Studies have demonstrated that many retirees face a shortfall with respect to these out-of-pocket health care expenses, but there has been little investigation into why. Are retirees unable to save for or otherwise finance such expenditures or are they unaware of the magnitude of likely future expenditures? This article explores this question by examining what individuals approaching and in retirement expect with regard to their own future out-of-pocket health care expenditures in retirement. By conducting a survey of 2000 individuals ages 40 to 80 through Rand Corporation's American Life Panel, this study ascertains respondents' expectations regarding insurance coverage and health care expenditures in retirement and compares their answers to policy experts' estimates. While, respondents' answers neared experts' estimates on some dimensions, such as median monthly expenditures, there was a significant discrepancy on others, including in lump-sum estimations and expectations regarding long-term care costs. Furthermore, responses signaled a greater gap between expectations and likely expenditures for women and younger cohorts. Finally, respondents failed to differentiate between sources of uncertainty in spending due to high individual costs, policy changes, or health care inflation, and underestimated the possible effects of these sources of uncertainty on individual expenditures. These results suggest and the article discusses important implications for financial education, health care policy, and further research.

Aging and the Macroeconomy: Long-Term Implications of an Older Population presents the fundamental factors driving the aging of the U.S. population, as well as its societal implications and likely long-term macroeconomic effects in a global context. The report finds that, while population aging does not pose an insurmountable challenge to the nation, it is imperative that sensible policies are implemented soon to allow companies and households to respond. It offers four practical approaches for preparing resources to support the future consumption of households and for adapting to the new economic landscape.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
