The benefit package is uniform and premiums do not rise with age. As Marilyn Moon shows in this important work, however, the characteristics of older women make them extremely vulnerable to gaps in Medicare coverage and other features of the program. Because older women are more likely to suffer from multiple chronic illnesses than men, Medicare's failure to cover most outpatient prescription drugs affects them the most. Because women live longer on average than men and have lower incomes from pensions and other sources, they are less able to meet the out-of-pocket costs associated with Medicare or supplemental insurance. Moon evaluates Medicare reform proposals with an eye to whether they would alleviate or worsen the existing position of women in Medicare. She argues that making Medicare benefits more comprehensive and simplifying the program's rules would improve the health of older women and reduce their financial burdens.

Book excerpt: Medicare provides essential health coverage for older and disabled adults, yet it does not limit out-of-pocket costs for covered benefits and excludes dental, hearing, and longer-term care. The resulting out-of-pocket costs can add up to a substantial share of income. Based on U.S. Census surveys, nearly a quarter of Medicare beneficiaries (11.5 million) were underinsured in 2013-14, meaning they spent a high share of their income on health care. Adding premiums to medical care expenses, we find that 16 percent of beneficiaries (8 million) spent 20 percent or more of their income on insurance plus care. At the state level, the proportion of beneficiaries underinsured ranged from 16 percent to 32 percent, while the proportion with a high total cost burden ranged from 11 percent to 26 percent. Low-income beneficiaries were most at risk. The findings underscore the need to assess beneficiary impacts of any proposal to redesign Medicare.

These people are referred to as "dual eligibles." Because Medicare and Medicaid have different coverage rules and provider access, and dual eligibles are generally a less healthy population, there are higher costs and greater challenges in providing health care for this population. Proposals include requiring all low-income older people to enroll in a managed care plan, which means the care they receive would need to come from doctors and hospitals in the provider network for that managed care plan.
