In addition to national statistics, this publication presents statistics on the inequalities in health outcomes and interventions coverage within countries, disaggregated by urban/rural setting, wealth/assets, and educational level. Such statistics are primarily derived from the analysis of household surveys and are only available for a limited number of countries.

This book insightfully explores these challenges in ways that put the perspectives of Africans themselves at centre stage. It begins with the long central and still compelling African health challenge of combating the pandemic of HIV/AIDS. It then examines the global governance responses by the major multilateral organizations of the World Bank and the World Trade Organization and the newer informal flexible democratically oriented ones of the Group of Eight. It also addresses the compounding health challenge created by climate change to assess both its intensifying impact on Africa and how all international institutions have largely failed to link climate and health in their governance response. The basic assumption is that the health of a population is not independent of broader demographic trends, and does follow the health transition model. Population health is an essential component of human and social development. As both a means and an end of development, health lies at the heart of underdevelopment, and ranks first on the list of international priorities. The WHO slogan ‘Health for all in 2000’ reflects the spirit of a more general movement in favor of health promotion throughout the world. But the developing world is far from reaching this aim. The health of populations has improved in developing regions but there are still deep inequalities, and serious problems remain, especially in Sub-Saharan Africa. After reviewing the core concepts of population health, the book examines health transition in developing countries, a process that has resulted in a double burden of diseases. A discussion of mortality in developing countries serves to highlight the high rates of child mortality in these regions. The book devotes a full chapter to women’s health, and its chapter-length analysis of morbidity highlights the double burden weighing down developing populations and concludes with an analysis of health systems in developing countries.

Book excerpt: The Department for International Development's (DFID) objectives continue to focus on achievement of the internationally agreed Millennium Development Goals with the overarching aim of poverty reduction. This focus remains valid. However, on current trends most of the Goals will not be met by the 2015 deadline. The prospects for achieving Millennium Development Goal 2 on universal primary education by 2015 concern the Committee. To meet the target of all children completing primary education by 2015, universal access to schooling will need to be in place by 2010. The necessary schools will therefore have to be built and teachers trained in less than two years. The global economic downturn may exacerbate the risk of failure if development assistance levels are not maintained and donor commitments on aid are allowed to lapse. In straitened economic circumstances it is vital that every pound spent achieves the maximum impact, not least so that public support for aid expenditure can be maintained. The Committee is not convinced that DFID's evaluation processes allow it to make an accurate assessment of what its funding is achieving. The Independent Advisory Committee on Development Impact has begun to improve evaluation within DFID. This process must continue with the full engagement of the Department. DFID's ability to deliver its objectives is beginning to be constrained, despite its rising budget, by the Government-wide requirement to reduce its administrative budget and therefore the number of staff it employs. The Government should urgently reassess whether DFID has sufficient staff in place effectively to deliver the objectives which it has assigned to the Department under its Public Service Agreements.
