Available in PDF, EPUB and Kindle. Book excerpt: David Healy follows his widely praised study, The Antidepressant Era, with an even more ambitious and dramatic story: the discovery and development of antipsychotic medication. Healy argues that the discovery of chlorpromazine (more generally known as Thorazine) is as significant in the history of medicine as the discovery of penicillin, reminding readers of the worldwide prevalence of insanity within living memory. But Healy tells not of the triumph of science but of a stream of fruitful accidents, of technological discovery leading neuroscientific research, of fierce professional competition and the backlash of the antipsychiatry movement of the 1960s. A chemical treatment was developed for one purpose, and as long as some theoretical rationale could be found, doctors administered it to the insane patients in their care to see if it would help. Sometimes it did, dramatically. Why these treatments worked, Healy argues provocatively, was, and often still is, a mystery. Nonetheless, such discoveries made and unmade academic reputations and inspired intense politicking for the Nobel Prize. Once pharmaceutical companies recognized the commercial potential of antipsychotic medications, financial as well as clinical pressures drove the development of ever more aggressively marketed medications. With verve and immense learning, Healy tells a story with surprising implications in a book that will become the leading scholarly work on its compelling subject.

Available in PDF, EPUB and Kindle. Book excerpt: When Ifirst read this manuscript, Iexclaimed to a colleague: "This is the most important and clinically relevant book on schizophrenia since Bleuler!" Time has not altered my initial enthusiastic evaluation. Drs. Strauss and Carpenter are among the most distinguished researchers in the field ofschizophrenia, butthey are also clinicians ofgreat experience, breadth, sensitivity, and flexibility. It is from this expertise, as well as theirwide familiarity with the world literature, thattheyhavebeenable to distill the essence of an exceedingly practical and comprehensive approach to the understanding, evaluation, diagnosis, and treatment of schizophrenia. They begin by unequivocally stating the inadequacy and futility of approaching schizophrenia via a single model. Standingalone, neither a biomedical, a social, nor a psychological model can adequately account for the complexities of this illness with regard to etiology, phenomenol ogy, course, or optimum treatment. While the advent of psychophar macologicalinterventionhas made a profound impact on both individual treatment and the responsive support systems, and is an important aspect ofmosttreatmentplans, to view schizophrenia as a phenothiazine deficiency disease is not only bad science but bad therapeutics. Their conceptualization of an "interactive developmental systems model" provides a framework upon which to build a broad medical approach to schizophrenia. This model relates variables drawn from different systems, interactive with one another, and contributing to a pathogenetic process across time. Within this bio-social-psychological matrix, one can then organize information relative to vulnerability, the manifest illness per se, the course of the disorder, and the multiplicity of factors relative to treatment planning.

Available in PDF, EPUB and Kindle. Book excerpt: Fungal diseases have been with us from antiquity; interest in the chemo therapy of fungal disease has exploded in the past decade. To plan and pro duce a book on the topic of antifungal chemotherapy has come as a personal challenge - and something of an eye-opener - towards the end of my re search career. A landmark publication which still merits reading is Antifungal Chemotherapy (John Wiley & Sons, Chichester, UK), edited by David Speller, which appeared in 1980. However, the fact that ketoconazole, the first of the modern, orally active, wide-spectrum antifungals, attracted no more than two sentences in it indicates just how far we have come in the 1980s. A steady stream of original papers and a number of conference proceedings have chronicled this progress in drug research; outstanding among the latter are the proceedings of an international telesymposium, entitled Recent Trends in the Discovery, Development and Evaluation of Antifungal Agents, edited by Robert Fromtling (J.R. Prous, Barcelona, 1987) and volume 544 of the Annals of the New York Academy of Sciences, entitled Antifungal Drugs, edited by Vassil St. Georgiev, and containing papers and posters presented at a most enjoyable 3-day conference held at Garden City, New York, in the autumn of 1987.

This book was released on 2012-12-06 with total page 410 pages. Available in PDF, EPUB and Kindle. Book excerpt: Sulfonium compounds have been known to the organic chemist for almost 90 years. During most of this period they remained curiosities, well suited for didactic purposes to illustrate certain similarities to ether adducts and to organic ammonium compounds. Their exploration remained largely in the academic realm, because no unusual practical applications were found in spite of a steady increase in the attention paid to them. An event that occasionally puts a class of compounds into the center of interest is the recognition of its occurrence in nature and of special biological significance. The first natural sulfonium compound was discovered less than 20 years ago, somewhat by chance rather than as the climax of some coveted biochemical problem. Gradually, however, the unique role of sulfonium compounds, especially of S-adenosyl methionine, in enzymatic group transfer reactions such as trans methylation was recognized, and the recent upsurge of interest has been exceptional. The present review is directed toward an interpretation of the properties and functions of biological sulfonium compounds with frequent reference to their structural chemistry. The data presented here may suggest novel or improved analytical techniques and new interpretations of the mechanism of group transfer. Furthermore, a stimulus may be derived for the design of sulfonium compound analogues. The availability of competitive analogues of biochemical key compounds has aided in the understanding of the function of virtually all metabolic intermediates, prosthetic groups of enzymes, vitamins, amino acids, and other compounds. The pattern will be similar in sulfonium biochemistry.
