Book excerpt: Historically, the treatment of multiple myeloma in transplant eligible patients has included induction chemotherapy followed by autologous stem cell transplant (ASCT) and a watch and wait approach until relapse. The introduction of maintenance chemotherapy has changed the traditional observation period into active treatment time. We examined the impact of lenalidomide maintenance on survival outcomes in the front line and relapsed setting. Our population included patients treated with bortezomib based induction followed by ASCT who went on to receive lenalidomide maintenance or no maintenance. Patient data was taken from the Myeloma Canada Research Network Canadian Multiple Myeloma Database (MCRN CMM-DB) which includes data from 13 academic cancer centers across Canada. Our data demonstrates that lenalidomide maintenance is associated with improved progression free (PFS) and overall survival (OS) as well as higher rates of favourable responses. We did not observe any unanticipated adverse effects. Rates of discontinuation for reasons other than relapse were low. Analysis of relapsed patients demonstrates that lenalidomide maintenance did not results in worse second PFS (time from second line therapy to second relapse, death or last follow-up) in patients who received the drug again in second line therapy. Lenalidomide maintenance was associated with improve overall survival from time of initiation of relapse therapy. Additionally, lenalidomide maintenance did not negatively impact second PFS, OS from time of initiation of relapse therapy, and rates of favourable response when lenalidomide used again in second line treatment. Our data is the first analysis of transplant eligible patients from the MCRN CMM-DB. Our data supports the continued use of lenalidomide maintenance as standard of care following ASCT. Importantly, our data suggests that the use of lenalidomide maintenance does not negatively impact survival or response outcomes when used again in second line therapy. As such, use of or relapse on lenalidomide maintenance should not be considered an exclusion criteria for use of lenalidomide in second line therapy.

Book excerpt: BACKGROUND: Patients with multiple myeloma (MM) who underwent autologous stem cell transplantation (ASCT), are known to benefit from maintenance therapy post-transplant. With signs of early relapse, physicians can either change maintenance therapy or use a new therapeutic regimen.OBJECTIVES: The purpose of this study is to analyze the clinical outcomes beyond biochemical relapse of patients with MM on maintenance therapy. Time to next treatment (TNT), progression free survival (PFS) and overall survival (OS) are assessed. Different approaches for relapse such as increase in lenalidomide (R) dose, addition of dexamethasone (D) or Bortezomib (V) or shift to different lines of treatment are possible.SETTING: A retrospective single center study approved by the institutional review board of the American University of Beirut Medical Center, Lebanon.METHODS: Between January 2011 and May 2017, 51 patients with MM who underwent ASCT, received Bortezomib/Lenalidomide/Dexamethasone (VRD) consolidation and maintenance therapy, mainly lenalidomide(R) 10mg/day for 21 days every 28 days, were included. RESULTS: The median age at transplant was 55 years (46-75). 46 (90%) patients received R maintenance, 3 patients received VRD maintenance for higher risk features. Median duration of R maintenance was 22 months (3-62). R dose was changed for toxicity in 17 (33%) patients (grade I-II). 29 (57%) patients relapsed. 13 (25%) patients changed treatment to different protocols. 6 patients (11%) were observed. 10 (20%) patients had increased lenalidomide dose with dexamethasone. 5 patients (31%) of these 2 groups required change of treatment later. The median follow up was 38 months (1-85). Median TNT was 32 months (7-62). At 2 years, the estimated PFS and OS were 39% and 97.5% respectively. The median OS and PFS2 (from therapy change) were 54 and 29 months for patients in the observation group versus 52 and 27 months in the R/D group as well as 45 months and 33 months with treatment change, respectively. No statistically significant difference was noted. CONCLUSION: Our small monocentric study is limited by its retrospective design and small sample size. However, it suggests that increasing lenalidomide dose/adding dexamethasone in selected patients can postpone change to different lines of treatment without affecting survival.

Written in alternating voices, Liza details her treatment, the complex decisions she had to make throughout her course of chemotherapy and radiation, including clinical trial participation and an elective double mastectomy, the added complexity of being treated at the cancer center of which John was the chief of hematology and oncology, and the emotional impact of knowing she may die as a young woman with young children. John, who lost his own mother to cancer at the age of 13, provides an inside look into the world of cancer care and research, but also the perspective of someone who understands the medicine but who was unprepared for assuming the role of caregiver and worried husband. John adds insights into his world of running the clinical operations of the cancer center where Liza would receive her care, commentary on the breast cancer machine, the need for clinical research, the high cost of cancer care, and an easy to understand explanation of the clinical and scientific background of oncology. While they both felt that they were already expert commentators on their own "Cancer Channel" during the course of Liza's illness, they both came to realize how little understanding they truly had of what a cancer diagnosis does to the patient, caregivers, children, family members, and friends.
