Book excerpt: Orthodontic attachments must be able to bond to a wide range of tooth and prosthetic surfaces. Despite the high prevalence of fluorosis in many parts of South Africa (Louw A, Chikte U 1997), only limited information is available on the integrity of the bond between orthodontic brackets and fluorosed teeth. The objective of this study was to measure and compare Shear Bond Strengths (SBSs) of metal and ceramic orthodontic brackets on fluorosed and non-fluorosed teeth. One hundred and twenty (60 fluorosed and 60 non-fluorosed) extracted premolar teeth were divided into four groups A to D, consisting of 30 teeth in each group. BluGloo® was used as an orthodontic adhesive to bond brackets on the buccal surface of each tooth. The experimental groups consisted of Group A, in which Nu-Edge® metal brackets were used and Group B, in which InspireIce® ceramic brackets were bonded to fluorosed teeth. Group C and D consisted of Nu-Edge® metal brackets and InspireIce® ceramic respectively, bonded to non-fluorosed teeth. Bonding techniques were kept the same and standardised for all four groups. An Instron testing device was used to debond and measure the SBSs. SBSs were compared using ANOVA with posthoc analysis done using Dunnett’s C test for pairwise comparisons. Significance was set at P0.05. The results showed that SBS of Group BGroup C>Group D>Group A. Ceramic brackets bonded to fluorosed teeth had the highest SBS with a mean of 15.78 (SD=9.07) Megapascals (MPa), while metal brackets bonded to fluorosed teeth produced the lowest SBS of 8.41 (SD=4.68) MPa. The SBSs of ceramic brackets bonded to fluorosed teeth was significantly higher than that of SBS of metal brackets bonded to fluorosed teeth, but not significantly different from SBSs obtained from either brackets bonded to non-flurosed teeth. The BluGloo adhesive if used to bond ceramic brackets to fluorosed teeth can produce adequate SBS for clinical use. The recommendation from this study is that ceramic brackets can be used efficiently to bond to fluorosed teeth. A follow up study should be carried out to assess the nature of enamel damage caused during debonding of flourosed teeth. This is a laboratory study and thus the clinical application should be interpreted with caution.

Book excerpt: The purposes were to evaluate the difference in SBS of bonding and rebonding orthodontic attachments when using a CEP compared to a SEP and to evaluate if re-etching is necessary prior to rebonding with either a CEP or a SEP. Orthodontic metal buttons were bonded to extracted human molars according to 4 protocols: In Group A, teeth were bonded with the CEP for the first and second bondings. In Group B, teeth were bonded with the CEP for the first bonding but re-etching was omitted for the second bonding. In Group C, teeth were bonded with the SEP for both bondings. In Group D, teeth were bonded with the SEP for the first bonding but re-etching was omitted for the second bonding. The SBSs when the SEP was used for the first bonding were significantly lower (P

Methods: 180 molars were randomly divided into 12 groups. Teeth were bonded with a stainless steel button using Transbond XT composite. The bonding agents were iBOND, G-Bond, Transbond Plus SEP and Transbond XT primer (control). SBS's immediately, 24hours and 3months were measured with their Adhesive Remnant Index (ARI). Results: Transbond XT primer (11.22MPa) had a significantly higher immediate SBS than Transbond Plus SEP (5.32MPa), iBOND (6.69MPa) and G-Bond (8.30MPa). 24 hours, Transbond XT primer (16.65MPa) and Transbond Plus SEP (13.20MPa) had a significant increase in their SBS. No difference was observed at 3 months. ARI were significantly different between the groups at all time points. Conclusions: iBOND and G-Bond yielded sufficient SBS's over the three time points that may be sufficient to withstand occlusal and light archwire forces.

Book excerpt: Demineralization of enamel has been one of the iatrogenic effects of fixed orthodontic therapy, for which various authors have suggested the use of fluoride to reduce the decalcification and development of white spot lesion. In the present study, the shear bond strength of light-cure composite adhesive, self-cure composite adhesive, light-cure glass ionomer cement and acidic primer were tested at the end of 24 hours after bonding. The results proved that the acidic primer system had highest shear bond strength while Light-cure glass ionomer system had the least shear bond strength. But the mean values met the minimal shear bond strength required for orthodontic bonding. After bond failure, least amount of glass ionomer cement was present on the enamel surface, followed by acidic primer system, light-cure composite and self-cure composite system. So, it is advantageous to use light-cure glass ionomer cement for bonding in terms of less amount of time spent and no enamel damage during cleaning procedure after debonding.

Book excerpt: Introduction: There is a growing demand for esthetic appliances during orthodontic treatment, with a specific interest in WildSmiles shaped brackets amongst children and adolescents. Bond failure of brackets over the course of orthodontic treatment can result in increased treatment time, increased overhead costs, and feelings of frustration for the patient and practitioner. Objective: The purpose of this study was to the evaluate the effect of orthodontic bracket base shape on shear bond strength (SBS) to human enamel, in vitro, at two time points, as well as the nature of debonding fractures using the Adhesive Remnant Index (ARI). Methods: Orthodontic brackets with different shaped bases (flower, heart, rectangular) were bonded to 120 extracted human third molars. SBS was measured using a Servohydraulic Test System at 24 hours and 2 months after initial bonding. ARI scores were evaluated under 10x magnification to assess the amount of resin left on the tooth. Results: The control bracket with a rectangular base shape had the highest mean SBS (26.78 ± 8.19 MPa), and significantly differed from flower (17.19 ± 4.45 MPa) and heart (18.86 ± 3.50 MPa) base shapes (p 0.001). The mean SBS of flower and heart shaped brackets did not differ considerably from each other (p 0.05). The mean SBS between debond times at 24 hours (21.50 ± 7.46 MPa) and 2 months (20.38 ± 6.69 MPa) were not statistically significant (p > 0.05). Analysis of ARI scores showed a significant difference between Flower-24 hours vs Heart- 2 months (p=0.039), Flower-24 hours vs Heart- 24 hours (p=0.004), and Control-2 months vs Heart-24 hours (p=0.015). The heart shaped bracket had a greater proportion of adhesive bond failures at the enamel-resin interface. Conclusion: Bracket base shape has an effect on SBS, with the rectangular base shape having a higher mean SBS compared to flower and heart base shapes. All bracket shapes had bond strengths above the clinically acceptable range of 6-8 MPa, and thus may provide adequate SBS in a clinical situation. Variations in ARI scores occurred based on bracket shape and were of a mixed adhesive-cohesive nature.
