Book excerpt: The therapeutic alliance, one of the most important common factors of change and therapeutic outcome in psychotherapy research has been gaining significant attention in the past years. The quality of the therapeutic alliance has not only had significant impact in treatment motivation and positive treatment results, but also in therapy dropout rates and recidivism reduction. Latinos, one of the largest and most rapidly growing minority groups in the United States currently face a myriad of life stressors that make them especially vulnerable to a variety of mental heath problems. Yet, they are the largest populations with the highest attrition and poor attendance in psychotherapy. Different reasons for their underutilization of mental health services have been posited including high levels of stigma towards mental health and cultural miscommunication in the therapeutic relationship. Given the under-utilization of mental health services by this population in the United States, it is crucial that their cultural values be addressed in all facets of psychotherapy, including the therapeutic relationship. Therapist self-disclosure (TSD) has been recognized as an element that contributes to the therapeutic alliance, but few studies have examined the use of this technique as a component of culturally competent care or as a method of strengthening the therapeutic alliance with the Latino population. This systematic review closely examined eight articles that provided reviews of the self-disclosure literature examining its association to the working alliance, treatment progress, and outcome. Accordingly, the purpose of this literature review is to identify best practices for effectively utilizing TSD with Latinos, explore factors contributing to the effectiveness or ineffectiveness of self-disclosure with this population, as well as identify positive and negative outcomes associated with TSD with the Latino population. Results showed that TSD was linked to the development and strengthening of the overall therapeutic relationship, softening the client-therapist power differential and creating a more collaborative alliance. Best practices for effectively utilizing therapeutic self-disclosure were identified as: aside disclosures, neutral disclosures, similar disclosures, humanizing disclosures, disclosures of feelings, insight, and disclosures with more intimacy in later session of treatment, brief disclosures, always returning the focus back to the client, responsive to their needs and the emerging therapeutic relationship, and disclosure of personal religious background at the outset of therapy with individuals high in religiosity. Future directions were proposed. It is hoped that this review will raise awareness and motivation for researchers to conduct future research in the area of therapist self-disclosure and therapeutic alliance with the Latino client.
