Book excerpt: En la actualidad la obesidad es la enfermedad metabólica más importante, sobre todo en los países industrializados, donde está adquiriendo rasgos de epidemia. Lo que hace que la obesidad en cualquiera de sus grados, y más aún la obesidad mórbida, sea un problema de tal magnitud es la aparición de numerosas complicaciones asociadas (comorbilidades) tales como diabetes, hipertensión arterial, síndrome de apnea del sueño, etc. que determinan un deterioro severo en la calidad de vida de estos pacientes así como una disminución en su esperanza de vida. Nuestro objetivo principal ha sido demostrar que el bypass gástrico es una técnica adecuada a largo plazo para el tratamiento de la obesidad mórbida y para ello hemos planteado unos objetivos concretos, analizar la evolución de los parámetros antropométricos tras la intervención, estudiar la evolución de las comorbilidades, analizar las complicaciones del tratamiento quirúrgico a corto y largo plazo, estimar las pérdidas en el seguimiento tras la cirugía, estudiar las reintervenciones tras la cirugía inicial y por último evaluar la Calidad de vida (BAROS) y de ingesta en pacientes obesos tras la realización de un bypass gástrico. Hemos realizado un estudio retrospectivo de la serie de casos remitidos a la Unidad de Obesidad del Servicio de Cirugía General I para el tratamiento quirúrgico de la Obesidad Mórbida. Analizamos 99 pacientes intervenidos por obesidad a los que se les realizo un bypass gástrico, los pacientes fueron intervenidos entre 1996 y 2001 y se estudia su evolución durante 15 años, hasta el año 2011.Estos 99 pacientes se engloban en una serie global de 673 pacientes intervenidos entre 1991-2011. Tras el estudio de la serie de casos de bypass con un seguimiento a largo plazo (15 años) encontramos que una vez analizados todos los parámetros propuestos, el bypass gástrico sería una técnica eficaz, con baja tasa de complicaciones y con un mantenimiento de los resultados durante una largo periodo de tiempo.
