Book excerpt: Excerpt from The Treatment of Infantile Paralysis The great prevalence of infantile paralysis in America since 1907 has not only directed the attention of the medical profession to the disease in a very practical way, but has led to a rapid advance in our knowledge of the affection and of its treatment. Indeed so rapid has been this progress, particularly on the therapeutic side, that it has not had time to find its way into text-books, but exists almost wholly in the fugitive periodical literature. It is to meet the situation just described that this book has been written and it is concerned wholly with treatment except for a short introductory chapter dealing with other aspects of the disease in their bearing on treatment. This limitation was adopted because my experience has been chiefly in this department and because conditions arc not yet sufficiently settled with regard to pathology, etiology, transmission, etc., to make it possible to write a book with any promise of permanence which should deal with the whole subject. The aim has been to present a practical, plain and perhaps rather elementary statement of the various therapeutic measures which I believe to be the soundest and best, and although I have not hesitated to quote from the work of others the personal point of view has been emphasized. It has seemed best to discuss the question of the operative treatment of the affection in a comprehensive way but not to attempt to write an operative surgery. In this section my personal experience is responsible for my comments on the relative value of the different operations. The manual is as free from technical terms as is possible in the discussion of a disease whose lesions involve the nervous system and the intricate functions of the body which the nervous system initiates, co-ordinates, and directs. The most recent publication on the subject from the Rockefeller Institute of Medical Research is a monograph said to be based on the study of 161 cases of acute poliomyelitis. This number of cases of poliomyelitis is little more than the average number of cases seen daily in the Infantile Paralysis Department of the Hospital for Deformities and Joint Diseases. We regard the daily attendance for treatment of 100 to 150 children the best testimonial from the parents of the benefit derived by their own children, and the children of others who fall under observation as they await treatment. The chapters on physical therapy and manipulative treatment arc written in plain language, that the parents of a case may read what is needed for a child, and why. The spontaneous improvement which occurs in a certain percentage of cases may be augmented, deformities prevented, and cases with serious disability remedied, by the reasonable methods of treatment here outlined. We have omitted reference to the obsolete and frequently misleading literature of the period which antedated the present pandemic of poliomyelitis. Being contained in their secretions, it is readily distributed by coughing, sneezing, kissing, and by means of fingers and articles con taminated with these secretions, as well as with the intestinal discharges. Moreover, as the virus is thrown off from the body mingled with the secre tions, it withstands for a long time even the highest summer temperatures, complete drying, and even the action of weak chemicals, such as glycerin and carbolic acid, which destroy ordinary bacteria. Hence mere drying of the secretions is no protec tion; on the contrary, as the dried secretions may be converted into dust which is breathed into the nose and throat, they become a potential source of infection. The survival of the virus in the secre tions is favored by weak daylight and darkness, and hindered by bright daylight and sunshine. It is readily destroyed by exposure to sunlight.
