Par comparaison avec les autres pays développés, le système de santé français paraît relativement coûteux pour des performances sanitaires moyennes et une priorité implicite donnée à la médecine curative sur la prévention. De plus, si les inégalités d'accès aux soins se sont réduites depuis une quinzaine d'années, certaines disparités importantes persistent au détriment des populations les plus fragiles. Le mode de régulation du système de soins mis en œuvre en France depuis une vingtaine d'années, qui repose largement sur une planification centralisée des quantités (nombre de praticiens, équipements hospitaliers et enveloppes globales), n'a pas permis de réduire durablement le rythme de progression des dépenses de santé, contrairement à ce que l'on peut observer dans d'autres pays européens. Michel Mougeot propose de rééquilibrer l'intervention publique en accordant plus de place à la mise en œuvre de mécanismes incitatifs décentralisés, afin de faire jouer un rôle plus important aux signaux tarifaires - comme la tarification hospitalière " à la pathologie " - ou aux procédures de concurrence par comparaison (appels d'offre). Par ailleurs, l'Assurance-maladie devrait jouer pleinement son rôle d'acheteur de soins. Le rapport de Michel Mougeot est commenté par Yves Dimicoli, Jean-Jacques Rosa et Tony Atkinson. Il est complété par trois contributions de Béatrice Majnoni d'Intignano, Dominique Henriet et Jean-Charles Rochet, Dominique Bureau et Laurent Caussat.
