Objectifs : Déterminer les caractéristiques cliniques des troubles de déglutition chez les cérébrolésés graves en phase d'éveil de coma, et comparer les caractéristiques cliniques initiales différenciant les traumatisés crânio-encéphaliques sévères ayant repris une alimentation orale à celles des traumatisés crânio-encéphaliques sévères ne l'ayant pas reprise à 3 mois de l'admission. Matériel et méthode : Les patients inclus de manière prospective et consécutive, étaient âgés de 18 ans et plus, et admis au CRMPR Les Herbiers entre le 01 décembre 2013 et le 30 juin 2014 pour rééducation et réadaptation suite à une lésion cérébrale acquise grave, ayant motivé une prise en charge en service de réanimation, en l'absence de pathologie pré-existante pouvant induire des troubles de déglutition. Les données sociodémographiques et cliniques générales initiales étaient recueillies. L'évaluation de la déglutition comportait un examen clinique analytique et fonctionnel orienté, ainsi qu'un test de déglutition à visée fonctionnelle et une nasofibroscopie avec test de déglutition per endoscopique. L'éventuelle reprise alimentaire orale était recueillie, ainsi que la survenue de complications liées aux troubles de la déglutition (perte de poids ou infection broncho-pulmonaire). Résultats : Parmi les 13 patients admis en Unité d'éveil, 11 ont été inclus dont 81,8% d'hommes. L'âge moyen était de 40,7 +/- 14,6 ans. Tous les patients avaient des troubles déglutition à l'admission. Le premier test de déglutition à visée fonctionnelle montrait un trouble du transport du bol alimentaire qu'il soit oral (77,8%) ou pharyngé (66,7%) 28 prépondérant avec la consistance compote, tandis que les altérations des mécanismes de protection des voies aériennes prédominaient avec la consistance liquide gazeux (80% des patients testés). Le test de déglutition sous contrôle nasofibroscopique mettait en évidence, quelque soit la consistance, un trouble de la coordination de la déglutition, le plus souvent important, chez 55,6% des patients testés. Sept (63,6%) des patients ont repris une alimentation orale dans un délai moyen de 6 semaines après l'admission et 4 semaines après la lésion cérébrale acquise, avec nécessité du maintien de textures solide et liquide adaptées pour 6 (85,7%) d'entre eux. Il n'est pas survenu de complication liée aux troubles de la déglutition durant l'étude. Les troubles de déglutition observés étaient homogènes quelque soit la cause de la lésion cérébrale acquise. Les patients TCE sévères, n'ayant pas repris une alimentation orale à 3 mois de l'admission, avaient une trachéotomie avec ballonnet gonflé (p= 0,14) et un encombrement bronchique (p = 0,14) à l'admission en Unité d'éveil, une stase et des fausses routes salivaires objectivées par nasofibroscopie (p = 0,14), et n'avaient pu être sevrés de la canule de trachéotomie (p = 0,14). Conclusion : Les troubles de déglutition sont systématiques chez les cérébrolésés graves en phase d'éveil. Une évaluation précoce de la déglutition au lit du patient est donc indispensable afin de les détecter et de proposer une prise en charge médico-rééducative adaptée à ces patients en état de conscience altérée.
