differences were not found for attitudes of mental health stigma and help seeking intentions. Implications of findings are discussed

Book excerpt: Autism spectrum disorder (ASD) is a developmental disability that affects 1 in 54 children in the United States. This developmental disability impacts an individual's behavior, communication patterns, and social interactions. ASD strikes 17.9 per 1,000 Asian American and Pacific Islander children in the United States. A dearth of knowledge exists about ASD within the Filipino American community. Shame and stigma related to ASD negatively affect how Filipino Americans perceive the developmental disability, and contribute to underreporting and delayed intervention. Grounded theory was utilized to explore Filipino American cultural beliefs and perceptions about ASD, describe how Filipino American parents manage care for children and adults with ASD, and generate an explanatory framework about processes involved in Filipino American parental management of care for children and adults with ASD. Social constructionism was used as a theoretical framework and symbolic interactionism was used as a philosophical underpinning. Research participants partook in a personal interview to capture cultural beliefs and perceptions about ASD using Kleinman's Explanatory Model of Illness and explore parental management of children and adults with ASD. Demographic information and levels of acculturation and parent adjustment to illness were also collected. Coding processes, memos, reflexive journals, diagrams, and field notes were utilized for data analysis. Eighteen participants enrolled in the research study. Traditional Filipino cultural values and generational differences influenced beliefs and perceptions about ASD. Causal beliefs of ASD were attributed to environmental factors, medical conditions, pharmaceutical products, prenatal events, genetic history, and/or superstitious beliefs. Management of ASD involved recognizing signs and symptoms of ASD, seeking help, learning about ASD, organizing daily schedules, integrating into social settings, de-escalating challenging behavior, incorporating self-care, worrying about the future, navigating cultural awareness, accepting the diagnosis, defining parenting role, and being an advocate. Filipino American cultural beliefs and perceptions influence how ASD is understood, perceived, and managed. Integration of culturally tailored screening tools, educational programs, and interventions are essential to ensure that nurses provide comprehensive, holistic care for Filipino Americans with ASD. Raising awareness about ASD in the Filipino American community is necessary to further understand the developmental disability, which has no cure, throughout the lifespan.
