Book excerpt: In order to guarantee the quality, safety and efficacy of Tissue Engineered Medical Products (TEMPs), the application of risk management strategies by all parties involved in the life cycle of TEMPs is of the utmost importance. Due to the novelty of TEMPs, their risks have not been clearly and completely identified yet. Extrapolation of experiences with related medical products indicates that the most important risks of TEMPs will relate to the transmission of disease, biocompatibility and efficacy. Many of the risk control measures included in current regulations and standards covering related product categories like medicinal products, blood (products) and medical devices are applicable in generic form to TEMPs. From a regulatory perspective control measures for TEMPs should be combined into a regulatory framework that is complementary to and connects with current European regulation on medical products. Decisions on risk acceptability should preferably be based on predefined standards. Development of internationally accepted standards is therefore essential.

There were individual breakout sessions on standards for: Biomaterials, Therapeutic and Drug Delivery Devices, Auditory Diagnostic and Prosthetic Devices, Manufacturing of Prostheses, and Tissue Engineered Medical Products (TEMPs). Cross-cutting issues of Harmonization of Standards, Data, and Sterilization were also addressed. The session on Standards for TEMPs placed its most significant needs on the development of test methods and materials characterization. Action items for NIST to consider were separated into those relative to the Food and Drug Administration (FDA) approval process and those relative to industry needs not directly related to the FDA approval process. This paper summarizes the needs identified for TEMPs and the status of NIST-related activities.

Natural materials include alginate, chitosan, collagen and hyaluronate. These materials support cell growth and differentiation on TEMPs substrates and scaffolds. Natural materials have been used in a variety of applications, including encapsulation, cell seeding, development of "memory" biomaterials, as well as degradable scaffolds, growth factor/nucleic acid delivery vehicles, and as a carrier to improve product handling characteristics. These materials have typically been very poorly characterized as to their chemical, physical and biological properties. This has resulted in variability in the products produced from these starting materials. The development of Standard Guides and Test Methods for characterizing natural materials is anticipated to reduce the variability of these starting materials and to aid in the assessment of the safety of the subsequent TEMPs.

The device is validated for sterility and freedom from pyrogens before it can be tested on animals or humans. Medical devices are classified as class I, II or III depending on their invasiveness. Class I devices can be marketed by submitting notification to the FDA. Class II and III devices require either that they show equivalence to a device marketed prior to 1976 or that they receive pre-marketing approval. The time from device conception to FDA approval can range from months (class I device) to in excess of ten years (class III device). Therefore, much planning is necessary to pick the best regulatory approach. 2. Wound Dressings and Skin Replacement 2.1 Introduction Wounds to the skin are encountered every day. Minor skin wounds cause some pain, but these wounds will heal by themselves in time. Even though many minor wounds heal effectively without scarring in the absence of treatment, they heal more rapidly if they are kept clean and moist. Devices such as Band-Aids are used to assist in wound healing. For deeper wounds, a variety of wound dressings have been developed including cell cultured artificial skin. These materials are intended to promote healing of skin damaged or removed as a result of skin grafting, ulceration, burns, cancer excision or mechanical trauma.
