Book excerpt: In the last 40 years, the study of aortic heart valve replacements has had its roots in the work of great clinicians, scientists and biomedical engineers who established the techniques for heart valve substitutes. Recently the research on the development of aortic heart valve replacements has shifted toward tissue engineering (TE), a multidisciplinary research that involves biological, biomaterial and bioengineering sciences. This book introduces current valve substitutes and their limitations and the futuristic tissue engineering of aortic heart valves (TEHV). It highlights the substantial achievements that have been accomplished in the last two decades in TEHV research and discusses the much needed exertion in defining the functional requirements of clinical implementation and in developing the means to produce a functional aortic valve.

Available in PDF, EPUB and Kindle. Book excerpt: Due to population aging, calcific aortic valve disease (CAVD) has become the most common heart valve disease in Western countries. No therapies exist to slow this disease progression, and surgical valve replacement is the only effective treatment. Calcific Aortic Valve Disease covers the contemporary understanding of basic valve biology and the mechanisms of CAVD, provides novel insights into the genetics, proteomics, and metabolomics of CAVD, depicts new strategies in heart valve tissue engineering and regenerative medicine, and explores current treatment approaches. As we are on the verge of understanding the mechanisms of CAVD, we hope that this book will enable readers to comprehend our current knowledge and focus on the possibility of preventing disease progression in the future.

Available in PDF, EPUB and Kindle. Book excerpt: Myocardial tissue engineering (MTE), a concept that intends to prolong patients’ life after cardiac damage by supporting or restoring heart function, is continuously improving. Common MTE strategies include an engineered ‘vehicle’, which may be a porous scaffold or a dense substrate or patch, made of either natural or synthetic polymeric materials. The function of the substrate is to aid transportation of cells into the diseased region of the heart and support their integration. This book, which contains chapters written by leading experts in MTE, gives a complete analysis of the area and presents the latest advances in the field. The chapters cover all relevant aspects of MTE strategies, including cell sources, specific TE techniques and biomaterials used. Many different cell types have been suggested for cell therapy in the framework of MTE, including autologous bone marrow-derived or cardiac progenitors, as well as embryonic or induced pluripotent stem cells, each having their particular advantages and disadvantages. The book covers a complete range of biomaterials, examining different aspects of their application in MTE, such as biocompatibility with cardiac cells, mechanical capability and compatibility with the mechanical properties of the native myocardium as well as degradation behaviour in vivo and in vitro. Although a great deal of research is being carried out in the field, this book also addresses many questions that still remain unanswered and highlights those areas in which further research efforts are required. The book will also give an insight into clinical trials and possible novel cell sources for cell therapy in MTE.

Available in PDF, EPUB and Kindle. Book excerpt: Cardiovascular diseases are still the leading cause of death in developed countries. Revascularization procedures such as coronary artery and peripheral bypass grafts, as well as access surgery represent a 2$ billion market yearly for the US alone. Despite intense research over many decades, no clinically suitable, shelf-ready, synthetic, vascular, small-caliber graft exists. There is therefore still a quest for such a clinical vascular prosthesis for surgical revascularization procedures and access surgery. Many approaches have been tried and are currently under investigation with promising results. These range from acellular and cell-based, stable or bio-degradable, synthetic scaffolds to biological or decellularized grafts, not forgetting self-assembly technologies for in vitro or in vivo VTE. All these approaches can be further enhanced by functionalization, e.g. with growth factors and drug elution. This updatable book aims to cover all the relevant aspects of Vascular Tissue Engineering (VTE) and novel alternatives to develop vascular grafts for clinical applications. The chapters in this book cover different aspects of manufacturing scaffolds with various polymers, mechanical characteristics, degradation rates, decellularization techniques, cell sheet assembly, 3-D printing and autologous mandril-based VTE. All the necessary in vitro tests such as biocompatibility and thrombogenicity are reviewed. Pre-clinical assessment of in vivo experimental models include patency, compliance, intimal hyperplasia, inflammatory reaction, cellular ingrowth and remodeling. Finally, early clinical trials will be periodically updated regarding results, regulatory aspects and post-marketing quality assessment. Furthermore, the reader should get an insight into various approaches, technologies and methods to better understand the complexity of blood surface and cell interactions in VTE. Translational research has yielded early human applications clearly showing the enormous need of research in the field to provide better solutions for our patients and this continuously updated book will hopefully become a reference in the field for life sciences.

While the past 40 years have brought major progress in cardiac valve repair and replacement, there remain large patient populations that do not receive such therapies. This, in turn, implies a great need for future basic, applied, and clinical research and, ultimately, therapeutic developments. Heart Valves is a state-of-the-art handbook dedicated to: 1) cardiac valve anatomy, 2) models for testing and research methods; 3) clinical trials; and 4) clinical needs and applications.

Book excerpt: Heart valve disease is an increasing global burden affecting patients of all ages, ranging from pediatrics to the elderly. Unfortunately, there are currently no diagnostics for early detection or therapeutic treatment strategies. The only remedy for end-stage valve disease is a prosthetic heart valve replacement. However, these non-living prostheses do not possess the ability to remodel, integrate, and respond biologically with the patient, leading to life-long medications or multiple resizing surgeries. Tissue engineering offers an enticing strategy to fabricate living, biological heart valve conduits with growth and integration potential. While there has been advances in fabricating tissue engineered heart valves, there remains a challenge of producing a heterogenous valve. The focus of this dissertation was to develop and evaluate biomaterials and a bioreactor system that can better provide environments for cells to grow and remodel. A hybrid hydrogel biomaterial developed by incorporating solubilized decelluarlized aortic leaflets into a bioprintable base material promoted a myofibroblastic phenotype in encapsulated cells and led to more matrix deposition (Chapter 2). Next, a composite biomaterial was produced by conjugating nanocellulose crystalline with methacrylated gelatin. The material enhanced material properties and promoted a chondrogenic-like phenotype in encapsulated HADMSC (Chapter 3). Finally, a bioreactor system was built to capture a wide range of pressures and frequencies found in the pediatric and adult populations (Chapter 4). The system was validated by culturing ex vivo porcine heart valves and conditioning a bioprinted tissue engineered heart valve. Overall, the completion of this work advanced the field of tissue engineering heart valves by providing insights on two types of biomaterials that can modulate stem cell behavior and phenotype. The bioreactor system proved to be useful in future studies involving both engineered and ex vivo heart valves.

In view of the latest applications of several 3D printing systems, their advantages and disadvantages are also discussed. A basic understanding of the entire spectrum of organ printing provides pragmatic insight into the mechanisms, methods, and applications of this discipline. Organ printing is being applied in the tissue engineering field with the purpose of developing tissue/organ constructs for the regeneration of both hard (bone, cartilage, osteochondral) and soft tissues (heart). There are other potential application areas including tissue/organ models, disease/cancer models, and models for physiology and pathology, where in vitro 3D multicellular structures developed by organ printing are valuable.
