The goal is to improve access to medical marijuana for patients with refractory debilitating neurological disorders, cancer, and chronic pain as an alternative to ineffective pharmacotherapy and potentially addictive pain medications. The ultimate test of our nations resolve to ensure the welfare of our sickest patients is the enactment and implement of effective public health reform in the area of medical marijuana, also known as medical cannabis.This book evolved out of the present need for a definitive volume on the science and public health aspects of medical cannabis to fuel this national narrative. The ethnographic research presented in the concluding chapter was inspired by Professor Miriam W. Boeri and colleagues, at Bentley University in Waltham, MA. They examined views of community stakeholders including medical marijuana dispensary entrepreneurs, health care professionals, and patients in a state that legalized medical marijuana in 2013, yet there continued to be confusion and misunderstandings in the interpretation and implementation of medical marijuana guidelines during the period of policy shifts. Apparent gaps in policy development and implementation signaled the urgency for a comparison study addressing stakeholder views in New York State, where its medical marijuana program has legally dispensed the drug since 2014. The resulting pilot study was carried out in the Division of Health Policy and Management of the City University of New York School of Public Health. The research model incorporated ethnographic and grounded methodologies to detail the views of physicians, pharmacists, educators, patients, and entrepreneur stakeholders; with triangulation of data and application of dominant themes into a socioecological framework model to identify areas of public health policy reform. The findings of this study detail that New York, like other states that recently legalized the dispensation of medical marijuana, faces challenges beyond policy transparency, communication and education explicitly to improve the implementation process for applying and registering medical cannabis dispensaries, referring physicians, and qualified patient recipients.Ken Langone, Chairman of the Board of New York University Langone Health, and Steven Galetta, Chair of Neurology in the School of Medicine, where the authors is senior staff in neuroepidemiology, motivated him to pursue doctoral training in Health Policy and Management. The author has had the good fortune of interacting with thought-provoking medical students, neurology trainees, public health doctoral students, and professors who reinforce the high ethical standards in medical and public health practice and research. However, his patients still educate him in empathy and humanity. The author is grateful to his family, including his spouse Holly and sons Adam and Seth, who serve as his daily compass, encouraging him to take on projects that promote core values of medicine and humanity.

Book excerpt: The concept of expressing acidity as the negative logarithm of the hydrogen ion concentration was defined and termed pH in the beginning of the 20th century. The general usefulness of the pH concept for life science was recognized and later gained importance to analytical research. Reports on results of pH measurements from living skin established the term acid mantle - the skin's own protective shield that maintains a naturally acid pH. It is invisible to the eye but crucial to the overall wellbeing of skin. Chronic alkalization can throw this acid mantle out of balance, leading to inflammation, dermatitis, and atopic skin diseases. It is therefore no surprise, that skin pH shifts have been observed in various skin pathologies. It is also obvious that the pH in topically applied preparations may play an important role. Optimal pH and buffer capacity within topical preparations not only support stability of active ingredients and auxiliary materials, but may also increase absorption of the non-ionized species of an acidic or a basic active ingredient. They may even open up opportunities to modify and "correct" skin pH and hence accelerate barrier recovery and maintain or enhance barrier integrity. Further efforts are needed to standardize and improve pH measurements in biological media or pharmaceutical/cosmetic vehicles to increase and ensure quality, comparability, and relevance of research data. In this volume, we present a unique collection of papers that address past, present and future issues of the pH of healthy and diseased skin. It is hoped that this collection will foster future efforts in clinical and experimental skin research.

One of the most controversial–as well as one of the most popular–alternatives is homeopathy, a wholly Western invention brought to America from Germany in 1827, nearly forty years before the discovery that germs cause disease. Homeopathy is a therapy that uses minute doses of natural substances–minerals, such as mercury or phosphorus; various plants, mushrooms, or bark; and insect, shellfish, and other animal products, such as Oscillococcinum. These remedies mimic the symptoms of the sick person and are said to bring about relief by “entering” the body’s “vital force.” Many homeopaths believe that the greater the dilution, the greater the medical benefit, even though often not a single molecule of the original substance remains in the solution. In Copeland’s Cure, Natalie Robins tells the fascinating story of homeopathy in this country; how it came to be accepted because of the gentleness of its approach–Nathaniel Hawthorne and Henry Wadsworth Longfellow were outspoken advocates, as were Louisa May Alcott, Harriet Beecher Stowe, and Daniel Webster. We find out about the unusual war between alternative and conventional medicine that began in 1847, after the AMA banned homeopaths from membership even though their medical training was identical to that of doctors practicing traditional medicine. Copeland was a student of both conventional and homeopathic medicine, an eye surgeon who became president of the American Institute of Homeopathy, dean of the New York Homeopathic Medical College, and health commissioner of New York City from 1918 to 1923 (he instituted unique approaches to the deadly flu pandemic). We see how Copeland straddled the worlds of politics (he befriended Calvin Coolidge, Herbert Hoover, and Franklin and Eleanor Roosevelt, among others) and medicine (as senator, he helped get rid of medical “diploma mills”). His crowning achievement was to give homeopathy lasting legitimacy by including all its remedies in the Federal Food, Drug, and Cosmetic Act of 1938. Finally, the author brings the story of clashing medical beliefs into the present, and describes the role of homeopathy today and how some of its practitioners are now adhering to the strictest standards of scientific research–controlled, randomized, double-blind clinical studies.
