Book excerpt: Since their introduction, the use of endosseous implant for replacements of missing teeth has increased dramatically. To date, however, there is no evidence-based answer to the important clinical question: What implant-supported prosthesis designs provide the best outcomes? Cochrane systematic reviews have been used to critically assess scientific literature, to answer a wide array of questions. This approach was used to focus on implant-related treatment outcomes, including patient-based outcomes. Data showed that in most cases clinical and patient-mediated outcomes did not differ significantly from one another regardless of the prosthesis used. However, long-term prognosis of implant-supported overdentures using magnet attachments may produce inferior outcomes when compared to other attachments. In this dissertation it was shown that long-term results following immediate loading of endosseous implants with overdentures were successful based on biological and clinical parameters, cost to patient, and patient-based outcomes. Moreover, it was noted that well-designed clinical trials evaluating efficacy of immediate loading with fixed prostheses are uncommon. Furthermore, a parallel randomized controlled clinical trial focusing on this issue was carried out. In this trial, the author investigated the effects of immediate and delayed loading of implants with mandibular fixed prostheses. The implant-related outcomes underscore that even when implants are loaded during the healing period (i.e. immediate loading), successful osseointegration can be achieved and maintained. Patients. satisfaction and oral health-related quality of life indicated that the two loading schemes addressed patient's need equally.

Book excerpt: This comprehensive guide to short implants will take the reader through their research and development, explain the clinical indications, evaluate the outcomes achieved with various implants, and explore restorative and laboratory considerations. Short implants have steadily gained greater market share in the last decade as practitioners sought alternatives to traditional length implants in order to avoid grafting procedures. Current manufacturers offer a variety of implant lengths and widths, allowing surgeons and restorative dentists the ability to select the best implant for each clinical circumstance. Cutting edge information is provided on the research and clinical results achieved utilizing a range of implants, specifically those developed by Nobel Biocare, Straumann, Jack Hahn, and Bicon. Readers will also find an extensive description of the role of ultra-short implants involving reconstruction in both cleft patients and cancer patients who have lost portions of their mandible and/or maxilla. This book is a must-have for those interested in learning how the use of short and ultra-short implants offers both surgeons and restorative dentists an opportunity to stand out from those that use only the traditional length implants.

Available in PDF, EPUB and Kindle. Book excerpt: Implant dentistry has become a standard option for the rehabilitation of fully and partially edentulous patients. With the ever-increasing number of dentists involved in implant dentistry, it is essential to ensure that their treatment methods follow the highest standard. The ITI Treatment Guide series, a compendium of evidence-based implant-therapy techniques in daily practice, is written by renowned clinicians and provides a comprehensive overview of various therapeutic options. Using an illustrated step-by-step approach, the ITI Treatment Guide shows practitioners how to manage different clinical situations, with the emphasis on sound diagnostics, evidence-based treatment concepts, and predictable treatment outcomes. The second volume of the ITI Treatment Guide is devoted to the restoration of partially dentate patients. Central to this volume of the ITI Treatment Guide are loading protocols available to the clinician and the patient and how they relate to various treatment indications, including both single and multiple missing teeth in the posterior and anterior regions of the mouth. Among potential topics for upcoming volumes are implant placement in extraction sockets, loading protocols in edentulous patients, implant therapy in the esthetic zone in extended edentulous spaces, and many more.

Book excerpt: BackgroundtIt has been well documented that conventional implant supported fixed prostheses (ISFP) provide excellent treatment outcomes, however in the case of patients with multiple missing teeth, they generally require a higher number of implant placements. Bounding of the removable dentures with implants, such as implant overdenture (IOD) or implant assisted removable partial denture (IA-RPD), is a treatment option, which combines the advantages of the implants and simplicity of the removable denture system with reducing the drawbacks of invasiveness and cost of multiple implants.tAimThe aim of this study was to evaluate the treatment outcome of implant assisted removable dentures (IARDs) as evaluated by oral health related quality of life (OHRQoL) in comparison with those of ISFPs.Material and methodThis study was conducted at the department of Prosthodontics and Implant Center at Showa University Dental Hospital. Patients, who were classified as Eichner B3, B4, C1, C2 and C3, were recruited consecutively between April 2008 and April 2015. In total, 33 patients (65.3u00b110.6 years old, female rate 66.7%) agreed to participate into this study after giving informed consent. Fifteen patients received ISFP, while 18 patients did IA-RPD or IOD. OHRQoL was evaluated by using the Japanese version of Oral Health Impact Profile (OHIP-J), which was administered before and one month after delivering the prostheses. The OHIP summary score as well as 4 sub-domain scores, which represented u201cOral Functionu201d, u201cOrofacial Painu201d, u201cOrofacial Appearanceu201d, and u201cPsychosocial Impactu201d, were calculated and analyzed to determine the effect of the types of treatments on these OHIP scores(t-test, p

Furthermore, it describes treatment considerations for geriatric populations, covering all relevant aspects from physiology to treatment planning and patient management in the surgical and prosthetic phases. The phenomenon of aging is a global concern for policy makers, providers, and the public. Dentists worry especially about the burden their aging patients face to maintain their oral health-related quality of life and well-being. Furthermore, older patients require health care technologies that will enable them to maintain their oral health. Over the past few decades, mandibular implant-assisted complete prostheses have attracted the attention of both patients and clinicians, as research on the biological, functional, esthetic, and psychosocial benefits has increased. This book will be of value for all with an interest in the subject..

Book excerpt: Abstract: Objectives To analyze the effect of implant treatment in edentulous patients rehabilitated with implant-supported fixed complete dentures (IFCDs) or implant overdentures (IODs) on dental patient-reported outcomes (dPROs). Materials and Methods In January 2022, Medline, Embase, CINAHL, Cochrane Library, PubMed Central, Web of Science, and ClinicalTrials.gov were screened for prospective clinical studies on completely edentulous patients treated with IFCDs and/or IODs, reporting pre-treatment and follow-up dPROs. Hedges' g effect sizes (ES) with corresponding 95% confidence intervals (CI) were calculated. Afterward, meta-analyses were conducted using random effect models. Results A total number of 1608 records was initially identified. Of those, 28 studies reporting dPROs from 1457 patients were finally included. The applied dental patient-reported outcome measures (dPROMs) included several versions of the Oral Health Impact Profile (OHIP) or specific items assessing satisfaction with Visual Analogue Scales (VAS). The overall ES was large for rehabilitation with IFCDs (1.68 [CI: 1.15, 2.20]) and IODs (1.26 [CI: 0.99, 1.52]) with no significant difference (p = .165) between the two. Denture stability was the only factor rated significantly higher for IFCDs (ES difference: 2.37 [CI: 0.21, 4.54]; p = .032). Subgroup analyses revealed moderately higher ES for IODs on two implants relative to one implant (ES difference: 0.73 [CI: 0.34, 1.12]; p .001).brbrConclusions
