Book excerpt: Although there is substantial functional limitation and disability among veterans of all ages, relatively little is known about veterans' uptake of Department of Veterans Affairs (VA) Disability Benefits and Social Security Disability Insurance (DI). This project uses data from the 1992, 1993, 1996, 2001, 2004, and 2008 Survey of Income and Program Participation to examine veterans' participation in VA and DI programs. The results indicate that the majority of veterans do not receive VA or DI benefits, but veterans' use of these programs has been increasing over time. A higher percentage of veterans receive VA compensation only, which ranges from 4.9 percent in 1992 to 13.2 percent in 2008, than DI compensation only, which ranges from 2.9 percent in 1992 to 6.7 percent in 2008. Furthermore, the rate of joint participation in these two programs is low, ranging from less than 1 percent in 1992 to 3.6 percent in 2008. Veterans experience relatively few within-panel transitions between VA and DI programs. Overall, the likelihood of any disability program use is higher among veterans who served during multiple time periods, are older, black or Hispanic, currently married, and have less than a high school education. Among users, the likelihood of any VA use in contrast to only DI use is higher among veterans who served since 1990, are younger, Hispanic, highly educated, and currently married. Among users, variation in the likelihood of any DI use relative to only VA use generally mirrors variation in the likelihood of any VA use, although there are differences in associations with race/ethnicity, education, and marital status.

On the other hand, VDC is not insurance, but is a compensation program in that payments are made to veterans who develop medical conditions that are related to their service in the military. With such an extensive cash outlay and such a large beneficiary population, it is important to use updated scientific, workforce, and economic information to evaluate claims for disability benefits. Over time, progress in the fields of medicine and technology has provided a better understanding of how disease and injury affect the ability to work. Likewise, changes in the labor market have affected the skills needed to perform work and the settings in which work occurs. Together, scientific advances and labor market changes redefine the extent that physical or mental conditions affect the ability of people with disabilities to work. If federal disability programs do not update scientific and labor market information used in assessing program eligibility, they risk overestimating the limiting nature of some disabilities while underestimating others. Moreover, not keeping abreast of this information puts federal programs at risk of undermining their efforts to help some persons with disabilities achieve economic independence or work to their full potential. Although the three largest federal disability programs differ in their underlying purpose, they face a similar underlying challenge. SSA administers both the Disability Insurance (DI) program and the Supplemental Security Income (SSI) program. DI provides benefits to workers with severe long-term disabilities who have enough work history to be insured for coverage under the program.

The current rating schedule emphasizes impairment and limitations or loss of specific body structures and functions which may not predict disability well. 21st Century System for Evaluating Veterans' Disability Benefits recommends that this schedule could be revised to include modern concepts of disability including work disability, nonwork disability, and quality of life. In addition to the need for an updated rating schedule, this book highlights the need for the Department of Veterans' Affairs to devote additional resources to systematic analysis of how well it is providing services or how much the lives of veterans are being improved, as well as the need for a program of research oriented toward understanding and improving the effectiveness of its benefits programs.
