Book excerpt: The last five years have witnessed an increasing interest in the subject of transmyocardial laser revascularization (TMR) as illustrated by the number of abstracts presented at the meetings of the American Heart Association and the American College of Cardiology (Figure). The ideas and concepts associated with this particular method of myocardial revascularization have changed dramatically over even this short period of time. The original premise of "de-evolving" mammalian hearts to recreate a reptilian-like myocardial circulation by making multiple channels through the myocardium has been almost (but perhaps not quite) completely dismissed. Now, the most popular notion is that there is an angiogenic response to myocardial channel making. It is this development of new blood vessels that is thought to be responsible for the apparent improvements in symptoms and blood flow. Along the way, the idea that a channel could stay open and allow blood to flow directly from the ventricular chamber has found little support. Rather than directly explore all of these issues and merely duplicate previously published articles, our aim was to take a novel approach: that is, to step back from these arguments and provide perspective from the vantage point of distance. In the case of trans myocardial revascularization, distance comes both in terms of history and in terms of methodology and knowledge from other fields of research. Historically, innovative methods of myocardial revascularization are by no means uncommon. The first two chapters deal with this historical' perspective.

Improved lasing techniques in the cardiac catheterization suites and operation rooms have been implemented for treatment of ischemic coronary syndromes, peripheral arterial occlusive disease and other atherosclerotic thrombotic conditions. Moreover, during this 20 year time frame, several multicenter and single center clinical studies have been published focusing on the role and utilization of lasers in coronary and peripheral revascularization. And within the rapidly expanding field of interventional cardiac electrophysiology, laser technology has recently revolutionized the management of fractured, abandoned and malfunctioning leads of cardiac pacemakers and automatic defibrillators. Consequently, replacing a notoriously cumbersome and high risk open heart surgery with safe and markedly efficient percutaneous laser based extraction. Book excerpt: Transmyocardial laser revascularisation (TMR) is a procedure that mimics the circulation of the reptilian heart by creating 20-40 myocardial channels in the left ventricle using laser ablation. The operation is performed on the beating heart without using the heart-lung circulation machine. Both CO2 and holmium lasers can be used in this procedure. The application estimates approximately 50-100 patients will need the TMR each year in a large teaching hospital providing this service, with no more than 400 procedures per year in Australia. However, expert opinion indicates this is likely to be an underestimation of usage. TMR is associated with 3-5 per cent peri-operative mortality and the common complications associated with any thoractomy. The estimated incremental cost effectiveness ratios for the first year after TMR are: $18,159 per extra patient free of unstable angina; and $21,237 per extra patient free of disabling angina. Cost savings of up to $4,297 per patient per year may occur for the second year and subsequent yaers follow-up, if there is sustained symptom relief. MSAC found there is insufficient evidence to conclude that the clinical benefits of TMR outweigh the potential risks as: the procedure is associated with 3-5 per cent peri-operative mortality; and there is uncertainty in symptom relief beyond 12 months. In addition, the cost effectiveness ratios are considered unfavourable.
