Book excerpt: In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment.

Book excerpt: In the United States and the rest of the world, the 2019 novel Coronavirus pandemic has continued spreading and brought significant attention to underlying issues within our healthcare system. Our healthcare system has greatly turned its attention towards telehealth to deliver accessible care during this difficult time. The Centers for Medicaid and Medicare Services (CMS) took a significant step in implementing new policy changes for Telemedicine. But these policies have only been extended to the end of this year, and once the Pandemic has passed, these new policies may be discontinued. This qualitative study examines the Centers for Medicaid and Medicare Telehealth Policy regulations in response to the 2019 coronavirus pandemic and how those changes impact ethnic and racial disparities in our healthcare system. The Kingdon's Conceptual Framework was utilized to evaluate the three streams of the model that included problem (externalities), policy (price sensitivity), and stakeholders (supply and demand). Qualitative analysis of archival data was gathered from peer-reviewed articles published from 2017 to 2021. Relevant material was located through academic databases where terms searched included Medicaid and Medicare Regulations AND Telemedicine AND Covid 19. Additional non-peer-reviewed articles were gathered through government cites as well. Findings showed that there has been a shift from practice-centric care to patient-centric care while addressing the utilization of Telemedicine amongst minority groups and underserved populations. Furthermore, Policy implications included lingering racial and ethnic disparities with Telemedicine access and state reimbursement and financial disparities between in-office visits and telemedicine visits leading to reducing incentives for providers to use telemedicine services. There is a clear indication that despite all the hardships this pandemic has brought on, it has created an opportunity to change the healthcare field with a much-needed push towards including more minority and underserved populations with access to Telemedicine.

Book excerpt: The COVID-19 pandemic has demonstrated the fundamental importance of telecommunications networks. This paper utilizes data from a large North American network operator to examine the role of telecom networks in enabling public health and economic responses to the pandemic. During the pandemic, data usage grew significantly, with growth in wireline data use outstripping growth in wireless data use, 53% to 27%, between March and December 2020. Yet, even at pre-pandemic levels, data use is growing exponentially, doubling every 1.3 years between 2017 and 2020. This paper also considers three examples of public health and economic responses enabled by telecom networks: staying at home, adopting telemedicine, and teleworking. First, based on de-identified data on customer movement and location, this paper estimates that compliance with stay-at-home orders was about 20% lower in the 2nd wave than during the 1st wave, despite much cases counts, suggesting that Canadians suffered from isolation fatigue. Across Canada's six largest cities, controlling for population and GDP per capita, a 1% decrease in compliance is associated with an additional 700 COVID-19 infections. Second, based on data on two telemedicine apps, this paper highlights the potential for the rapid adoption of telemedicine. At the start of the pandemic, adoption of these apps doubled and then was sustained throughout 2020, with usage patterns reflecting digital divides in age and gender. Third, Canada's teleworking rate changed in lockstep with COVID-19 cases, even while the unemployment rate remained constant, suggesting that employers are adopting teleworking as a flexible way to adapt to evolving public health conditions and restrictions without resorting to layoffs. When viewed against this backdrop, telecommunication policy can support public health and social outcomes, in addition to economic outcomes.

The policy implications suggest that each state should continue to provide healthcare through telehealth by extending the disaster-related state plan amendments in order to increase accessibility to care. There is a clear indication that Telehealth can be used as a way to improve healthcare access while reducing the risk of exposure to COVID-19.
