Available in PDF, EPUB and Kindle. Book excerpt: In Transactional Analysis of Schizophrenia: The Naked Self, Zefiro Mellacqua presents a full assessment of the relevance and value of transactional analysis in understanding, conceptualizing and treating schizophrenia in contemporary clinical settings. Opening with a review of Eric Berne’s ideas, Mellacqua applies theory to the understanding and psychotherapeutic treatment of people suffering from first-episode schizophrenia and to those already living with more long-lasting psychotic levels of self-disturbance. The chapters address a series of crucial methodological themes, including the need for both intensive and extensive analytic sessions; the therapist’s tolerance of uncertainty and not knowing; the informative quality of both therapist’s and patient’s embodiment(s); the emergence of the transference-countertransference relationship; the link between silent transactions and unconscious communication; dream analysis; and the value of regular supervisions. Mellacqua’s approach incorporates meetings with family and caregivers, as well as emphasising multidisciplinary work with patients in a variety of settings, such as in hospitals, outpatient clinics, and psychiatric home treatment. The book is illustrated with engaging clinical case studies throughout, which illuminate the schizophrenic experience and provide examples of how these tools can be used to help patients. Transactional Analysis of Schizophrenia demonstrates how those who suffer from acute schizophrenia, especially those at their very first episode of psychosis, can make an effective recovery and live a satisfying life through the therapeutic application of transactional analysis. It will be essential reading for transactional analysts, psychodynamically oriented psychotherapists, psychologists, psychiatrists, nurses, social workers, academics and all mental health professionals working with people suffering from schizophrenic psychoses. Available in PDF, EPUB and Kindle. Book excerpt: Breaking through Schizophrenia builds on the ideas of Jacques Lacan who argued that schizophrenia is a deficient relationship to language, in particular the difficulty to master the metaphoric dimension of language, which children acquire by the Oedipal restructuring of the psyche. This book is thus a countercultural move to present a less damaging view and a more efficient treatment method for schizophrenic persons. Through a collection of published and unpublished articles, Ver Eecke traces the path of Lacanian thought. He discusses the importance of language for the development of human beings and examines the effectiveness of talk therapy through case studies with schizophrenic persons.

Available in PDF, EPUB and Kindle. Book excerpt: Originally published in 1961, this book outlines a new, unified system of individual and social psychiatry that were introduced in the United States around that time with remarkable success in various hospitals and other psychiatric establishments. Essentially designed for group therapy, this approach is now used by institutions, group workers, and in private practice with neurotics, psychotics, sexual psychopaths, psychosomatic cases, and adolescents. Transactional analysis begins its program by initiating the individual patients into the theory upon which the treatment is based. First attaining a measure of self-knowledge through private sessions with the analyst, the patient then meets with other patients in group therapy, participating in a series of personally meaningful relation-ships in which he becomes increasingly aware of the cause and nature of his illness, preparing at the same time to overcome it. “A comprehensive method of treatment that has no precedent in its concreteness of structure without at the same time diminishing the dynamic quality of the treatment....No one to my knowledge has presented such a new approach.”—Dr. Milton Schwebel, Professor of Education, New York University

Available in PDF, EPUB and Kindle. Book excerpt: Do psychotic disorders make sense? Are psychotic symptoms amenable to interpretation? Understanding Psychosis: A Psychoanalytic Approach takes the various pathways to psychotic illness outlined by psychoanalytic clinicians and scholars and integrates them into a model that allows a systematic assessment of relevant psychodynamic dimensions in the diagnosis of psychotic disorders, and which serves as a guide to psychotherapy with psychotically ill patients. Joachim Küchenhoff reviews and integrates various psychoanalytic concepts and theories about psychosis into a multi-dimensional psychodynamic model that allows an assessment and understanding of the patient’s subjective experience, objective psychological capabilities, and interpersonal resources. Küchenhoff helps the therapist to establish a basic attitude in working psychodynamically with patients and to understand the dynamics of the therapeutic relationship. Understanding Psychosis also addresses specific issues that can arise in work with clients experiencing psychosis, including understanding imminent crises or precursor states, elucidating semiotic qualities in seemingly negative symptoms, differentiating the psychotic and a non-psychotic part of the personality and providing a dynamic approach to the psychopharmacological treatment. Clinical vignettes and three detailed case reports are included in the book. Understanding Psychosis will be an essential guide for psychiatrists, psychotherapists and psychoanalysts working with patients experiencing psychosis. It will also be of use to psychologists, and academics and students of psychotherapy, psychiatry and psychoanalysis for psychosis.

Book excerpt: This book looks at why, despite profound advances in psychological science and neuroscientific analyses of schizophrenia, outcomes for the disorder have changed little over the past 100 years. It analyzes the limiting role on treatment development of diagnostic classifications and views of the disorder as caused by a core pathology, and instead promotes the idea of individually tailored, multimodal treatment for distinct disorder features (e.g., positive symptoms, cognitive deficits).

Available in PDF, EPUB and Kindle. Book excerpt: Inevitably, every psychotherapist has some experience with severely disturbed patients. Consequently, they will turn with excitement to this important new book which is a stunning attempt by two knowledgeable, persevering psychotherapists to present their understanding and sound therapeutic approach to these difficult and challenging patients. The authors argue that the treatment of choice is clearly psychotherapy and that such treatment can be successful and as long lasting for schizophrenic patients as it is for neurotic patients, but the journey may be longer and it may take more time to traverse. The task of therapy is to untangle the past from the present to make the future conceivable. The volume provides a thorough historical overview of the theoretical and clinical approaches to the problem of schizophrenia, including the views of leading contemporary clinicians on the topic. In general, the major clinical controversies have been regarded as issues of whether to focus on past, present or future; reality or fantasy; affects; exploration or relationship; whether the therapist should be active or passive; and how to handle regression. The authors argue that these are the wrong issues. They say that the task of therapy is to untangle the past from the present to make the future conceivable. Reality and fantasy are intertwined and must both be dealt with. Affects are central to all therapy, and emphasis on anger, despair, loneliness, terror, and shame are all necessary, as is the clarification of affect, and the acceptance of positive affect. Activity versus passivity is again in the wrong question; the right one is what action is helpful, when it is helpful, and when is not doing anything helpful? Regression is inevitable; should one accept it fully or try to limit it? This has no general answer other than do what is necessary (i.e., unavoidable) or most helpful to a particular patient at a particular time.

Available in PDF, EPUB and Kindle. Book excerpt: A powerful account of how cultural anxieties about race shaped American notions of mental illness The civil rights era is largely remembered as a time of sit-ins, boycotts, and riots. But a very different civil rights history evolved at the Ionia State Hospital for the Criminally Insane in Ionia, Michigan. In The Protest Psychosis, psychiatrist and cultural critic Jonathan Metzl tells the shocking story of how schizophrenia became the diagnostic term overwhelmingly applied to African American protesters at Ionia—for political reasons as well as clinical ones. Expertly sifting through a vast array of cultural documents, Metzl shows how associations between schizophrenia and blackness emerged during the tumultuous decades of the 1960s and 1970s—and he provides a cautionary tale of how anxieties about race continue to impact doctor-patient interactions in our seemingly postracial America. Book excerpt: This innovative book presents state-of-the-art thinking on using transactional analysis (TA) to change the structure, relationships and culture in organizations. The book is arranged according to the three levels of organizations described by Eric Berne – the structural, interpersonal and psychodynamic levels – and the chapters expand on his concepts at each level. With contributions by an international range of authors, incorporating a selection of practical case studies, the book illuminates key themes including group and team dynamics, psychological safety, emotion and, most foundationally, boundaries. Exploring the tensions of boundaries that can determine both the stability of a system as well as its innovative potential, this book provides a strong structural framework for TA coaches, consultants and analysts, as well as other professionals working with and within organizations.

It sets them in social, cultural and political contexts, and considers a number of important implications of this particular relational turn in psychotherapy. The book advances relational transactional analyses and, in doing so, reflects the creativity and vibrancy of contemporary TA. The editors have skilfully brought together different generations of TA practitioners in an accessible and stimulating volume.
